Welcome back to The Peds NP Acute Care Faculty series! This series was created and peer-reviewed by national leaders in acute care PNP education collaborating with one another to meet the needs of our current and future colleagues. In the push for competency-based education where faculty verify the skills of what a student can do, rather than their knowledge, our series focuses on the application of didactic content with a practical approach so that you can learn nuances of clinical skills before you reach the bedside.  This episode explores wellness for the pediatric nurse practitioner. We first discuss burnout in terms of its physical and mental impact on the provider as well as poorer patient outcomes and healthcare systems burdens. After we identify symptoms of burnout and compassion fatigue, we'll discuss what wellness looks like in various systems at work from organizational culture of wellness to ease of daily work and finally personal resilience. We end with a discussion on self-compassion and introduce the evidence in support of meditation as a strategy to promote mind-body wellness. For some, this concept is new, so the technical aspects of meditation are unpacked to facilitate your own path to daily wellness. Knowing that I'm not going through this alone.” -Vanessa Naum   We're excited to welcome back Physician Marriage Coaches, Dr. George (Dr. Jeep) and Vanessa Naum.    In this episode, Vanessa discusses her experience dealing with Fibromyalgia and how being married to a physician has impacted her journey living with a chronic illness. Dr. Jeep and Vanessa give advice on how to best help a partner or loved one dealing with a chronic illness and the importance of understanding and compassion.  You don't want to miss this inspiring episode!   According to a report by the Centers for Disease Control and Prevention (CDC) published in 2018, approximately 60% of adults in the United States have at least one chronic condition, such as heart disease, diabetes, or cancer.    Furthermore, a survey conducted by the National Alliance for Caregiving and AARP Public Policy Institute found that about one in three adults in the United States, or 34.2 million people, have provided unpaid care to an adult aged 50 or older in the past 12 months. Of these caregivers, 60% reported providing care for someone with a chronic illness.   It's worth noting that these statistics are from studies conducted before the COVID-19 pandemic, which may have had an impact on the prevalence of chronic illnesses and caregiving in the United States.   According to a study published in The Journal of the American Medical Association (JAMA) in 2020, 16.5% of physicians in the United States reported having a chronic illness and 14.2% reported providing care for an adult family member with a chronic illness.    The study also found that female physicians were more likely than male physicians to have a chronic illness or care for someone with a chronic illness. Tait Shanafelt, MD is an outstanding human being and doctor.  In addition to being a well-respected and renowned medical oncologist, he is the Chief Wellness Officer at Stanford Medicine and one of the most prolific writers of medical literature regarding wellness and burnout in medicine.  In this episode of OWZ, we discuss both individual and institutional approaches to wellness highlighting both the responsibility of each of us, as surgeons, to our patients and institution, as well as the responsibility of the field of medicine, our institutions, and hospitals to us to promote a culture of wellness and well-being.  As we better understand well-being and all of its implications on patient care, satisfaction, job turnover, etc., we recognize the only recipe for success involves physicians, surgeons, administrators, and hospital working together.  We briefly touch on his concept of “Physician Well-Being 2.0” and the article link can be found here: https://pubmed.ncbi.nlm.nih.gov/34607637/.  I really enjoyed speaking with Dr. Shanafelt, I hope you enjoy listening.         In S3 E9 I am delighted to welcome Dr Daniel Murphy, MD to the podcast.  Dr Murphy is a Professor of Pediatrics(Cardiology),Emeritus . He is  the co-Director, Department Wellbeing Directors Council and the Course Director, Stanford WellMD Physician Well-being Director Course at Stanford University School of Medicine.   Our paths first crossed when I was a participant on the  Stanford Wellbeing Directors course in 2021.  After this virtual meeting I was delighted to be able to meet him in person this year at the CWO course and learn more about him and his leadership work at the  WellMD Center.         Dan has been affiliated with the WellMD Center since its inception, working primarily with the Department WellBeing Directors.  He is a member of the Clinician Wellness Working Group of the American College of Cardiology.  He also remains involved in clinical quality and process improvement at Lucile Packard Children's Hospital.     In this conversation we discuss in more detail the Stanford conceptual model of professional fulfilment, exploring each of the domains ( The Individual, Culture of Wellness and Efficiency of Practice) with a leadership and Wellbeing 2.0 lens.    Dan tells me about his own experiences  of cultivating a mindfulness and self-compassion practice.  We explore team culture from a leadership perspective, how to build coalitions in healthcare  and the role and value of connections made through commensality groups.          With extensive expertise and valuable perspective in a number of clinical and non-clinical leadership roles Dan shares practical wisdom, marrying both art and science, on what wellness-centred leadership looks like at its best.  In their recent paper on this topic ( linked below) the WellMD team, emphasise three key prerequisites :1.Care about People Always2.Cultivate Individual and Team Relationships3.Inspire Change   It is will be very evident to listeners that this is Dan's approach.  I throughly enjoyed our conversation and know I will re-visit it several times gaining a new pearl, depth and perspective each time. Chris is the host of "On Air with Chris Shanafelt," "The Chris & DJ Show," and the "Limestone Livewire" with Limestone football head coach Mike Furrey. Chris covers the Chicago Bears and gave a rundown of the Bears roster for this season. He also looked at the new coaching staff led by head coach Matt Eberflus and how the staff will work with second year quarterback Justin Fields. We then looked around the NFL at some of the league's news and notes from the preseason. The Ravens' 4th-round pick, TE Isiah Likely, had an explosive game last week with eight receptions for 100 yards and a touchdown. The former Coastal Carolina tight end appears to be in for a prominent role in the Ravens' offense. Next, we went around the NFL and evaluated some of the league's quarterback competitions. Former Bears QB Mitch Trubisky is battling with rookie Kenny Pickett for the starting job in Pittsburgh, while Baker Mayfield beat out Sam Darnold for the Carolina Panthers job. Marcus Mariota and Desmond Ridder have shown flashes for the Atlanta Falcons this summer. Finally, we looked at USFL star KaVontae Turpin's explosive playmaking ability with the Dallas Cowboys this preseason and broke down the Eagles' win over Cleveland. According to a paper published by Shanafelt and Noseworthy of the Mayo Clinic in 2016, over 50% of physicians are experiencing symptoms of burnout! They also describe how this issue can influence quality of care, patient safety, physician turnover, physician turnover and patient satisfaction. So the Learn Psych Podcast wanted to further explore this topic, so we welcome back Dr. Kate Baker to discuss her role in our department’s house officer mental health program. We also discuss different coping strategies and ways to combat burnout that anyone can benefit from. On this episode Dr. Philip Kroth an Internist and Chief of Clinical Informatics from the University of New Mexico schools us on how electronic health records (EHR) relate to burnout and tips to promote physician wellness.
