On Friday, February 18th, 2011, six-year-old Jack Adcock was admitted to the hospital by his general practitioner for nausea, vomiting, and diarrhea. His doctor, Hadiza Bawa-Garba, started him on fluids and requested tests. At the time she had just returned from 13 months of maternity leave and was brand new to the hospital. Her supervisor was a town away and she was covering the role of two other doctors. Around 7PM that night, Jack's parents gave him his medication for high blood pressure, just as they would have at home. Jack went into cardiac arrest and a team immediately started to resuscitate him. However, the electronic health records system was down that day, causing Dr. Bawa-Garba to confuse Jack for a different patient that had occupied his room earlier with orders for a DNR. They resumed life saving efforts but it was too late, Jack was pronounced dead at 9:20 PM that evening. Dr. Bawa-Garba was charged and convicted with manslaughter by gross negligence. She was also banned from practicing medicine. The medical community was shocked and outraged. Dr. Bawa-Garba was able to appeal the decision and reduce the ban to one year, but the manslaughter conviction remained. Fellows and Trainees in Australia and New Zealand can be assured that personal or reflective notes are registered under Commonwealth legislation for protection. This clarification from Michael Gorton AM, from Russell Kennedy Lawyers, stems from a negligence case in the UK where it was suggested reflective notes were used against Dr Bawa-Garba in her criminal conviction. Michael says the requirements for a criminal charge in Australia and New Zealand, which have been tested in the courts in both countries, require a higher standard than the UK.

Yesterday I did a podcast entitled 'Complicating the Uncomplicated' and today I want to follow on from that with this podcast entitled 'Strategic Blindness, Situational Awareness, Dr Hadiza Bawa-Garba and Physical Skills Training', because it adds more weight to the argument that teaching physical skills in a complex manner only adds to the cognitive workload of the learner, increases the margin for error to occur which in turn increases the risk of injury, serious assault and even death. In this episode, I'm also going to draw on the findings in the recent court appeal case of Dr Hadiza Bawa-Garba, as there are some interesting insights in this case that highlight the failings of a system as opposed to the failings of the individual.
