    Par difficulté à avoir un rendez-médical ou pour tout simplement pour gagner du temps, on peut être tenté par l'automédication, c'est-à-dire le fait d'avoir recours à des médicaments sans avis médical. Il s'agit parfois de soigner de petits maux, tels que le rhume, une migraine ou des troubles digestifs passagers… L'automédication peut aussi consister à la prise d'antibiotiques sans prescription, au recours à des médicaments périmés ou contrefaits. Surdosage, antibiorésistance, interactions médicamenteuses... Quels sont les risques de l'automédication ? En cas de céphalées ou de maux de gorge, le réflexe est d'aller rechercher dans sa propre armoire à pharmacie le cachet ou le sirop, qui permettra de trouver le soulagement. L'automédication, acheter ou s'administrer un traitement, sans consultation médicale, est une pratique courante. Mais à côté de l'automédication responsable, attention aux imprudences dommageables pour la santé, d'où l'importance d'identifier les situations à risque qui peuvent entraîner plusieurs complications ou effets indésirables. Les limites de l'automédication  Pour certains profils de patients, l'automédication est à éviter. C'est le cas pour l'enfant de moins de six ans, les personnes âgées de plus de 70 ans, chez la femme enceinte ou encore les malades chroniques, qui suivent déjà au long cours un traitement médicamenteux.  Chez ces derniers, la prise simultanée de plusieurs médicaments sans avis médical peut entraîner des interactions indésirables.  Il faut également mettre en garde contre l'utilisation de produits dits naturels (huiles essentielles, compléments alimentaires, remèdes traditionnels...) qui peuvent interagir avec d'autres préparations et/ou être mal tolérés. Identifier les risques  L'automédication peut aussi exposer à une réaction allergique, car de nombreux médicaments sont désignés par des noms ou des marques qui ne permettent pas forcément d'identifier la molécule active.  Même s'ils sont en vente libre, certains médicaments présentent davantage de risques que d'autres, comme l'aspirine, de certains traitements du rhume, des anti-inflammatoires et du paracétamol. L'automédication peut également poser un véritable problème en termes de surdosage, de consommation de substances (alcool).  Les mises en garde concernant l'automédication visent également la prise d'antibiotiques, y compris lorsque l'on termine une boîte entamée pour des symptômes a priori voisins. Ce recours abusif aux antibiotiques est l'un des facteurs majeurs de l'antibio résistance, qui  expose au développement de bactéries ultra-résistantes.  Et dans certains contextes, l'achat de médicaments sur internet ou sur la voie publique, expose aux contrefaçons et à l'absence de composé actif.     In a recent episode of the Joe Rogan show, evolutionary biologist and public intellectual Bret Weinstein observed that two emerging features of contemporary societies, especially, though not exclusively in the West, are challenging the very meaning and purpose of human life: 1) the decoupling of human sexuality from human reproduction—defining sex primarily as recreational and 2) with the rise of AI and robotics, the real possibility that having a job will become entirely optional in the future. By secular standards, pursuing both of these goals seems entirely rational, if not laudable: raising children and going to work are, indeed, challenging, so why shouldn't we live in a world in which both become increasingly rare? Weinstein, however—who doesn't profess adherence to any religious tradition—suggests that humanity may lose something important, if not essential, if we continue down this path. Is he right to be concerned? Is it, in fact, wise to relegate having children and going to work—which defined how most people spent most of their adult lives throughout history—entirely to the realm of subjective preference?     Every other week I'm republishing one of my most popular or impactful episodes. This week I'm highlighting Episode 336, which is all about local anesthetic systemic toxicity (LAST). You'll definitely want to give this one a listen. Local anesthetics are medications that block nerve impulses so that pain signals aren't transmitted to the brain. The result is that the anesthetized area is numb and pain is controlled.  Just because local anesthetics are “local” doesn't mean systemic toxicity can't occur. LAST occurs when local anesthetics extend into the system at toxic levels. It most typically occurs due to the physician accidentally injecting the medication into the intravascular space.
