      The authors analyzed data from nearly 3,000 breast MRI exams administered at the Baltimore institution between 2011 and 2016. They pinpointed 185 individuals who met the study’s criteria, with a provider labeling them as BI-RADS category 3.  About 57% of study subjects received their follow-up examination in a timely fashion within 10 months, and another 18% did so after a slightly longer period of time. Meanwhile, about 24%, or 45 individuals, never visited for their follow-up MRI.    Women initially deemed to have a less than 2% chance of developing breast cancer often skip the recommended follow-up MRI six months later. Johns Hopkins researchers are attempting to understand why, and recently published some early insights into the issue. They estimated that about 24% of patients categorized as a 3 in the Breast Imaging Reporting and Data System (BI-RADS) did not return for their re-examination. Factors fueling that failure included high out-of-pocket expenses and a lack of any past history of breast cancer in the family, according to the study, published Oct. 8 in the Journal of the American College of Radiology. BIRADS 3 often indicates the need for a 6-month follow-up mammogram. ... If a mammogram is classified into the BI-RADS category 3 it tends to have a very low positive predictive value (less than 2%), meaning the chance of being diagnosed with breast cancer is low. “Our data suggest that there may be subsets of patients who would benefit from additional support and resources to help increase their likelihood to follow through with recommended short-interval follow-up,” concluded Emily Ambinder, MD, a Johns Hopkins radiology instructor, and colleagues. Those could include automated reminders or more concerted communication after the visit, they wrote. Ambinder and colleagues found no connection between sociodemographic factors (i.e., race, marital status, living situation) and compliance. However, they did find that women with higher out-of-pocket expenses tended not to return; noncompliant patients averaged $352 compared to $83 for compliant. Having a family or personal history with breast cancer also made patients more likely to return for an MRI six or so months later, they found. Reasons for the initial examination are also important. For instance, half of the patients who presented to their doctor for “problem solving” were noncompliant with the follow-up MRI. That’s compared to 60% compliance for women who underwent high-risk screenings. There are several noted limitations of the study, according to the authors. The study population was small at just 185 and confined to only Johns Hopkins, and the authors were not able to obtain out-of-pocket costs or education for all subjects. BI-RADS Category System BIRADS or ‘BI-RADS stands for Breast Imaging Reporting and Data System and was established by the American College of Radiology. BI-RADS is a scheme for putting the findings from mammogram screening (for breast cancer diagnosis) into a small number of well-defined categories. Although BIRADS started out for use with breast screening mammography, it was later adapted for use with Magnetic Resonance Imaging (MRI) and Breast Ultrasound as well. BI-RADS 1 means that the mammogram was negative (ie, no cancer) and that you should continue your routine screening. BI-RADS 2 mean?     New blood test detects brain tumors with 87% accuracy

    A new study has introduced a promising blood test that health professionals may soon use to accurately detect brain cancer. Dr. Matthew J. Baker, a reader in the Department of Pure and Applied Chemistry at the University of Strathclyde in Glasgow, United Kingdom, is the lead author of the new research. He and his colleagues have now published their findings in the journal Nature Communications. Of the study, Dr. Baker says, "This is the first publication of data from our clinical feasibility study, and it is the first demonstration that our blood test works in the clinic." Although it is quite rare, brain cancer often has a poor outlook. According to the National Cancer Institute, around 0.6% of people will develop brain cancer or another cancer of the nervous system in their lifetime. However, the 5 year survival rate for those who do receive such a diagnosis is less than 33%. Largely, the poor outlook is due to the fact that brain tumors have very nonspecific symptoms, which makes them more difficult to distinguish from other conditions. Study co-author Dr. Paul Brennan — a senior clinical lecturer and consultant neurosurgeon at the University of Edinburgh in the U.K. — explains, "Diagnosing brain tumors is difficult, leading to delays and frustration for lots of [people]."The problem is that symptoms of brain tumor are quite nonspecific, such as headache, or memory problems. It can be difficult for doctors to tell which people are most likely to have a brain tumor," he adds.The lack of cost effective tests that can help doctors triage people with brain tumors in primary care also means that it takes longer to accurately diagnose brain cancer. This ultimately results in a poorer outlook. The team's new blood test brings much needed hope in this regard. Dr. Baker and colleagues used infrared light to create a "bio-signature" of people's blood samples and applied artificial intelligence to scan for signs of cancer. The test correctly identified brain cancer in a cohort of 104 people 87% of the time.As the researchers explain in their paper, they used a technique called attenuated total reflection-Fourier transform infrared (ATR-FTIR) spectroscopy and coupled it with machine learning technology to detect brain cancer. The authors explain that the technique is "a simple, label free, noninvasive, nondestructive" way of analyzing the biochemical profile of a blood sample without requiring extensive preparation of the sample. The ATR-FTIR technique allowed the researchers to work out a biochemical "fingerprint" of brain cancer.Dr. Baker and team trained a machine learning algorithm to use these biochemical fingerprints to diagnose brain cancer in a retrospective cohort of 724 people. This cohort included people with primary and secondary cancers as well as control participants without cancer. They then used the algorithm to predict brain cancer cases in a sample of 104 participants. Of these, 12 people had cancer, including four cases of glioblastoma. This is one of the most aggressive forms of brain tumor. The findings revealed a sensitivity of 83.3% and a specificity of 87% for the blood test. "With this new test, we have shown that we can help doctors quickly identify which [people] with these nonspecific symptoms should be prioritized for urgent brain imaging," says Dr. Brennan. "This," he adds, "means a more rapid diagnosis for people with a brain tumor, and quicker access to treatment." Hayley Smith — an ambassador for the Brain Tumor Charity in Hampshire, U.K. — adds that it is "very encouraging to hear that this blood test can lead to a quicker diagnosis for brain cancer.

