According to a paper published by Shanafelt and Noseworthy of the Mayo Clinic in 2016, over 50% of physicians are experiencing symptoms of burnout! They also describe how this issue can influence quality of care, patient safety, physician turnover, physician turnover and patient satisfaction. So the Learn Psych Podcast wanted to further explore this topic, so we welcome back Dr. Kate Baker to discuss her role in our department’s house officer mental health program. We also discuss different coping strategies and ways to combat burnout that anyone can benefit from.     Today's show is about how to diagnose a phobia, with Dr. Kate Baker of the University of Michigan. In two weeks, we will be back with a second episode on treatment of phobias. This episode may be most helpful for pre-clinical and clinical students, as well as residents early in their training. In today's episode we cover: - Specific phobias (like fear of flying, fear of getting an injection) - Social phobia (also known as social anxiety disorder), which can include things like fear of public speaking - Agoraphobia - Fear neurocircuitry   Specific Phobia criteria: - intense anxiety, almost always provoked by stimulus - avoidance / endured with intense anxiety - anxiety out of proportion to actual danger - clinical distress or impairment (social, occupational, etc) - duration over 6 months - not better explained by another disorder   Social phobia: one additional criterion to the specific phobia criteria is: individual fears that he/she will act in a way or show anxiety symptoms that will be negatively evaluated, such as being rejected or embarrassed. Agoraphobia: The additional criterion to specific phobia is two or more of the following: Fear of: - using public transportation - being in open spaces - being in enclosed spaces - being in a crowd or line - being outside the home alone     Basic steps of neurocircuitry: 1. signal of danger/trauma inputted into brain 2. brain puts context around this information 3. if brain interpretation is of concern, then fight/flight activated. Key brain structure in the fear response is the amygdala.   Resources: You may find it helpful to review fear neurocircuitry from the National Neuroscience Curriculum Initiative website.

    The biopsychosocial formulation is a way of writing an assessment of a patient in psychiatry. As the name implies, it combines biological, psychological, and social information to develop a full picture of the presentation. Although it was actually created to be used in a wide range of medical fields, it is most often found in psychiatry. After discussing the origins of the biopsychosocial formulation, we create an example formulation using a well-known fictional story from popular culture. Then, we further discuss how to sort information into the biological, psychological, and social categories. This episode may be most helpful to pre-clinical students, clinical students, and residents early in their training. This is a topic frequently covered on exams, especially for clinical students, who may be asked to write an example formulation. Providers outside the field of psychiatry may find the episode helpful to encourage extra consideration of the psychological and social aspects of patient care. Or, maybe you frequently consult psychiatry and want to better understand their notes.

    Episode 7 - Assessment of Decision Making Capacity Today’s show is about how to resolve competing interests when a patient wants to do one thing, but the medical team thinks something else would be better. For example, what happens when you have a patient who has a bad infection and needs a surgery to survive, but the patient is refusing the surgery? The surgical team wants to operate, but the patient says no, though it appears the patient is a little confused and may not quite understand the situation. How do you reconcile these differences of opinion and figure out how to proceed? We will explore this topic of assessing decision making capacity, often just called capacity for short, on this episode. To help explore this topic, we will again be joined by Dr. Charles Surber of the University of Michigan. This episode is most likely to be helpful for clinical students currently doing a psychiatry rotation, particularly on a consult-liaison service. Pre-clinical students might also find it helpful. Others who may find it useful include psychiatric residents, especially early in residency. Other physicians who consult psychiatry on their patients may also find this material helpful.
