On June 25, the day after the Supreme Court overturned Roe v. Wade, the White House advertised an online Health and Human Services flier containing a link to AbortionFinder.org, a network that connects women wanting abortions to local facilities that will perform them and provides information on the legality of abortion in their state. The website has a search engine to find abortionists based on the seeker’s location and age, including for minors “15 or younger.” AbortionFinder.org also includes a link to the If/When/How Judicial Bypass (JB) Helpline, which is designed to help minors bypass their parents or guardians to obtain an abortion. This Judicial Bypass system cuts 14-16-year-olds off from their support systems, isolating them and allowing them to kill their children in utero and put their bodies through the trauma of abortion with no familial protection. There’s a picture in the article, and you can find the article in our show notes by the way… Despite studies showing that women who go through abortions suffer increased mental health issues than women who decide to give birth to their children, Biden and his administration are capitalizing on the outrage sparked by the overturning of Roe to target American youth. After the Dobbs decision, the Department of Health and Human Services immediately published a website, “ReproductiveRights.gov,” for the express purpose of providing “accurate and up-to-date information about access to and coverage of reproductive health care and resources.” This page — which is itself a misnomer since abortion is neither health care nor a right — though branding itself as full-range coverage of the choices available to women, made no reference at all to a woman’s choice to keep her baby and instead pandered almost entirely to abortion-related services. Environmental Protection Agency. The 6-3 ruling sides with West Virginia, which sought to limit the Clean Air Act. At issue were the EPA's plans to use regulation to "reduce carbon pollution mostly by moving production to cleaner sources." Once the EPA determined this course of action, they began to limit allowable emissions from fossil fuel production. They enacted goals that they viewed as a "'reasonable' amount of shift which it based on modeling how much more electricity both natural gas and renewable sources could supply without causing undue cost increases or reducing the overall power supply." Chief Justice Roberts wrote in his opinion: "Since passage of the Act 50 years ago, EPA has exercised this authority by setting performance standards based on measures that would reduce pollution by causing plants to operate more cleanly. In 2015, however, EPA issued a new rule concluding that the 'best system of emission reduction' for existing coal-fired power plants included a requirement that such facilities reduce their own production of electricity, or subsidize increased generation by natural gas, wind, or solar sources." This meant that the EPA was effectively regulating coal businesses to wind down toward the end goal of no longer being in the business of providing energy. Roberts asked the question as to "whether this broader conception of EPA’s authority is within the power granted to it by the Clean Air Act." Whoops! The Phoenix Suns and Miami Heat are among the teams Durant has on his wish list for a potential trade, sources told Wojnarowski. However, the Nets are not tied to honoring any of Durant's preferred destinations and plan to make a deal that allows them the greatest return of assets, sources told Wojnarowski. There’s also rumor that Kyrie Irving wants to try and move with KD to his new team. Irving eventually opted into the final year of his contract after being unable to find a suitable sign-and-trade for the Nets to make from his preferred list of destinations -- one that included the Los Angeles Lakers, LA Clippers, New York Knicks, Heat, Dallas Mavericks and Philadelphia 76ers. Of those teams, however, only the Lakers were interested in giving Irving a max contract, sources told Wojnarowski -- and they didn't have any package to offer the Nets that was appealing enough to get a sign-and-trade deal done. According to a source, that process is underway. Big Ten presidents and chancellors will meet to vote on the addition of USC & UCLA, sources told ESPN. Multiple sources told ESPN on Thursday that the move is expected to happen. A source called the next steps "formalities.” The reason this move would be less disruptive than potential moves in the ACC is that USC and UCLA have a grant of rights tied to the current Pac-12 television contract, which expires after the 2023 football season and 2023-24 school year. That's why the expectation is that both schools can go to the league for the 2024-25 season and not suffer any financial penalty. Pac-12 officials had been nudging both Los Angeles schools for years to extend the grant of rights. The fact that they didn't hinted that they had greater ambition. "We just got Sooner'd and Horn'd," a high-ranking university official at one of the Pac-12 schools told ESPN, referring to the decision made last year by Texas and Oklahoma to leave the Big 12 conference for the SEC.     At the center of the Supreme Court's decision to overturn Roe v. Wade is the Mississippi case Dobbs v. Jackson Women's Health Organization. In 2018, Mississippi passed a 15-week abortion ban. The abortion clinic Jackson Women's Health challenged the law. “When I took office, that case had been sitting at the Fifth Circuit and it needed to be appealed,” Mississippi Attorney General Lynn Fitch said. “We looked at it and said, absolutely, we wanted to appeal this case to the United States Supreme Court.” Fitch joins “The Daily Signal Podcast” to talk about Mississippi's role in overturning Roe. However, anticoagulant activities of curcumin have not been studied.  The anticoagulant properties of curcumin and its derivative (bisdemethoxycurcumin, BDMC) were determined by monitoring activated partial thromboplastin time (aPTT), prothrombin time (PT) as well as cell-based thrombin and activated factor X (FXa) generation activities.  