Lung Cancer Histology and Staging*Workup for a nodule that is concerning: **Ensure there is a dedicated CT scan of the chest to evaluate **Try to obtain old imaging; the rate of change is important **Can get PET, but even if a lesion if not FDG-avid, but growing quickly we should consider biopsy anyway**Referral to pulmonary medicine, who can assist with biopsy and also regional lymph node evaluation (important – more below)**PFTs are often ordered because it provides information about lung function in anticipation of possible surgery for treatment Lung Cancer Histology: *Non-small cell lung cancer (NSCLC)**Umbrella term for a variety of cancers**Increased risk in smokers**More common types: ***Adenocarcinoma (~50% of all lung cancers)****Most common overall; cancer of the mucus producing cells****IHC: TTF-1, NapsinA, CK7 positive***Squamous Cell Carcinoma (22.7%)****More often seen in patients with a smoking history ****IHC: p63 positive and cytokeratin pearls***Remaining ~15% are the other types of lung cancer / mixed histologies**Small cell lung cancer (SCLC)***Neuroendocrine tumor with very different pathology***Much more aggressive than NSCLC***Oncologic emergency***IHC: Chromogranin and synaptophysin positive IHC pearls: TTF-1 usually means lung cancer (but can be negative in squamous cell lung cancer). This will be important in the future (we promise :])*Staging for NSCLC:**Nodal evaluation: lymph node evaluation is part of the workup for NSCLC**Single digit = central/mediastinal nodes (higher risk)**Double digit = peripheral/hilar/intrapulmonary lymph nodes (lower risk)**“R” vs. “L” is direction *Pearl: Why is this important? Colin Kaepernick has yet to be signed by an NFL team for the 2017 season, and I'm growing tired of hearing from the "Race Relations Peanut Gallery".....S.T.F.U. Already! Colin is not signed and we all know why. It is shameful and disgraceful, and serves as a representation of a larger issue. Stop comparing CK7 to convicted felons, accused murderers, and other athletes both past and present, that opted to make bad decisions for the lives and careers. In the present study, we examined the distribution of 6 groups ofintermediate filaments (IFs; cytokeratins, CKs, vimentin, synemin,desmin, glial fibrillary acidic protein and lamins) in oocytes andfollicular walls of the Japanese quail (Coturnix japonica) during theirdevelopment using immunohistochemical and ultrastructural techniques. Adistinctly vimentin- and synemin-positive Balbiani body, which is atransient accumulation of organelles (mitochondria, Golgi complex andendoplasnnic reticulum) that occurs in the oocytes of all vertebratesincluding birds, could be detected in the oocytes of primordial andearly pre-vitellogenic follicles. In larger pre-vitellogenic follicles,the Balbiani body has dispersed and the positivity of the granulosacells appeared to concentrate in the basal portion of their cytoplasm.Our ultrastructural data demonstrated that the matrix of the Balbianibody consists of fine IFs, which may play a role in the formation anddispersion of the Balbiani body. Of the CKs studied (panCK, CK5, CK7,CK8, CK14, CK15, CK18 and CK19), only CK5 showed a slight positivestaining in both the theca externa and the Balbiani bodies ofpre-vitellogenic oocytes. In conclusion, our data, which describe thechanges in avian IF protein expression during folliculogenesis, suggestthat the functions of the IFs (vimentin and synemin) of oocytes andfollicular walls are not primarily mechanical but may be involved in thetransient tethering of mitochondria in the area of the Balbiani body andin the gain of endocrine competence during the differentiation ofgranulosa cells.

Hepatic progenitor cells from adult human livers for cell transplantation.

Objective: Liver regeneration is mainly based on cellular self-renewal including progenitor cells. Efforts have been made to harness this potential for cell transplantation, but shortage of hepatocytes and premature differentiated progenitor cells from extra-hepatic organs are limiting factors. Histological studies implied that resident cells in adult liver can proliferate, have bipotential character and may be a suitable source for cell transplantation. Methods: Particular cell populations were isolated after adequate tissue dissociation. Single cell suspensions were purified by Thy-1 positivity selection, characterised in vitro and transplanted in immunodeficient Pfp/Rag2 mice. Results: Thy-1+ cells that are mainly found in the portal tract and the surrounding parenchyma, were isolated from surgical liver tissue with high yields from specimens with histological signs of regeneration. Thy-1+ cell populations were positive for progenitor (CD34, c-kit, CK14, M2PK, OV6), biliary (CK19) and hepatic (HepPar1) markers revealing their progenitor as well as hepatic and biliary nature. The potential of Thy-1+ cells for differentiation in vitro was demonstrated by increased mRNA and protein expression for hepatic (CK18, HepPar1) and biliary (CK7) markers during culture while progenitor markers CK14, chromogranin A and nestin were reduced. After transplantation of Thy-1+ cells into livers of immunodeficient mice, engraftment was predominantly seen in the periportal portion of the liver lobule. Analysis of in situ material revealed that transplanted cells express human hepatic markers HepPar1 and albumin, indicating functional engraftment. Conclusion: Bipotential progenitor cells from human adult livers can be isolated using Thy-1 and might be a potential candidate for cell treatment in liver diseases.
