    2025 ha sido un año importante en la política internacional. Ha estado marcado por el regreso de Donald Trump a la Casa Blanca. Su hiperactividad y una presencia constante en los medios, han dominado el año. Varios países han ganado y otros han perdido. Entre los primeros está China que, contra todo pronóstico, ha sido uno de los mayores beneficiados. Trump, consciente de los altos costes de una guerra comercial, firmó en noviembre una tregua que suavizó las restricciones chinas a las tierras raras. Esta distensión ha permitido a Xi Jinping centrarse en los problemas internos y consolidar a China como una superpotencia económica equivalente a EEUU. En Oriente Medio Arabia Saudita se ha convertido en el socio indispensable de Trump en Oriente Medio. Esto posiciona al país como el principal contrapeso a Irán y cabeza del mundo árabe. No muy lejos Israel se ha subido también al carro de los vencedores gracias a victorias como la destrucción del programa nuclear iraní y el final de la guerra de Gaza que ha dejado a Hezbolá y a Hamás muy debilitadas. Netanyahu termina el año con Israel en su posición más fuerte en varias décadas. En Sudamérica la Argentina de Javier Milei no sólo se está recuperando poco a poco de la crisis económica que sufre desde hace años, es que se ha convertido en el aliado preferencial de EEUU en Hispanoamérica. Trump ve en Milei un socio muy confiable que servirá de contrapeso a la influencia china en la región. En Siria las cosas también han mejorado. Tras la caída de Assad en 2024 el nuevo Gobierno ha conseguido que les retiren casi todas todas las sanciones, lo que permitirá reconstruir el país y atraerá inversión extranjera. Para los sirios 2025 ha sido el primer año de paz en los últimos tres lustros. En el lado opuesto varios países han sufrido reveses importantes. Irán ha encajado el mayor golpe: su programa nuclear está destruido, aliados como Hezbolá muy debilitados y, para colmo de males, nuevas sanciones y la crisis económica va a más agravada por la inflación desbocada y protestas callejeras. Sus aliados venezolanos están incluso peor. El régimen chavista se encuentra aislado y Nicolás Maduro no tiene muy claro que el régimen pueda sobrevivir al bloqueo naval que decretó Donald Trump hace unas semanas. Rusia y China se han puesto de perfil, tanto con Irán como con Venezuela. En el primer mundo la Unión Europea atraviesa un estancamiento económico que se ha cronificado a causa de una regulación excesiva, altos impuestos y rigideces de todo tipo. Esto alimenta el auge de partidos de derecha identitaria que crecen como la espuma cabalgando sobre el descontento y la inmigración. En Canadá las cosas no van mucho mejor. El nuevo Gobierno de Mark Carney está batallando contra una crisis similar a la europea. En el Reino Unido pasa exactamente lo mismo. El año geopolítico ha sido, por lo demás, especialmente movido y ha dejado muchos asuntos abiertos para los próximos meses. Pero es bueno recordar que ninguna victoria o derrota es definitiva: los ganadores de hoy pueden ser los perdedores de mañana y viceversa. What rarely makes it into the headlines is what it feels like to stand in the middle of that chaos, especially when you're a young police officer with only weeks of experience on the street.     La invasión de Ucrania, que en breve cumplirá cuatro años, ha marcado un punto de inflexión en la evolución de la guerra moderna. Un conflicto que se esperaba rápido y de tipo convencional se ha convertido en una guerra de desgaste prolongada y caracterizada por una innovación tecnológica acelerada y tácticas asimétricas. Esta guerra ha demostrado que la superioridad numérica o el armamento tradicional ya no garantiza imponerse en el campo de batalla. La adaptación rápida a nuevas tecnologías como los drones aéreos y marítimos, la inteligencia artificial y el dominio de la guerra electrónica están demostrando ser elementos fundamentales que explican por qué esta guerra se está prolongando tanto. El cambio más revolucionario ha sido la irrupción de los drones como arma dominante. Los drones no es algo que se acabe de inventar, ya se utilizaban anteriormente, pero de forma mucho más limitada. Los drones de combate eran caros, sofisticados y sólo unas pocas potencias podían permitírselos. Todo eso ha cambiado en Ucrania. Al principio drones como los turcos Bayraktar TB2 fueron de gran importancia para detener el avance ruso. El Kremlin se vio obligado a mejorar sus defensas aéreas y a dedicar