Michael S. Chen graduated with a degree in investment banking. He worked at Hambrecht Quest, a smaller investment bank focusing on high tech and healthcare companies in New York City. After two years in investment banking, he decided to pursue medicine, inspired by his father's career as a cardiologist. He then went on to study internal medicine at UCSF, Cleveland Clinic, and then joined cardiac associates in Gaithersburg, Maryland. He has been with the practice for 18 years and is now the Chair of Cardiology at  Shady Grove Medical Center, and Medical Director of the Cardiac Catheterization Laboratory at White Oak Medical Center. Challenges Faced by Cardiac Catheterization Labs Michael discusses the challenges faced by cardiac catheterization labs in meeting performance measures and ensuring proper procedures. He highlights the need to balance equipment requests with limited budgets and prioritize patient care. He talks about the importance of cardiologist performance in heart attack patients, focusing on door to balloon time and other metrics like aspirin and blood thinners. He mentions the importance of assessing the overall performance of the hospital as a whole, focusing on reducing time spent on administrative tasks and ensuring patients are on the right medications.The conversation turns to steps patients can take to ensure they are prescribed the right medication. Interventional Cardiology Explained Michael goes on to explain what interventional cardiology is, what the new trends are, and he addresses common misconceptions. He discusses the learning process for new techniques and tools, such as fellowships and medical device reps. Michael notes that fellowships are the most effective method for learning new techniques, but it's important to stay updated and stay updated with medical device reps. Other methods include working with other doctors, attending training sessions, or learning online through YouTube videos. There are various ways to learn and improve in this field. Physicians and The Business of Healthcare Michael discusses the business of healthcare and the factors driving physicians to work with larger healthcare companies. He explains that larger practices have more bargaining power and can offer lower rates and financial security. However, there are drawbacks, such as the need to balance autonomy with financial security. Michael talks about the increasing trend of private equity investing in physician groups, such as anesthesia and cardiology. Private equity is seen as a way to ensure return on investment, which can sometimes be more expensive than providing the best care possible. Stressors for cardiologists include cash flow, meeting payroll, and meeting electronic medical records and government regulations. However, overall, the fit has been good and cardiologists have been happy with the situation.  Participating in the National Senior Games The conversation turns to fitness and sports. Michael keeps active in terms of exercise, running and working out. He recalls running a 5k five years ago and receiving a gift certificate from Panera for winning first place in his age division. He then qualified for the National Senior Games, (open to anybody 50 years and older) which features various events such as a 5k 10k, pickle ball, tennis, track and field, ping pong, basketball, and softball. Michael has competed in the National Senior Games in Fort Lauderdale and Pittsburgh. His mom competed in the 5K powerwalk and Michael competed in the 5K and 10 K road race. Michael came in 2nd in the 10K (and won a silver medal!) and 4th in the 5K in Pittsburgh in 2023, His mother placed 8th in the 5K Power walk in Fort Lauderdale in 2022. Michale mentions that he has a 5k coming up on Labor Day week, with his son running it with friends, and the National Senior Games.   Influential Harvard Professors and Courses Michael mentions Richard Taylor and TA Tom Roberts, and studying bipeds and quadruped to compare the energetics and mechanics. Part of the study included placing turkeys and chickens on a treadmill. They found that turkeys were quite stubborn and not smart, as they were not smart enough to run on a treadmill. They learned that turkeys were efficient at running and that their locomotion mechanics were efficient. They were fortunate to receive a Hoops Prize for their work, which ends up in the Lamont library. David Cohen is one of the smartest docs on Twitter. I learned a bunch talking with him. The procedure called transcatheter aortic valve implantation or TAVR is a damn miracle. In days of old, a heart surgeon would have to saw open the chest and cut out the heavily calcified immobile aortic valve and sew in a new one. I watched a case as a young doctor and came away shocked that patients survive this surgery. TAVR is even more stunning. Doctors place a valve up the aorta, across the diseased valve, and then place the new valve into the old valve. The verbs squishing or smooshing come to mind. The other unbelievable thing about TAVR is that strokes are less common than you'd think. When I first heard about TAVR, I thought: how is it not limited by all that debris going into the brain? Well, there is less debris than I would have thought. But not zero debris. In fact, there is one device on the market that forms a barrier between the aorta and the brain. We call it an embolic protection device (EPD) or cerebral embolic protection (CEP).Early studies show that the device catches debris that would have occluded blood vessels in the brain—iow, caused stroke. The pictures almost sell the device—because, obviously, catching debris has to be beneficial. But. But. There