He provides an example from viral hepatitis to prove his point.Roger asks how to motivate primary care practitioners who are overloaded with tasks they need to complete during short, overly scheduled patient visits.Jeff notes this is a challenge today, with a solution that lies in education that not only teaches about the disease but also stresses the cost of inaction. To Jeff, teaching FIB-4 to primary care and automating the algorithm to make it available in every electronic patient record is the minimum we should strive for today.Jörn focuses on the cost of inaction from a different direction: physicians who are afraid they will demoralize patients when they diagnose a disease with no available treatment. Jeff responds, "Do we not raise awareness because we don't have out-of-the-box solutions?" He goes on to describe different types of solution that might work for a patient, ranging from digital apps to linking patients to social agencies that can provide support and services. Jeff notes that "the field will need to change as we find more patients," due to increased demand on public health systems and social agencies.

One thing that, oddly, works in MASLD's favor is that most patients with advanced MASLD will have concomitant diseases requiring involvement with other specialties. Today, teams are looking at system issues to find the largest, most intractable structural problems so that solutions can emerge. After that, "we'll start to see bigger and faster improvements in the field." Jeff talks about several positive things happening in New York. First, he mentions a New York Times article about junk food and how manufacturers target aggressive marketing to poor and marginalized populations, with the net result that these populations have processed and ultra-processed foods making up a large share of their diets. Also, a "visionary" health commissioner is creating a program, Healthy NYC, with the goal of increasing life expectancy by reducing specific diseases.MASLD is not part of the effort today but with clinician support. we can get there over time. Why do complex systems self-organise? What is cellular uncertainty and stem cell plasticity? Can we create artificial digital life that's subject to the same creative adaptability that nature and life demonstrate? Today we have the extraordinary phenomena of self-organisation in Complex Systems to look into. We're going to be looking into the conditions for a system to be considered complex, how a certain amount of randomness in the system releases the creativity required to permit adaptability, and how the feedback loops within that adaptability lead to a self-correcting organisational principle that keeps the system's order and randomness in balance as it evolves. We're going to be seeing how that self-organisation is operative at almost every level of scale in the universe and in life and death, and trying to get our heads around what that means for the nature of reality and consciousness. So who better to discuss this with than stem cell biologist and diagnostic pathologist Neil Theise. Neil is is a professor of pathology at the NYU Grossman School of Medicine and a pioneer of adult stem cell plasticity research. In 2018 the news of his discovery of the interstitial, a vast communication network throughout the human body went viral and was featured in the New York Times and Scientific American among many others. Theise is also a long term student of Zen meditation and Kabbalah. And his studies of complexity theory, summarised in his new book “Notes on Complexity: A scientific theory of connection, consciousness and being”, have led to interdisciplinary collaborations in fields as diverse integrative medicine, consciousness studies and the science-spirituality interface. Since speaking with biologist Michael Levin on Cellular cognition, and cognitive scientist John Vervaeke on collective intelligence, in the last series; I've been keen to speak to Neil about stem cell plasticity and self-organising systems, as their elegant sophistication begs so many questions about the nature of reality and consciousness. If successful, the groundbreaking medical procedure could prove life-saving for thousands of people waiting for liver transplants around the world. So far, no complications have been seen from the procedure, but it will be several months before we know if the treatment is working as hoped in the first of 12 trial participants with end-stage liver disease.Even on a remote island untouched by tourists, fishing, pollution and development, the climate crisis is still wreaking havoc on the coral of Australia's Great Barrier Reef. Reporter James Woodford visited One Tree Island, a refuge ordinarily spared from the reef's past catastrophic bleaching events, and discovered that this year's marine heatwave has managed to reach even that protected spot.