Men may return to work in as little as 2 days Researchers at the University of California in Irvine followed the cases of 11 men who had complications after penis enlargement surgery, also known as Penoplasty. Common complication noticed included: deformity, infection, gangrene, chronic ulceration, and a 'buried penis'.

I am proud of my almost 8 year record as the Mayor and Police Commissioner which has demonstrated my commitment to the working families of our great city to keep them safe. When I first took office our city was in shambles. Due to financial mismanagement of the prior administration our police force was the subject of numerous layoffs and its moral was at an all-time low. As your Mayor and Police Commissioner I restored the police department to its full strength by hiring over 30 police officers. Under my leadership we restored moral and through modern initiatives reduced the crime rate in our city by 17%. Currently the Town of West New York is using an antiquated video surveillance system. There are approximately 50 cameras in use of which 20 are in or around City Hall. It does not make sense that almost half of our cameras are focused on town hall instead of protecting our citizens from crime. Recently I have learned that some of my colleagues are seeking to install more cameras in Town Hall instead of using them to fight drugs, gangs and crime. Sadly, the camera currently being used in the streets are antiquated and many are not working correctly. These must be upgraded or replaced.

    Hoy cubriremos un tema que se escucha mucho en las redes sociaes y es ¿que si Tweeter tiene una tendencia polarizante al socialismo? y por que se penaliza mas a los de pensamientos moderados y de derecha cancelándoles sus cuentas y protegiendo usuarios con pensamientos socialistas sin darles ninguna penalidad ni cancelando sus cuentas. Twitter es una plataforma digital de medios sociales que tiene aproximadamente 336 millones de usuarios y se estima que diariamente 106 millones de personas hacen uso esta cuenta, pero al mismo  tiempo  70 millones de usuarios fueron desconectados de sus cuentas fueron canceladas en el año 2018. Ustedes se preguntarán porque, porque Twitter tiene una tendencia en cancelar muchas  de sus cuentas y existe también una incomodidad en muchos de los usuarios los cuales sienten que Twitter tiene una tendencia polarizarse por la izquierda,  por la parte socialista progresiva los Estados Unidos y que muchas de estas cuentas que  se le quitan a  republicano en Estados Unidos o aquellos que tiene un completamente moderadas en el resto del mundo.  Es por eso porque muchas de las personas que se han manifestado en contra de la estructura democrática este país sobre todo la parte social progresión siente que Twitter los beta los cancela, sobre todo cuando hacen ese tipo de crítica  despectivas que incitan  violencia como el caso de Covington,  como lo vimos con el con el caso de la muerte de aquella persona en Maryland en Florida y sobre todo cuando hacen críticas que incitan violencia a muchos nos no se olvide políticos incluyendo los senadores, como  el senador que fueron sacados de ciertos tipos de restaurantes como le pasó al senador Cruz y también contra varios representantes de la Cámara cómo te fue en el caso de Scalisi.  Muchos de estos casos fueron incitados en Tweeter,  sin embargo Twitter nunca les canceló la cuenta mientras que cuando aquellos que tienen pensamientos moderador y de de acuerdo socialista, tweeter   les cancela la cuenta automáticamente. Entonces la pregunta y el debate que yo les traigo con estos momentos es si Twitter tiene una polarización política?,  si Twitter se polariza Completamente con la tendencia socialista progresiva? y su su tendencia es apoyar al socialismo?  está uno a las preguntas que yo les dejo y me encantaría discutir en este segmento de Arley 360 pod cast.
