I thrive in having schedules, projects, and plans. These bring me a strange sense of peace. I know if I am checking off the items associated with those then I am achieving my goals. But I also know that schedules, projects, and plans go awry. That's why I have found it helpful to have a back-up plan. Or, as a mentor of mine says, being kind to My Future Self. This can come in all fashions like preparing coffee the night before or filling up my car with gasoline on my way home so I don't have to do it on my way out when I am inevitably in a rush. Why I can't leave the house 45 seconds earlier is beyond me but I feel like I am always cutting it close to the wire.As I think about what the coming year looks like, I know I feel the greatest sense of peace when I don't have unexpected setbacks. I don't deal with interruptions well ( just ask my family when I am trying to write) and I can easily become discombobulated when something goes off schedule.That's why I take a little extra time to be kind to My Future Self. Advance care planning is a term used to describe when a person prepares for future management of serious or terminal illness, including developing an advance care directive or what is sometimes is called a living will.Beginning in January of 2016, Medicare made it possible for certain clinicians to bill for their work for patients to develop advance care plans.Despite the new billing option, uptake has been quite slow.In health care, we often use financial incentives to motivate behavior change. You might have expected that simply creating a payment option for advance care planning would make it happen.Keren Ladin from Tufts University joins Health Affairs Editor-in-Chief Alan Weil on A Health Podyssey to discuss the reasons Medicare's payment policy has not led to the greater pursuit of advance care planning.Ladin and coauthors published a paper in the January 2022 issue of Health Affairs examining the limited use of advance care planning billing codes among clinicians. A World War II Era underground storage tank containing over 100 million gallons of jet fuel at the Red Hill Fuel Facility has been leaking into the aquifer that provides water to the people of Oahu. Until recently, the Navy denied there were leaks or that the water was unsafe despite multiple complaints of health problems and an oil sheen found in the tap water. Now, the Oahu Water Protectors have gained the Navy's attention and that of local and state elected officials. Wayne Tanaka of the Sierra Club of Hawaii speaks with Clearing the FOG about the fuel facility, what is at stake and how they are organizing to hold the US Navy accountable and shut the facility down.