muchos más recursos a la guerra electrónica. Esto trajo la proliferación de drones FPV de fabricación china y muy bajo precio que fueron de una endemoniada efectividad. Ucrania pasó de importar a producir millones de drones cada año. Hoy en el país hay más de 200 empresas dedicadas ello. Rusia respondió con la fabricación a gran escala de drones Shahed de origen iraní y variantes propias que pueden lanzarse formando enjambres. No se ha parado de innovar sobre estos artefactos no tripulados. Han aparecido drones de fibra óptica, drones autónomos movidos por inteligencia artificial y enjambres coordinados que saturan las defensas enemigas. La guerra electrónica se ha convertido en un frente más, un frente invisible pero no menos decisivo. Ambos bandos saturan el espectro electromagnético con “jammers”, trazando señales para lanzar contraataques. Esto ha comprimido ciclos de innovación que durarían meses o años a pocas semanas. Un dron efectivo hoy puede volverse obsoleto en solo unos días. En Ucrania han desarrollado sistemas para operar sin GPS, Rusia, por su parte, emplea drones con SIM ucranianas clonadas. La adopción de sistemas de inteligencia artificial no ha hecho más que acelerar la innovación. La IA permite el reconocimiento autónomo de objetivos y la evasión de defensas. En estos momentos los drones autónomos ucranianos tienen una precisión del 80% en sus ataques lo que ha nivelado el campo de batalla ya que Rusia exhibe mayor poder artillero y dispone de muchos más efectivos sobre el terreno. Desde el punto de vista táctico, la guerra de Ucrania ha dejado anticuados las grandes ofensivas mecanizadas de otros tiempos. Los tanques son demasiado vulnerables a los drones, lo que favorece la guerra de posiciones con trincheras y dispersión de fuerzas. En el mar los drones ucranianos han diezmado la flota rusa del mar Negro. La guerra híbrida compuesta por ciberataques, desinformación y sabotajes ha hecho el resto. Lo que estamos viendo en Ucrania es una “democratización” de la guerra. Contendientes que sobre el papel son muy inferiores pueden plantar cara a superpotencias con inventiva, adaptación tecnológica y producción a gran escala.     Hi listeners. It's that time of year when we all take a few of those precious moments between the holidays and starting the New Year to look back, plan ahead, and savor those moments before heading back to work. This is the final DiepCJourney® podcast of 2025 and I decided to go solo for the first time since beginning this podcast during the pandemic in January of 2021. Let's take a look back at what we have provided courtesy of my outstanding guests. I always say, a podcast is only as good as its guests, and I am so lucky to have met them via my podcast and many I have met in person. When I had my first podcast with Dr. C in January of 2021, we had been working together in this patient education advocacy space for six years. That number has now rolled over to 11 years working together and he has been a cherished guest on so many topics on the podcast. Since that first podcast in January of 2021, I have interviewed surgeons, both microsurgeons and breasts surgeons from across the world. Thank you to each of you for providing such valuable information to the listeners of the podcast. I set this podcast up originally as a stand-alone platform but now because of changes in social media platforms I am able to share most of the podcast over on the DiepCFoundation YouTube channel. You can find them on the channel listed as Podcasts if you'd like to watch them and actually see my guests. It's a fun alternative and really what some followers like over listening to the podcast. With a niche podcast like this, my priority is creating content that truly serves the community—not chasing numbers. In a space as specific as breast reconstruction, I measure success by connection and education rather than statistics. The beauty of a niche podcast is that it reaches exactly who needs it most, so I focus on value over metrics. Last year in January we kicked off the year with my friend Rory Zura of Foobs and Fitness. Understanding and knowing the research behind the benefits of exercise for breast cancer patients is so important and a topic that is asked about often. Thanks Rory! She is a survivor with her own unique story. Patients stories matter to many, not only other patients, but healthcare professionals who can improve their communication and protocols for patients based on patient