We are finally back with another ATLS podcast. Mike Sharma and Chip Lange together discuss the complex but important subject of thoracic trauma. They break it down this time by addressing key aspects that come up during the primary and secondary assessments. This topic also broaches how to manage the traumatic circulatory arrest patient without a pulse.

Last year at WEMSA2019 we had an amazing session that included speakers like Michael Perlmutter, Leon Eidelman, Michael Lauria, and Chip Lange. Each speaker had 20 minutes and the talks were all recorded. My goal was to put these out A LOT sooner, but the stupid Catalina Mac update prevented me from opening Adobe Audition files. Utilizing an extensive Google search and Russian hacking skills - I was able to access these talks. This is a 20ish minute talk by friend of the show, flight medic, and current medical student, Michael Perlmutter (@DitchDoc14). Michael is speaking on the application and use of Ketamine "from analgesia to RSI (yes and DSI)."

Recently, our own Chip Lange was on The Skeptic's Guide to Emergency Medicine (The SGEM) for Podcast #249 covering ultrasound to confirm endotracheal tube placement. This had previously been discussed in detail on our own Podcast #118 along with the technique. However, after some discussions on social media it was decided to expand on this discussion further regarding the ways to confirm beyond ultrasound.
Awhile back Aidan Baron (@Aidan_Baron) tweeted, "If you can't explain the pre-test probability, then you don't get to pick up the ultrasound.. no stats-no scan." This got me thinking about the way we approach testing in the pre-hospital setting. Are we performing EKG's, POC Labs, Ultrasound scans, and screens on patients just because we need to check off a box on our PCR, or do we truly believe the pre-test probably is high enough to justify the test? My thought process had always been that this is a way of triaging your mental bandwidth, and that it was only applicable when seeing more than just one patient. A physician, PA, or NP could not physically perform every test on every person without complications and decreased attention to top tier priorities. "You can't teach an old dog new tricks" is clearly repeated by an elderly man who's family got concerned he was having a stroke when they noticed some gait ataxia. EMS does a quick Cincinnati Stroke Scale and doesn't detect any deficits, 12 lead looks normal, and blood glucose is within normal limits. Should he be evaluated in the emergency room? Should EMS be concerned about a central lesion? Does a negative Cincinnati mean the patient is not having a CVA?
