Achilles Tendonitis - Episode 3 Series 2 Podiatry Detectives

Achilles tendinitis is a common condition that occurs when the large tendon that runs down the back of your lower leg becomes irritated and inflamed. The Achilles tendon is the largest tendon in the body. It connects your calf muscles to your heel bone and is used when you walk, run, climb stairs, jump, and stand on your tip toes. Although the Achilles tendon can withstand great stresses from running and jumping, it is also prone to tendinitis, a condition associated with overuse and degeneration. Simply defined, tendinitis is inflammation of a tendon. Inflammation is the body's natural response to injury or disease, and often causes swelling, pain, or irritation. There are two types of Achilles tendinitis, based upon which part of the tendon is inflamed. In noninsertional Achilles tendinitis, fibers in the middle portion of the tendon have begun to break down with tiny tears (degenerate), swell, and thicken. Tendinitis of the middle portion of the tendon more commonly affects younger, active people. Insertional Achilles tendinitis involves the lower portion of the heel, where the tendon attaches (inserts) to the heel bone. In both noninsertional and insertional Achilles tendinitis, damaged tendon fibers may also calcify (harden). Bone spurs (extra bone growth) often form with insertional Achilles tendinitis. Tendinitis that affects the insertion of the tendon can occur at any time, even in patients who are not active. More often than not, however, it comes from years of overuse (long distance runners, sprinters).

The sinus tarsi is a tube or tunnel between the talus and the calcaneus bones. Sinus tarsi syndrome is pain or injury to this area. Traumatic injury to the ankle/foot (such as an ankle sprain) or overuse (such as repetitive standing or walking) are the main causes of this syndrome. It may also occur if the person has a pes planus or an over-pronated foot, which can cause compression in the sinus tarsi. Some characteristics are pain at the anterior lateral side of the ankle (anterior to the lateral mallelous) and a feeling of instability or difficulty walking on unstable surfaces.

Everyone can get skin cancer regardless of your ethnicity and skin tone. How many people use sun screen on the tops of feet and ankles? Most skin cancers are associated with ultraviolet (UV) radiation from the sun or tanning beds, and many people of color are less susceptible to UV damage thanks to the greater amounts of melanin (the protective pigment that gives skin and eyes their color) darker skin produces. But people of color can still develop skin cancer from UV damage. Additionally, certain skin cancers are caused by factors other than UV — such as genetics or other environmental influences — and may occur on parts of the body rarely exposed to the sun. For example, darker-skinned people are more susceptible to acral lentiginous melanoma (ALM), an especially virulent form of melanoma (the deadliest type of skin cancer) that typically appears on the palms of the hands and soles of the feet. Different ethnicities are at higher risk for particular skin malignancies: Latinos, Chinese, and Japanese Asians tend to develop basal cell carcinoma (BCC), the most common skin cancer. But the second most common, squamous cell carcinoma (SCC), is more frequent among African Americans and South Asians.