Data showed that curcumin and BDMC prolonged aPTT and PT significantly and inhibited thrombin and FXa activities. They inhibited the generation of thrombin or FXa. In accordance with these anticoagulant activities, curcumin and BDMC showed anticoagulant effect in vivo.  Surprisingly, these anticoagulant effects of curcumin were better than those of BDMC indicating that methoxy group in curcumin positively regulated anticoagulant function of curcumin. Therefore, these results suggest that curcumin and BDMC possess antithrombotic activities and daily consumption of the curry spice turmeric might help maintain anticoagulant status.   Probiotics may prevent breast cancer: Study Western University (Ontario), June 27, 2022   A new study has found probiotics may prove to be a critical factor in preventing breast cancer.   Dr Gregor Reid, the professor of microbiology, immunology and surgery at the Western University in Ontario, Canada, said the bacteria having the potential to abet breast cancer are present in the breasts of cancer patients, while beneficial bacteria are more abundant in healthy breasts.   In the study, Reid's PhD student Camilla Urbaniak obtained breast tissues from 58 women, who were undergoing lumpectomies or mastectomies for either benign (13 women) or cancerous (45 women) tumors as well as from 23 healthy women, who had undergone breast reductions or enhancements. Researchers found that women with breast cancer had elevated levels of Escherichia coli and Staphylococcus epidermidis, two bacteria known to induce double-stranded breaks in DNA in HeLa cells, which are cultured human cells. Their goal: to partake in a brewed concoction made from a vine plant Banisteriopsis caapi, traditionally used by indigenous people for sacred religious ceremonies. Drinkers of Ayahuasca experience short-term hallucinogenic episodes many describe as life-changing. The active ingredient responsible for these psychedelic visions is a molecule called dimethyltryptamine (DMT). For the first time, a team led by Michigan Medicine has discovered the widespread presence of naturally-occurring DMT in the mammalian brain. The finding is the first step toward studying DMT-- and figuring out its role -- within the brains of humans. "DMT is not just in plants, but also can be detected in mammals," says Jimo Borjigin, Ph.D., of the Department of Molecular and Integrative Physiology. Her interest in DMT came about accidentally. Before studying the psychedelic, her research focused on melatonin production in the pineal gland. In the seventeenth century, the philosopher Rene Descartes claimed that the pineal gland, a small pinecone-shaped organ located deep in the center of the brain, was the seat of the soul. Since its discovery, the pineal gland, known by some as the third eye, has been shrouded in mystery. Scientists now know it controls the production of melatonin, playing an important role in modulating circadian rhythms, or the body's internal clock.  The core idea seems to come from a documentary featuring the work of researcher Rick Strassman, Ph.D. with the University of New Mexico School of Medicine. In the mid-1990s, he conducted an experiment in which human subjects were given DMT by IV injection and interviewed after its effects wore off. In a documentary about the experiment, Strassman claims that he believed the pineal gland makes and secretes DMT. Borjigin sought to discover how and where DMT was synthesized.  They found DMT in other parts of the brain, including the neocortex and hippocampus that are important for higher-order brain functions including learning and memory." A paper published in 2018 by researchers in the U.K. purported that DMT simulates the near death experience, wherein people report the sensation of transcending their bodies and entering another realm.    Puffing curcumin may blast Alzheimer's Vanderbilt University, Jun 22, 2022   The new delivery method may be more effective than others in getting the compound past the blood-brain barrier and into the brain, where it can fight the plaque that leads to Alzheimer's.   Deep breaths of curcumin may be key to fighting Alzheimer's disease, according to a new study from Vanderbilt University. Curcumin, a compound in the spice turmeric, has a demonstrated ability to smash the plaques in the brain that lead to the neuron loss that causes Alzheimer's, according to the study's senior author,Wellington Pham, Ph.D The challenge, however, has been getting the curcumin into the brain. Pham and colleagues at Shiga University of Medical Science in Otsu, Japan, developed a new delivery strategy. They created a curcumin moleculte that could be tracked with an MRI, to be administered as an aerosol through a nebulizer. This method delivers the dose more directly to the brain than taking the compound orally and digesting it. After tests in mice, the team found that “delivery to the cortex and hippocampal areas is more efficient using aerosolized curcumin than intervenous injection in a transgenic mouse model of Alzheimer's disease,” said Pham.   Anti-anxiety medication limits empathetic behavior in rats University of Chicago, June 27, 2022   Rats given midazolam, an anti-anxiety medication, were less likely to free trapped companions because the drug lessened their empathy, according to a new study by University of Chicago neuroscientists.   The research, published in the journal Frontiers in Psychology, validates studies that show rats are emotionally motivated to help other rats in distress. In the latest study, rats treated with midazolam did not open the door to a restrainer device containing a trapped rat, although control rats routinely freed their trapped companions. Midazolam did not interfere with the rats' physical ability to open the restrainer door, however. In fact, when the restrainer device contained chocolate instead of a trapped rat, the test rats routinely opened the door. The findings show that the act of helping others depends on emotional reactions, which are dampened by the anti-anxiety medication.   "The rats help each other because they care," said Peggy Mason, PhD, professor of neurobiology at the University of Chicago. Even though she was a pastor in a wonderful ministry and she has an amazing husband that's very supportive of her. Eventually, she had broken down at the office and started thinking about things a lot. This helped her think of a goal that will change her life. She started to establish a habit of drinking water. Using a six-ounce cup that she fills multiple times a day. She also started cooking more at home for her and her husband's meals. So she can control the nutritional value of what they eat more. Eventually, she started doing some physical activity too. Starting by walking to the end of the street. She persevered through it even though she thought she felt she was going to die doing it. Eventually, she got what she wanted and became healthier and actually happy with herself. And now she helps out those in need of assistance with their own health problems.     I wanted to remind everyone about this show from last year because it's becoming increasingly relevant. We have this weird thing going on where everybody seems to be talking about physician incentives and payments and financial implications but so often disregards patient incentives and payments and financial implications. Consider that we're at a place in the time-space continuum where it is inarguable that financial toxicity has become clinical toxicity. Patients are increasingly in huge numbers abandoning care, splitting pills, doing all kinds of things to save money that are clinically toxic. And these are patients with “good insurance” that we are talking about here. So, here's a role play: Provider organization is actually paying doctors for outcomes. In wanders a patient with a huge deductible. Doc says, “Wow, Patient … so important that you take your insulin or med as directed or get a follow-up on that scary colonoscopy finding.” Patient says, “Sorry, Doc. Can't afford it.” And the doc gets dinged because the patient outcomes are avoidably poor. That's what this show with Dr. Mark Fendrick digs into: aligning patient incentives (aka benefit designs etc) with value-based payments on the provider side. And with that, here's your encore: And here I thought I knew a lot about value-based care. In this healthcare podcast, I am speaking with Mark Fendrick, MD, who is the director over at the University of Michigan Center for Value-Based Insurance Design. This conversation is for those of you who already know pretty much about value-based care concepts. If you do not, I'd go back and listen to, say, Encore! EP206, with Ashok Subramanian, before this one.   Dr. Fendrick talks in this healthcare podcast about what it takes for value-based care to happen in the real world. No kidding, it's about making sure that reimbursement is aligned with good things (no great surprise there). But two light bulb moments I had in this conversation with Dr. Fendrick: At the beginning of the year, how many doctors and nurses, inspired to do the right thing, have told their patients with diabetes, say, to go get an eye exam to check for diabetic retinopathy? No one would disagree that this is definitely a good idea. Diabetic retinopathy causes blindness. But here's the reality of that conversation. Doc says, “Go get an eye exam.” And patient says, “I can't. My deductible is huge, and I can't afford it.” So, the patient doesn't get the follow-up care and winds up in the hospital or blind. And the doctor gets dinged on his or her quality scores. Suboptimal outcomes all around, I'd say. This also happens on the pharmacy side of the equation, but I think a lot of us are a little bit more familiar with that scenario—like type 1 diabetics who can't afford to pick up their insulin because of a Medicare Part D or commercial deductible that they haven't met yet. I just never really connected the dots back to the provider getting black marks because their patient has a benefit design that's not aligned with the quality measures. In a majority of benefit designs, consumer price sharing is based not on the value of the service but on how expensive the service just happens to be. Wow! So, we're trying to get our plan members to be consumers and use the power of their wallets to make good healthcare choices. And what we're really doing is driving them toward cheap things or no care and discouraging them from indulging—and I say that sarcastically—in expensive things. But the expensive things might be the high-value care, and the relatively cheap things might be crap that's fully unnecessary or harmful and, over a whole population, adds up to a lot of zeros. Healthcare is not like a consumer market where the expensive things are usually a better version of the cheap things. For all you economists out there, you don't want the demand curve to be elastic when what's cheap and what's expensive has no correlation to quality or necessity. Nobody should be super flabbergasted when a $35 cure-all supplement peddled on YouTube makes some random influencer a millionaire. That's how supply and demand works. Much to ponder in this episode. Mark Fendrick, MD, is a professor of internal medicine in the School of Medicine and a professor of health management and policy in the School of Public Health at the University of Michigan. Dr. Fendrick received a bachelor's degree in economics and chemistry from the University of Pennsylvania and his medical degree from Harvard Medical School. He completed his residency in internal medicine at the University of Pennsylvania, where he was a fellow in the Robert Wood Johnson Foundation Clinical Scholars Program. Dr. Fendrick conceptualized and coined the term Value-Based Insurance Design (V-BID) and currently directs the V-BID Center at the University of Michigan (vbidcenter.org), the leading advocate for development, implementation, and evaluation of innovative health benefit plans. His research focuses on how clinician payment and consumer engagement initiatives impact access to care, quality of care, and healthcare costs. Dr. Fendrick has authored over 250 articles and book chapters and has received numerous awards for the creation and implementation of value-based insurance design.
