Both physios and athletic development coaches, Tracy Fober and Domhnaill Fox navigate through the world of rehab. Raising questions and starting a dialogue on how we work, problem solve and learn. Aiming to start a long and fruitful discussion series around all things Physio & the Art of Human Performance
This is a discussion about our exposure to and use of boxes, steps and blocks. We discuss where and how they were introduced to us, what we use them for and how we coach their use. We give an insight into the thought processes and decisions we make around progression and variation with boxes. We also talk about our own boxes which we both value so much as an essential part of our rehabilitation and athletic development toolbox.
This is a discussion about our exposure to and use the simple hurdle. We discuss where and how it was introduced to us, what we use it for and how we coach it. We give an insight into the thought processes and decisions we make around progression and variation with the hurdle. We also talk about our own hurdles which we both value so much as an essential part of our rehabilitation and athletic development toolbox.
This is a discussion about the origin and utility of the hex bar. We discuss why it was developed, what we use it for and how we coach it. We give an insight into the thought processes and decisions we make around progression and variation with the hex bar. We also talk about Tracy's Hexlite bar which we both value so much as an essential part of our rehabilitation and athletic development toolbox.
In this episode we interview one of the most thoughtful and progressive minds in applied sports and exercise science - John Kiely. John is most widely known for his papers questioning widely held beliefs around periodization. In this interview we look mostly at his other work including coordination, rehabilitation and stress. John gives us some brilliant insight into the complex science behind building relationships in rehabilitation and how coordination should be trained and relearned post-injury.
In this episode we discuss the term Performance Health and what it means to us across the continuum of physical development. We discuss how Performance Health looks in action while discussing it's art and science. We make a case for how we see it fit in the circumstances we each work in. We make regular reference to Xavier Roy's (our friend) recent article in SimpliFaster (https://simplifaster.com/articles/sports-academics-training-medical-school/) and Craig Pickering's recent series for HMMRMedia (http://www.hmmrmedia.com/pickering/).
In this episode we change the format to an interview. Our first guest Taylor Lorbiecki and co-host Donie Fox are those being interviewed by Tracy. Taylor is an Athletic Development coach working in the Air Force Academy, Colorado Springs. She has a unique perspective on sports participation and coaching based on her own sporting accolades. We discuss the real and perceived differences between male and female sports, and athletes. This episode is influenced heavily by Dr. Sophia Nimphius recent presentation on the topic - https://youtu.be/iq6zGfD9s-Y. As coaches of female teams both Donie and Taylor describe their approaches and how it has evolved from their own competing days. We discuss how athletic preparation differs between male and female sports and how that is only sometimes appropriate. We dive into the culture around the provision of facilities and funding for female sports and how that creates more of a disadvantage than perceived and generalised differences between males and females.
In this episode we discuss the demands of running and how we approach rehab in order to deal with these demands. We talk through our principles of progression in reintroducing running to a patient/athlete. We make a point of delineating terms related to this process and how jogging just doesn't cut it. We hesitate to offer session plans because we acknowledge this is a very individual process that requires care and attention from the rehab professional. We discuss the skill of running and how our foundational rehab builds the necessary tools to bridge the gap safely and progressively. In this episode we drove into how we create and restore trainability using Vern Gambetta's foundational legs programme. We start off by describing the barriers physios may face to using these methods. From there we add to the content of our articles by discussing some of the intricacies of the programme and how our thinking and understanding of it has evolved. We break down the loading on a squat and the options lunging gives us in rehab. We reference these two articles heavily in this episode, have them to hand while listening to get the most from the episode (http://ironmaven.blogspot.com/2020/04/foundational-legs-reflections-before.html?m=1 and http://www.hmmrmedia.com/2020/04/using-foundational-leg-exercises-as-the-cornerstone-of-rehabilitation/ . And look out for Donie's latest article coming out this week on HMMR media which looks at a "down the rabbithole" analysis of lunging progressions. In this episode we discuss an issue that we are very passionate about. We dive into the early rehabilitation process after knee surgery. We disembowel some sacred cows and offer up real world solutions that we use with all post-op knees. We talk about how we use gait as the starting point for the rehab process in general and how we take it way beyond the classic term of "gait retraining". We discuss barefoot sessions, proprioception and balance. We offer up ideas about progression from non weight bearing to independent walking. Our conversation generally circles around how to get a knee to flex and extend naturally again. In order to make this an audio and visual learning experience we have shared some of these progressions and ideas in a Google photos folder for you here - check each videos description for more info (https://photos.app.goo.gl/SiJtG3P4YbeAEjcXA). In this episode we discuss the skill of being able to appropriately progress training and rehab in the outdoors. With our current situation regarding Coronavirus we are now without our facilities and indoor training equipment for the foreseeable future. How do we ensure our quality of care does not suffer? We pay particular attention to staying true to your philosophy and not just taking the easy option of making people tired for the sake of it. We discuss careful planning and implementation of training using fields, hills and stairs. We mention means of progression and regression, and we try to paint a picture of how we run these sessions with our athletes and those we work with. In order to make this an audio and visual learning experience we have shared some of these progressions and ideas in a Google photos folder for you here - check each videos description for more info (https://photos.app.goo.gl/4UqvXhymZLB3wsPR8). In this episode we discuss some long held beliefs and traditions around strength training in sport. We attempt to make sense of the disconnect between sporting endeavours and weight room strength. Woven through the discussion is how we address strength development in athletic development and rehab, and how we have taken traditional strength training and modified it to the needs of those we are working with. In this episode we talk about the premium we place on having time and space to work with. We contrast traditional physio with a rehabilitation set up where the use of time and space is optimised for the experience of the person who is undergoing rehabilitation. We offer some insight in to how we navigate issues where time and space is in short supply. Tracy gives some great advice on how those working in clinics with a traditional layout can develop their skillset and bring a world class approach to the most conservative clinical spaces. In this episode we talk about our rehab concepts and how or methods were informed. We discuss those people who are responsible for major conceptual shifts in our thought processes. The conversation meanders between movement, education, progressions and gadgets. In part 2 of this episode we apply these principles and concepts to a case recently encountered in the clinic. We discuss various aspects of how that session transpired and the differences in our approaches. We finish by talking about the role of pain in movement and how gadgets can affect a patient's perspective.
