This case-based presentation addresses the sometimes difficult transition of care for patients ages 15 to 39 to an adult-focused team, as well as the unique late effects of cancer treatment that can go undetected and/or unresolved. It presents the scenario of a young woman who was treated for Hodgkin Lymphoma at age 21 and then developed breast cancer at age 32.
