Soon after Lisa Iezzoni MD was diagnosed with multiple sclerosis during her first year at Harvard Medical School, from which she graduated in 1984, faculty and administrators discouraged her from practicing medicine.  And in her final year they made it impossible for her when the dean's office refused to write a recommendation letter (now called a Medical Student Performance Evaluation).  This week marks the 35th anniversary of the American Disabilities Act which was signed into law on July 26th, 1990. Much has changed since but -- as Dr. Iezzoni, who went on to become an eminent scholar and national policy leader on disability, has documented -- not nearly enough.  She and others have documented wide disparities in the quality of care patients with disabilities still receive, and in 2021 she published the findings of a national study of physician attitudes towards disability that document's persistent stigma and misinformation. Disabled patients were described as a “disruption to clinic flow” and a majority of physicians believed that patients with significant disability inherently have a worse quality of life.  Practices are also ill-equipped to care for them, Dr. Iezzoni, who chaired the U.S. Access Board's advisory committee on accessibility standards for medical equipment observed that “healthcare has been among the most backward environments, in terms of making itself accessible. Movie theaters are accessible, sports stadiums are accessible, transit is accessible. But healthcare facilities—no.” In our conversation with her, we explore why.  How is it that physicians have such difficulty appreciating that disability is simply a part of the human experience? We have a long history in America of institutions and public systems that do not serve all members of the public equally. As Erin Kilmer-Neel shares on this episode of Power Station, the financial services industry, banks in particular, have routinely failed to approve loan applications for small businesses and mortgages from borrowers of color on par with their white counterparts. Perceptions of unworthiness is rooted in a well-documented history of racism in public and corporate policymaking. As executive director of the Beneficial State Foundation, the nonprofit that owns and partners with Beneficial State Bank, an equity-driven enterprise, Erin is powering a movement to unlearn the biases that undergird perceptions of risk and reimagine policies and practices that meet the capital needs of borrowers of color. Take Underwriting for Justice, an initiative that is upending racially discriminatory lending norms. It is engaging banks in signing on to methodologies that lead to systemic yeses. Erin is a bright light in the ongoing campaign to bridge the social equity and banking worlds. She credits the vision of Beneficial Bank co-founders Tom Steyer and Kat Taylor in creating an institution that builds power within, instead of extracting from, the communities it serves.

The department leveraged debt to make vital improvements, but the need for a sustainable funding solution persists.

Karen and Meta share an interest in Systems Thinking and its potential applications to horse welfare in sport. So when Dr Megan Ross and her co-authors published a paper on wicked problems and systemic issues in Canadian dressage she had to be a podcast guest!Dr Megan Ross has a background in dressage and eventing. She has an applied science degree from the University of British Columbia with a focus on Animal Welfare and has applied this background (and Pilates certification) as a trainer and riding instructor to support the horse-rider dyad. She recently completed a PhD at the Atlantic Veterinary College and the University of Prince Edward Islandunder the supervision of Dr. Caroline Ritter and Dr. Kathryn Proudfoot. When that happens, these breaks are often repaired by the PARP enzyme, and the cells can continue their replication process. If we block PARP, that repair cannot happen. It is one of those targeted therapies that does affect blood cell counts. So I'd say the blood cell count that is most commonly affected here is the hemoglobin. So, anemia very frequent complication that we see, probably a little bit more with olaparib compared with other drugs, but we see it as a class side effect. And we can also see neutropenia and thrombocytopenia with these agents, probably a little bit more with niraparib versus the others, but again, you can see it across all of these drugs.” TS 8:16 “We mentioned that rare risk of MDS and AML. This isn't a particularly scary thing if you talk to patients about it. Because of the rarity that we see this, it isn't something that we need to overemphasize, but I think careful monitoring of blood counts in is stressing the importance of that and early intervention here is very important.” TS 16:55 “This is a collaborative effort. And because of the home administration here, these patients do need to be followed very closely. So we are not laying eyes on them usually with the frequency that we do when we have patients actually coming into our infusion centers for treatments—so making sure that there is a plan for regular follow-up with these patients to ensure that they're getting that lab work done, that that's being looked at closely, that we're adjusting the dose if we need to based on that lab work, that we are managing the patient's fatigue. Again, that potentially dose reductions may be needed if patients are having that extreme fatigue.” TS 19:34 “I think one of those [misconceptions] could be that they're only effective in patients that have that BRCA1/2 mutation. And again, remember here that there is some data in particular disease states that we can use them and that they work in the absence of those mutations.” TS 25:12

Sharks have been cast as the ultimate villain ever since the movie “Jaws” was released 50 years ago this summer. The film tells the story of a great white shark terrorizing beachgoers, but in reality, humans pose the greater threat to sharks. Ali Rogin speaks with endurance swimmer Lewis Pugh, who wants to get that message out by swimming around Martha’s Vineyard, where “Jaws” was filmed. While searching the interweb for Rhino documentaries, Robbie came across a TED talk by a professor from the Uiversity of Arkansas Monticello called Elena Rubino talking about Rhinos. He was immediately intrigued as its not a place that one thinks discussions around Rhino Conservation and Trade come from. Robbie reached out, and it turns out the Elena did her Ph.D. work on the perceptions of Trophy Hunting, and naturally that was something he wanted to learn about. Robbie is joined today by Dr. Elena Rubino, a professor at the University of Arkansas-Monticello, to talk about a survey she undertook to understand perceptions around trophy hunting.
