Israel Hamas Peace Deal Hamas has agreed to a ceasefire and peace plan proposed by President Donald Trump. The agreement was reportedly signed in Egypt, with multiple Arab nations participating in mediation (Qatar, Egypt, Turkey). Hostage releases are a central part of the deal, including both living captives and the return of bodies of those killed since the October 7, 2023 attacks. Trump is quoted extensively, describing this as a “historic” moment and promising a new era of peace in Gaza. The document includes reactions from families of hostages, marked by emotional gratitude toward Trump. Trump’s remarks on rebuilding Gaza and establishing a “Council of Peace” suggest a broader reconstruction initiative funded by wealthy Middle Eastern nations. The final section shifts into Trump’s domestic commentary, highlighting supposed successes in reducing crime in Washington, D.C., and comparing local peace and safety efforts to the broader Middle East peace vision. Eric and I had the pleasure of doing a GeriPal Live! Podcast as the closing keynote for the recent Center to Advance Palliative Care (CAPC) National Assembly in Philadelphia PA. For this podcast, we invited 3 guests to each select an article of interest to them, and engage in a discussion about the article, including questions from the CAPC attendees in the audience. Our guests chose the following articles (in the order discussed) Matt Gonzalez used AI to select an article by Ravi Parikh on algorithm based nudges to default patients with advanced cancer into a palliative care consult, published in JAMA Network Open, finding 44% in the intervention arm received palliative care consults, vs 8% in the control arm.  We discussed use of AI to select the article, nudges, sludge, and the impressive though still less than 50% rate of PC consults. Kim Curseen selected an article by Harry Han in JPSM that surveyed palliative care fellowships, finding that clinical fellows spent 5 weeks in outpatient care and 24 weeks in inpatient care, on average.  We discussed the mismatch between outpatient training, and the needs of people with serious illness who spend most of their lives outside the hospital, and training in inpatient care, where most hospitals have robust services and needs. Training in non-onc palliative care was particularly low. Karen Bullock selected a letter she first authored in response to Ira Byock's white paper on a path forward in hospice and palliative care. We discussed the need to view all issues in geriatrics, hospice, and palliative care through an equity lens, particularly in view of the disparate impact on historically marginalized communities of concerning practices and trends in hospice and palliative care (e.g. private equity gobbling up hospices).  And at the end, we sang a tribute to the Lady Gaga of Palliative Care, with the whole Assembly singing along! “We don't view a person with chronic pain as someone who has a chronic illness and the effect of that is we can't follow patients continuously over prolonged periods of time,” says Dr. Jacob Hascalovici, a neurologist and pain specialist based in New York City.  In co-founding Bliss Health, Dr. Jacob, as he is known, has set out to create a continuous care model for chronic pain treatment that matches the approach taken for patients with diabetes or high blood pressure. The Bliss Health formula includes an initial meeting with a physician that produces a care plan; remote therapeutic monitoring on an ongoing basis; and a monthly meeting with a nurse to review data and determine next steps, including additional appointments with physicians as needed.  All of this occurs via a digital platform which provides a welcome option for patients with mobility issues and can fill gaps in access to specialists, especially in rural areas. 1. FBI Spied on Republicans: The FBI performed a “toll analysis” (a term for examining phone call records and metadata) on eight Republican senators and one congressman. This was supposedly part of a secretive investigation codenamed “Arctic Frost”. The surveillance allegedly occurred after Donald Trump’s Mar-a-Lago raid and during Jack Smith’s investigation into Trump and the January 6th events. The senators targeted include: Lindsey Graham, Bill Haggerty, Josh Hawley, Dan Sullivan, Tommy Tuberville, Ron Johnson, Cynthia Lummis, and Marsha Blackburn — with Representative Mike Kelly also mentioned. An introductory narrator asserting the FBI’s wrongdoing. Quoted remarks from Senators Ron Johnson, Chuck Grassley, Bill Haggerty, and others, condemning the alleged surveillance. Peaches ////// 877Part 1 of 1  www.TrueCrimeGarage.comWhile law enforcement continues to connect crimes and still more victims to the infamous Long Island Serial Killer investigation we decided to take a look at one of the more recent developments in this ongoing saga. In July of 2023 a family man and architect named Rex Heuermann was arrested in connection with three murders tied to the Long Island Serial Killer case or L.I.S.K. for short. Those names and a few others were given to a series of unsolved homicides where the bodies and remains of several young women, a male, and a toddler were discovered in and around Long Island, New York. It is undetermined at this time if all of the murders were committed by the same killer or killers. Since the arrest of Heuermann, investigators have continued to make progress.