are always ‘but's' in Medicine. The PROTECTED TAVR trial, which compared TAVR with and without an embolic protection device failed to show a statistically significant reduction in stroke. It was a good trial, but it did not close the door for the device. For two reasons: one was that the trial was underpowered. The lower bound of the 95% confidence interval allowed for a 1.7% lower rate of stroke in the treatment arm. Neurologists feel that a 1% risk reduction in stroke is clinically important. The other reason was that a secondary endpoint of “disabling” stroke was 60% lower with the device. We needed more data. Another trial is not likely going to happen. Trials are expensive and take a long time. This is where Dr Cohen's group comes in. They performed an observational study looking at more than 400k patients in a TAVR registry. About 13% got the device and 87% did not. This is where Sensible Medicine readers should start feeling a rash. Why? Because you know how scary it is to try and compare outcomes in two groups of patients who were not randomized. Cohen, however, tells me about a super-interesting way to approximate randomization in this comparison. It's called an instrumental variable analysis. He explains this to me in clear terms during our conversation. I love methods so I was enthralled. But that isn't all. The other thing is that his study, like the PROTECTED TAVR trial, came up with tantalizing close results. We discuss that as well. I loved our talk. In this episode, host Dr. Aaron Fritts interviews Dr. Peter Soukas taking a deep dive into novel balloon technologies, appropriate uses below the knee, and how these new balloons are highly effective in treating patients with critical limb ischemia (CLI). Dr. Soukas explains how these new balloon technologies can minimize the risk of dissections (therefore decreasing the need for bailout stents), create effective lumen gain in concentric and eccentric calcified lesions with minimal recoil, and keep pressures low compared to legacy products. --- CHECK OUT OUR SPONSOR Cagent Vascular Serranator https://www.cagentvascular.com --- SHOW NOTES Dr. Soukas is an Interventional Cardiologist who is the Founder and Director of the Brown Vascular and Endovascular Medicine Fellowship program, serves as the Director of the Interventional PV Lab at the Lifespan Cardiovascular Institute of Brown, and an Associate Professor of Medicine at the Warren Alpert School of Medicine. We begin by discussing the treatment of CLI, particularly with new below the knee balloon angioplasty devices like the Cagent Serranator and how balloon tech has evolved over time. These new technologies allow for 1000x more force than previous balloon models through unique serration technology at significantly lesser pressures, minimizing the risk of barotrauma and iatrogenic lumen dissections, while allowing for effective luminal gain, and showing success in treating CLI even when calcified lesions are present. What's more is that there is now a variety of serration balloon lengths available, which was definitely a huge shortcoming in prior scoring balloons with limited sizing. While IVL is the preferred option in terms of treating concentric (360°) calcified lesions, new serration balloons are cheaper and show success in treating both concentric and eccentric calcified lesions with minimal recoil. Dr. Soukas and Dr. Fritts also go on to discuss how using IVUS is critical in visualizing the size, shape, and depth of possible calcifications but also important in picking the correctly sized serration-balloon to get the job done. Dr. Soukas also explains how the serration balloon technology is easily deployable, tracks very well within vasculature, and can even be used below the ankle if needed (with some pre-dilation of the lumen) stating that if the IVUS can fit, usually so can the serration balloon. Show notes - Genetic Counseling & Family Screening in Arrhythmogenic Cardiomyopathies Notes (developed by Dr. Anjali Wagle)   What is the underlying pathophysiology of arrhythmogenic RV cardiomyopathy (ARVC)?  Fibrofatty replacement cardiac myocytes  Associated with genetically mediated disruption of desmosomal proteins   This leads to thinning and weakness of the heart that can lead to aneurysms and progressive dilatation and failure of the right ventricle (RV)  How is ARVC diagnosed?  2010 taskforce criteria (Marcus et al, 2010):    RV structural abnormalities including findings seen on echocardiogram, MRI, and RV angiography  Pathological criteria  Repolarization abnormalities   Depolarization/conduction abnormalities   Ventricular arrhythmias   Genetics and/or family history   How does ARVC present?   Young, healthy individual will have symptoms of arrhythmias (syncope, pre-syncope, SCD) or heart failure  Family screening   What are the inheritance and genetic factors of ARVC?  Autosomal dominant pattern  Low penetrance and variable expressivity   Half of patients who are index cases will be found to have a mutation in the desmosomal gene.   What are the most common mutations associated with ARVC?  Most commonly the genes involved are plakophilin-2 (PKP-2) and desmoplakin.   For PKP-2 the most common mutations are truncating mutations.   In patients who have inherited two truncating mutations, this will result in neonatal lethality.   Is there a difference in the genetic factors of left and right arrhythmogenic cardiomyopathy?   ACM is disproportionally a right dominated cardiomyopathy. Left dominated cardiomyopathy has a different genetic profile.   Pathogenic variants in desmoplakin disproportionally cause biventricular forms of ACM or left dominated forms.