stories. One of the most listened to podcast of 2025 was from Margaret Tueller Proffit, a young BRCA genetic mutation carrier who started her reconstruction with implants, transitioned to DIEP flap and then graced us with the conclusion of her reconstruction by sharing one of the best quotes I heard from a patient all year, "Go forth with confidence"! Sensation of the breast always gets a lot of listeners. Our interview with Dr. Lisa Hunsicker was popular. We tackled diastasis recti and why to repair it during DIEP with Dr. Kesley McClure of Southwest Breast and Aesthetics and found out what other procedures are performed in Phoenix at their facility with her colleague, Dr. Arvind Gowda. Dr. Craigie shared a very interesting interview with us on the delay procedure, the SIEA, and the TDAP. These alternative methods of using a patients own tissue are very important for patients who don't always find this information easily. Nipple reconstruction with Dr. Carpenter at PRMA came out of an in-person meeting we had at a microsurgery summit I attended. Thanks Dr. Carpenter for your warm smile and willingness to spend time with me to educate our audience.     Hey friend. I'm so glad you're here with me today. If you're listening to this, chances are life has been feeling a little… full. Not bad. Not wrong. Just full. Between work, kids, school, meals, and all the little decisions you make from the second your feet hit the floor, it's no wonder your heart is craving rest.   I'm just sitting here in our -20 degree weather batching some episodes as our next few weeks we are going to be enjoying all the family and Grandkids.  Good day to stay inside! Today I want to talk about something that has changed my life and has changed the lives of thousands of busy moms I've walked with…Simplicity. Not the Pinterest kind. Not the everything-is-perfect kind. But the kind that actually makes your life easier. The kind that brings peace back into your home. The kind that reconnects you with the moments that matter most. Most moms don't realize this, but complexity steals our joy one tiny decision at a time. Every extra step. Every scattered routine. Every night you walk into the kitchen wondering what on earth to cook, it all chips away at your energy. It's letting meals become easier by cooking once and eating all week. It's allowing grocery day to be simple by keeping the same staples on hand. It's trusting that less really is enough. When you take away the noise, you uncover a life that was already waiting for you - slower, calmer, more intentional. Simplicity doesn't just change your to-do list. It changes you. Your evenings feel peaceful instead of rushed. • Your kids get a more present, less frazzled version of you. • You start saving money because you're not grabbing takeout out of panic. • You stop living in survival mode and start living on purpose. • You begin to feel like you have your life back, even while working full-time. So many moms tell me, I didn't know it could feel this easy. But it can. And it starts with small shifts, often one simple system at a time. I didn't start out living simply, I learned it because I had to. As a nurse, I worked long shifts. I came home exhausted, hungry, and feeling like I had nothing left to give. I didn't want fast food to raise my kids, and I didn't want stress to define our home. So I started building small systems. Batch cooking. Simple routines. Staples I always kept on hand. La organización territorial de España es el resultado de un largo proceso histórico que refleja su diversidad cultural, política y geográfica. Desde la Antigüedad, la Península Ibérica fue un mosaico de pueblos prerromanos, pero sin unidad política. Roma impuso la primera estructura administrativa con provincias como la Tarraconense, la Bética y la Lusitania, una división que influyó en futuras fronteras. Tras la caída del Imperio Romano, los visigodos unificaron el territorio ubicando la Corte en Toledo y manteniendo a grandes rasgos la organización territorial romana, pero la invasión musulmana de 711 fragmentó la península, con un emirato que luego se transformó en califato en el sur y núcleos cristianos en el norte (Asturias y la Marca Hispánica). La Reconquista trajo reinos independientes como Castilla, León, Aragón, Navarra y los condados catalanes, que configuraron las bases de regiones actuales. Los Reyes Católicos unieron Aragón y Castilla por arriba, pero cada uno de los reinos siguió manteniendo sus