The Consumer Product and Safety Commission (CPSC) is warning people to immediately dispose of a magnetic game because it poses serious ingestion risks for children. The CPSC posted a warning that "Magnetic Chess Games" sold by JOMO contain magnets that do not comply with U.S. federal safety regulations. As a result, the "loose, hazardous magnets pose a risk of serious injury or death," according to the warning. The CPSC said it issued a violation notice to JOMO, but that the company has not agreed to recall its Magnetic Chess Games or provide a remedy. The commission urged people to stop using the game and throw it away immediately. The games were sold online at walmart.com in a blue box with the word "magnetic" on the front and back, according to the CPSC. They include about 20 loose black magnets but not chess-shaped pieces, despite its marketing. It's unclear when or how long these games were sold. A CPSC spokesperson said the commission could not provide further information since JOMO is not cooperating. A Walmart spokesperson said customers' health and safety are the company's top priority and that it began working to remove the games soon after the CPSC warning was issued. Experts have long noted the serious health hazards tied to swallowing magnets, with children particularly at risk. When high-powered magnets are ingested, the CPSC noted, they can attract each other or another metal object in the body and become lodged in the digestive system—potentially resulting in blockage, infection, blood poisoning or death. Overall, the CPSC estimates a total of 2,400 magnet ingestions were treated in hospitals annually between 2017 and 2021. In this podcast from the PREMIER conference 2024, Francesca Stedman, a consultant paediatric surgeon from Southampton Children's Hospital discusses the care of the child who has ingested a button battery. Button batteries are ubiquitous and come in various types and sizes. There are about 85 different kinds available or in use in the UK alone, found in everything from toys to hearing aids. The most notorious is the CR2032, which is about 20 millimeters in diameter and 3.2 millimeters thick. It contains lithium manganese oxide, and while its small size makes it convenient for electronics, it poses a significant risk if ingested. The danger with button batteries lies in their potential to cause severe injuries when lodged in the esophagus. They can cause necrosis, which is essentially tissue death, due to a strong alkaline substance produced by the battery. This substance acts like a potent oven cleaner, rapidly causing damage. The esophagus has three natural narrowing points where these batteries often get stuck, increasing the likelihood of injury. The situation becomes critical very quickly, often within two hours of ingestion. One of the most concerning aspects of these incidents is that button battery ingestions are rarely witnessed. Children might present with vague symptoms like drooling, difficulty swallowing, or even just being generally unwell. These can easily be mistaken for other common illnesses, leading to delays in diagnosis. In one particularly harrowing case, a child presented multiple times with symptoms of a respiratory infection, only for an x-ray to reveal a button battery lodged in the esophagus. By then, the damage was extensive. When ingestion is suspected, immediate action is crucial. Getting a chest x-ray is the first step, and if necessary, a lateral x-ray can confirm the presence of a button battery by revealing a characteristic double rim or halo sign. Pre-hospital measures can include giving honey or jam, depending on the child's age, to help mitigate the damage. However, these should never delay getting the child to the hospital. Once at the hospital, the primary goal is to remove the battery as quickly as possible to prevent further injury. Depending on the location of the battery and available specialists, either ENT surgeons or paediatric surgeons may perform the removal. Post-removal care involves monitoring for complications like perforations or fistulas, which can develop days or even weeks later. In severe cases, these injuries can lead to life-threatening conditions, such as aorto-esophageal fistulas, which require immediate surgical intervention. In summary, button battery ingestion is a serious and often underappreciated risk. Even in homes where precautions are taken, accidents can happen. The key is quick recognition and action. As parents and caregivers, we need to be vigilant about keeping these small, dangerous objects out of children's reach. Carolin Bapp is a scientist and recently completed her Masters Research from the University of Vienna. Nutrient timing involves the purposeful consumption of all types of nutrients at various times throughout the day to favorably impact the adaptive response to acute and chronic exercise. Old idea: you need to deplete glycogen for carb loading supercompensation to work. New idea: you only need to reduce training training and eat high carbs for 3-4 days (8-12g/kg/day). Old idea: consuming fructose or carbohydrate close to competition can interfere with optimum performance. New idea: if carbohydrate consumption causes gut distress don't consume them close to competition Here are the ISSN guidelines for nutrient timing, which we discuss in the podcast. Endogenous glycogen stores are maximized by following a high-carbohydrate diet (8–12 g of carbohydrate/kg/ day [g/kg/day]). If rapid restoration of glycogen is required (< 4 h of recovery time) then the following strategies should be considered: a) aggressive carbohydrate refeeding (1.2 g/kg/h) with a preference towards carbohydrate sources that have a high (> 70) glycemic index b) the addition of caffeine (3–8 mg/kg) c) combining carbohydrates (0.8 g/kg/h) with protein (0.2–0.4 g/kg/h) Extended (> 60 min) bouts of high intensity (> 70% VO2max) exercise challenge fuel supply and fluid regulation, thus carbohydrate should be consumed at a rate of ~30–60 grams of carbohydrate/h in a 6–8% carbohydrate- electrolyte solution (6–12 fluid ounces) every 10–15 min throughout the entire exercise bout, particularly in those exercise bouts that span beyond 70 min. When carbohydrate delivery is inadequate, adding protein may help increase performance, ameliorate muscle damage, promote euglycemia and facilitate glycogen resynthesis. Carbohydrate ingestion throughout resistance exercise (e.g., 3–6 sets of 8–12 repetition maximum [RM] using multiple exercises targeting all major muscle groups) has been shown to promote euglycemia and higher glycogen stores. Consuming carbohydrate solely or in combination with protein during resistance exercise increases muscle glycogen stores, ameliorates muscle damage, and facilitates greater acute and chronic training adaptations. Meeting the total daily intake of protein, preferably with evenly spaced protein feedings (approximately every 3 h during the day), should be viewed as a primary area of emphasis for exercising individuals. Ingestion of essential amino acids (EAA; approximately 10 g)either in free form or as part of a protein bolus of approximately 20–40 g has been shown to maximally stimulate muscle protein synthesis (MPS). Pre- and/or post-exercise nutritional interventions (carbohydrate + protein or protein alone) may operate as an effective strategy to support increases in strength and improvements in body composition. However, the size and timing of a pre-exercise meal may impact the extent to which post-exercise protein feeding is required. Post-exercise ingestion (immediately to 2-h post) of high-quality protein sources stimulates robust increases in MPS. In non-exercising scenarios, changing the frequency of meals has shown limited impact on weight loss and body composition, with stronger evidence to indicate meal frequency can favorably improve appetite and satiety. Ingesting a 20–40 g protein dose (0.25–0.40 g/kg body mass/dose) of a high-quality source every three to 4 h appears to most favorably affect MPS rates when compared to other dietary patterns and is associated with improved body composition and performance outcomes.
