Today's Headlines: Trump is reportedly willing to drop his $10 billion IRS lawsuit in exchange for a $1.7 billion government slush fund to pay out political allies — including $230 million for himself, a waiver on all family tax audits, and a public IRS apology — totally extortion. Meanwhile, Louisiana Senator Bill Cassidy, one of the only Republicans who voted to convict Trump after January 6th, lost his primary and became the first senator in 12 years to lose one, because minimal backbone has a price in today's GOP. Because hantavirus clearly wasn't enough to worry about, the WHO declared Ebola a global health emergency after outbreaks in Uganda and the DRC with 246 suspected cases, no available vaccine, and no functioning USAID to help — and the person Trump put in charge of the hantavirus response is a urologist who specializes in penile implants and has promoted pandemic conspiracy theories, so we're in great hands. In other news, Trump's new counterterrorism strategy ranks drug cartels above Islamist militants, considers leftists on par with al-Qaeda, and doesn't mention far-right extremism at all, though Sebastian Gorka did find time to call out Tucker Carlson specifically. In global matters, a commander of Iranian-backed militia Kataib Hezbollah was charged with plotting attacks on 20 Jewish sites across the US, Europe, and Canada, a drone struck a UAE nuclear power plant with no injuries, and Trump told Axios Iran will get "hit much harder" despite having no leverage — while US officials now believe China will invade Taiwan within four years, Cuba has acquired 300 military drones from Russia and Iran and is reportedly eyeing Guantanamo Bay or Key West, and Ukraine launched its largest drone attack of the entire war — 600 drones across 14 Russian regions — in retaliation for Russia's assault last week. Rob shares his story with openness and humour, talking about everything from preparing for surgery and losing weight beforehand, through to dealing with a catheter, continence recovery, penile rehabilitation, Peyronie's disease, and eventually having a penile implant.   Melissa reflects on working with Rob across different stages of his recovery, including using tadalafil, a vacuum pump, injections, and later navigating a period of Peyronie's disease. What comes through strongly in this episode is that recovery is rarely a straight line. Sometimes things improve steadily, sometimes there are unexpected detours, and sometimes the plan changes along the way. That does not mean things have failed it just means the journey is real and sometimes there is a bend in the road(pun intended).   Rob also talks about the emotional side of surgery and treatment. He was not necessarily thinking the worst would happen, but there was still worry about consequences, loss, and what life might look like on the other side. Melissa also brings in Linda, Rob's partner, which adds another important layer to the conversation, the support of a positive, involved partner can make a real difference during recovery.   One of the really valuable parts of this episode is the reminder that there is no single “right” recovery story. Rob had early continence challenges, but improved quite quickly with pelvic floor exercises, walking, and staying engaged in his rehab. He later developed Peyronie's disease, which Melissa discusses in the context of connective tissue tendencies and risk factors, and eventually went on to have a penile implant, which he describes as something he noticed was there but did not find especially troubling and has improved his quality of life.   Rob shares that his PSA is now undetectable, and despite the wobbly recovery path, he and his partner are active, travelling, getting on with life, and doing well. That matters, because many men fear that prostate cancer treatment means life is permanently diminished. In the first segment, Dr. Mistry and Donna Lee answer a listener's question about problems with his existing penile prosthesis. This listener writes, "I had a penile implant put in several years ago and it actually made my penis smaller. When I pump it up, one of the implants in my shaft I'm concerned is going to break the skin or cause pressure problems, which are huge for me because I'm a paraplegic...Can another implant be placed and increase the size of my penis, and how can I fix the problem of too much pressure?" The penile prosthesis is an inflatable silicon device implanted internally and completely concealed. The device is inflated using a pump implanted into the scrotum which, when activated, transfers fluid from a reservoir in your abdomen into the silicon cylinders in your penis to add both length and girth to your erection.  The inflated implant produces a rigid and satisfying erection, yet appears natural and soft when not in use. Dr. Mistry explains that the penile prosthesis is an excellent option for addressing a variety of patient concerns from end stage ED to nerve damage from diabetes or neurologic issues. Importantly, he points out that long-term ED actually diminishes the length of the penis over time, so the penile implant can actually help those struggling to achieve erections--no matter the reason--to maintain their size.
