Foot pain is a common issue that many of us face at some point in our lives. While plantar fasciitis is often the culprit, there's another- probably under-reported condition that deserves attention: plantar fibromatosis, also known as Ledderhose disease. In our latest podcast episode, Debra Schindler takes a dive deep into this lesser-known condition with MedStar Health podiatric foot and ankle surgeon, Dr. Paul Carroll.Plantar fibromatosis affects approximately 200,000 people in the United States and is characterized by the growth of fibromas—benign nodules—on the plantar fascia, the thick band of tissue that runs along the bottom of your foot. Unlike plantar fasciitis, which typically causes heel pain that improves throughout the day, plantar fibromatosis presents as a constant pain in the middle part of the foot. Patients often describe feeling a pea-sized bump on the bottom of their foot, which can become painful as it grows.Dr. Carroll explains that while this condition is generally benign, it can be quite painful and debilitating if left untreated. The demographic most affected includes males, diabetics, individuals with chronic alcohol use, and those who have experienced trauma to the foot. Interestingly, there is also a significant correlation between plantar fibromatosis and Dupuytren's contracture, a condition affecting the hands.So, what should you do if you suspect you have plantar fibromatosis? Dr. Carroll advises seeking medical attention as soon as you notice any unusual bumps or persistent pain in your foot. Early diagnosis can help manage the condition more effectively and may prevent it from worsening. While there are several treatment options available, including conservative measures like custom orthotics and steroid injections, surgical options are often considered when other treatments fail. However, surgery comes with its own set of challenges, including a high recurrence rate of the fibromas.One of the more promising treatments discussed in the episode is the use of hyaluronidase injections, which help to shrink and soften the fibromas, making them less symptomatic. Dr. Carroll shares his experiences and success stories with this treatment, providing hope for those suffering from this condition.If you're experiencing unexplained foot pain or have noticed a bump on the bottom of your foot, don't ignore it. If you're a runner with heel pain you may think you have plantar fasciitis. But if it is not getting better, your heel pain may be caused by "neuritis." I was just giving a lecture to a group of physicians getting their continuing medical education credits at the International foot Medical Foundation medical conference in Lake Tahoe. I was giving a lecture called “When Heel Pain is NOT Plantar Fasciitis in Runners.” One of the conditions I was talking about was "medial calcaneal neuritis." One of the doctors in the audience asked a really great question... He said, “I see a lot of people that have Baxter's neuritis, and it's down on the bottom of the heel it's plantar , it's not on the side of the heel, it's not medial neuritis, it's different. What would you do to tell the difference between these two conditions in a runner, given the circumstances you just explained in the talk?” Today on the Doc On The Run Podcast we're talking about medial calcaneal neuritis versus Baxter's neuritis in a runner.

Hi and welcome to Physiotutors podcast episode 51 with Henrik Riel. Henrik is a physiotherapist from Denmark and has obtained his PhD in the management of plantar heel pain. He is doing a post-doc fellowship at the university of Aalborg and he is an assistant professor at the physio department of the university of northern Denmark. I have idiopathic bilateral heel pain that means I can't stand for more than 10 minutes without being in agony. Every scan and test I've had has come back healthy/normal, other than hormones which I have since optimised to no effect. I've had over 300 appointments across the world with every possible type of practitioner in conventional medicine and included 14 surgeries. It has culminated in what my surgeon believes is a world's first surgery, as well as tibial nerve removal, also to no effect.