instituciones y leyes propias. Felipe V tras la Guerra de Sucesión, trató de centralizar el poder con los Decretos de Nueva Planta pero Navarra y el País Vasco conservaron sus fueros. En el siglo XIX, la reforma de Javier de Burgos (1833) dividió España en 49 provincias (50 tras dividir Canarias en 1927), inspirada en modelos franceses y liberales. Buscaba uniformidad administrativa para poder gobernar de forma más racional. Esta estructura, basada en capitales provinciales y criterios puramente administrativos se mantuvo en todas las constituciones, incluyendo la de 1978, y sirvió de base para las autonomías. El concepto moderno de regiones surge en el siglo XIX, pero la autonomía regional, que tuvo un primer intento en los años de la segunda República, solo se consolidó tras la Transición. La Constitución de 1978 reconoció “nacionalidades y regiones”, creando 17 comunidades autónomas entre 1979 y 1983. Años más tarde, en 1995, las dos plazas de soberanía del norte de África (Ceuta y Melilla) se convirtieron en ciudades autónomas.     Julie Colombino-Billingham was working in disaster response 15 years ago, and was in Haiti after the earthquake of 2010. She never went back.     Volodímir Zelenski y Donald Trump se reunieron este domingo en Mar-a-Lago para revisar un borrador de 20 puntos destinado a poner fin a la guerra entre Ucrania y Rusia, que ya dura casi cuatro años. La reunión ha traído algo de optimismo, pero muy moderado ya que no se anunció avance alguno. Trump dijo que las conversaciones estaban siendo muy productivas y aseguró que Ucrania y Rusia están "más cerca que nunca" de un acuerdo, pero reconoció que hay obstáculos importantes en el camino, especialmente en temas territoriales. Previo al encuentro Trump mantuvo una llamada telefónica de más de dos horas con el presidente ruso, Vladimir Putin, que calificó de "excelente" y "productiva". Según Trump, Putin expresó su disposición para que Ucrania prospere tras la guerra, se comprometió incluso a facilitar a Ucrania suministros energéticos a bajo coste. Ante esto Zelenski reaccionó con escepticismo. Rusia, recordemos, ha “celebrado” la Navidad arrojando bombas sobre civiles ucranianos. El Kremlin confirmó que Putin ha aceptado crear dos grupos de trabajo: uno sobre seguridad y otro sobre asuntos económicos. Ambos comenzarán su tarea en enero del año próximo. El plan de 20 puntos, revisado por negociadores estadounidenses y ucranianos a partir de una propuesta inicial de 28 puntos muy criticada hace dos meses por ser demasiado favorable al Kremlin, contiene elementos como un referéndum en Ucrania sobre cesiones territoriales, elecciones presidenciales, garantías de seguridad inquebrantables (algo similar al Artículo 5 de la OTAN) y la creación de una "zona económica libre" desmilitarizada en ciertas áreas del Donbás. Zelenski indicó que el borrador está acordado en un 90%, con coincidencia total en las garantías de seguridad, pero persisten algunos desacuerdos importantes como el control de aproximadamente el 20% de la región de Donetsk aún bajo control ucraniano (pero que Rusia exige en su integridad), el estatus de la central nuclear de Zaporiyia (ocupada en estos momentos por Rusia) y la secuencia de un posible alto el fuego, algo que los rusos por ahora rechazan. Trump insistió en la complejidad del proceso: "Esto no es un acuerdo de un día, es algo muy complicado” afirmó al tiempo que indicaba que un acuerdo definitivo podría demorarse semanas en alcanzarse. Eso sí, advirtió que, sin él, la guerra continuaría con más víctimas. Zelenski, por su parte, destacó la necesidad de mantener la presión sobre Rusia mediante sanciones y la defensa antiaérea, más aún tras los recientes ataques rusos que han dejado amplias zonas de de Ucrania sin electricidad. Desde Moscú el ministro de asuntos exteriores, Sergei Lavrov, criticó a Europa por ser, a su juicio, el "obstáculo principal para la paz" y acusó a Zelenski de no estar listo para unas negociaciones constructivas. El Kremlin se reserva, además, perseguir sus objetivos militares si no se alcanza ningún acuerdo. Tanto el enviado especial para Ucrania, Steve Witkoff, como el yerno de Trump, Jared Kushner, y el secretario de Estado, Marco Rubio, participan activamente en un proceso en el que, al menos por ahora, se está dejando básicamente de lado a las potencias europeas. La organización territorial de España es el resultado de un largo proceso histórico que refleja su diversidad cultural, política y