In this episode of the Market Pathways Podcast, Stephen Levin talks with medtech entrepreneur/innovator/investor Josh Makower, MD. Makower recently returned to run the Stanford Biodesign program following the retirement of Paul Yock, MD, with whom he co-founded the program. Makower is looking to enhance the pre-eminent Stanford program by adding a focus on medical device regulatory, reimbursement and policy issues, which today are critical for a successful medtech entrepreneur to understand. Makower talks here about the initial efforts Stanford Biodesign has launched to get the policy ball rolling, beginning with reimbursement, which has replaced regulatory as the biggest problem currently facing medtech companies. Kicking off this effort is a paper published recently in the Health Management Policy & Innovation Journal that makes the data-driven case for a need both on the part of patients and the innovation ecosystem for accelerated Medicare coverage of innovative technologies. Frederick St Goar, MD Dr. Frederick St. Goar is a cardiology specialist, physician inventor, and entrepreneur. He currently serves as vice chairman of the board of the Fogarty Institute in Mountain View, CA. His career was jump-started at Stanford when he joined Paul Yock in working on intravascular ultrasound imaging. Subsequently he became involved in launching Heartport. He invented the minimally invasive MitraClip system, co-founding Evalve. He consults for several established medical device companies and is active in the Stanford Biodesign program as well as the Fogarty Institute. Dr. St. Goar has over 36 years of experience in the field; he graduated from Harvard Medical School and completed a residency at Stanford University Medical Center. He currently practices at Advanced Cardiovascular Specialists and is affiliated with medical facilities such as El Camino Hospital Mountain View Campus and Mills-Peninsula Medical Center.  

When Stanford's Paul Yock was a young interventional cardiologist, he was frustrated by the complex, two-person process required to deliver life-saving stents. So, he invented a better way. His Rapid Exchange stenting and balloon angioplasty system, one of several inventions Yock is known for internationally, can be managed by just a single operator, making procedures like stent placement faster and safer. Yock is a man of many talents. He is a doctor, a professor of bioengineering and of mechanical engineering, and an innovator who combines these many interests to solve problems in healthcare. He says that the aha moments work beautifully when they come, but they are too often based in luck. For the rest of us, there is a better and more consistent path to transformative medical tools he calls the biodesign process. It starts, first, with a deep understanding of the medical need. The next step is brainstorming and iteration, preferably by non-hierarchical, multidisciplinary teams of doctors, engineers and business people. Diversity of insights and expertise leads to better inventions, Yock says.