geográfica. Desde la Antigüedad, la Península Ibérica fue un mosaico de pueblos prerromanos, pero sin unidad política. Roma impuso la primera estructura administrativa con provincias como la Tarraconense, la Bética y la Lusitania, una división que influyó en futuras fronteras. Tras la caída del Imperio Romano, los visigodos unificaron el territorio ubicando la Corte en Toledo y manteniendo a grandes rasgos la organización territorial romana, pero la invasión musulmana de 711 fragmentó la península, con un emirato que luego se transformó en califato en el sur y núcleos cristianos en el norte (Asturias y la Marca Hispánica). La Reconquista trajo reinos independientes como Castilla, León, Aragón, Navarra y los condados catalanes, que configuraron las bases de regiones actuales. Los Reyes Católicos unieron Aragón y Castilla por arriba, pero cada uno de los reinos siguió manteniendo sus instituciones y leyes propias. Felipe V tras la Guerra de Sucesión, trató de centralizar el poder con los Decretos de Nueva Planta pero Navarra y el País Vasco conservaron sus fueros. En el siglo XIX, la reforma de Javier de Burgos (1833) dividió España en 49 provincias (50 tras dividir Canarias en 1927), inspirada en modelos franceses y liberales. Buscaba uniformidad administrativa para poder gobernar de forma más racional. Esta estructura, basada en capitales provinciales y criterios puramente administrativos se mantuvo en todas las constituciones, incluyendo la de 1978, y sirvió de base para las autonomías. El concepto moderno de regiones surge en el siglo XIX, pero la autonomía regional, que tuvo un primer intento en los años de la segunda República, solo se consolidó tras la Transición. La Constitución de 1978 reconoció “nacionalidades y regiones”, creando 17 comunidades autónomas entre 1979 y 1983. Años más tarde, en 1995, las dos plazas de soberanía del norte de África (Ceuta y Melilla) se convirtieron en ciudades autónomas.     En las últimas décadas la alianza transatlántica ha sido un pilar esencial del orden mundial. Esta alianza surgió tras la Segunda Guerra Mundial y se fortaleció al término de la Guerra Fría. Europeos y estadounidenses comparten, aparte de cultura, valores como la democracia liberal, el Estado de derecho, el gobierno limitado y el libre comercio. Esa visión del mundo evitó grandes conflictos, derrotó al comunismo y impulsó la prosperidad de posguerra. Hoy, sin embargo, muchos dudan de que es alianza pueda sobrevivir. El canciller alemán Friedrich Merz, conservador y tradicionalmente atlantista, declaró recientemente que lo que entendíamos como Occidente ya no existe. Al mismo tiempo ha pedido a los europeos que prioricen sus intereses. Sus palabras no son más que el reflejo de un sentimiento generalizado en Europa: un enfriamiento profundo en las relaciones transatlánticas, con temores incluso a una ruptura total mientras Trump sea presidente. El núcleo del conflicto radica en visiones distintas sobre lo que es "Occidente". La administración Trump acusa a Europa de traicionar a la civilización occidental ya que, por culpa de la inmigración, está diluyendo sus raíces. Europa, además, carece de ambiciones expansionistas, a diferencia de los planes trumpistas de anexionar Canadá, Groenlandia o la zona del canal de Panamá. Esa retórica inflamada ha pasado al campo de la acción. La hostilidad verbal es continua, los aranceles punitivos, el plan de paz para Ucrania parece redactado en Moscú y la Estrategia de Seguridad Nacional que presentaron hace unas semanas es un ataque directo contra sus aliados europeos. Con ella pretenden intervenir directamente en la política europea para apoyar a ciertos partidos antiinmigración que a menudo son también prorrusos. La cuestión es que Occidente no se define por una etnia o una religión, sino por un tipo de sociedad abierta que descansa sobre la libertad individual, la tolerancia, el pluralismo político, la democracia liberal y el libre mercado. Estos valores son los que Europa y EEUU han defendido de la mano en las últimas ocho décadas. Se han exportado además con éxito a otras partes del mundo. Trump simplemente proyecta las guerras culturales internas estadounidenses sobre Europa, donde no se han presentado con la misma virulencia. El hecho es que el orden mundial tal y como está concebido beneficia fundamentalmente a Estados Unidos, que siempre ha contado con una extensa red mundial de aliados voluntarios e incondicionales, cosa que, por ejemplo, nunca ha tenido China.
