İlk kılavuzunu 2001 yılında yayınlayan, 2011 yılından bu yana da her yıl düzenli aralıklarla stratejilerini güncelleyen GOLD (Global Initiative for Chronic Obstructive Lung Disease), 2022 KOAH raporunu 2021'in sonunda yayınladı. Bu yazı, kılavuzun birebir çevirisi olmamakla birlikte, önemli bulduğum noktaları yorumlamaya çalıştığım bir içeriğe sahip olacak.

Not too long ago, the Global Initiative for Chronic Obstructive Lung Disease predicted that chronic obstructive pulmonary disease (COPD) would be the third leading cause of death by 2020. Having reached this significant milestone, we speak to consultant respiratory physician Dr Ernest Poh about recent updates on the management of COPD and whether there is any progress in reversing the burden caused by the disease.

AJN The American Journal of Nursing - Behind the Article

This Month in AJN –  September 2018 monthly highlightsSeptember 2018Editor-in-chief Shawn Kennedy and clinical editor Betsy Todd present the highlights of the September issue of AJN. Our first CE, “Too Much Sitting: A Newly Recognized Health Risk,” examines compelling evidence that overall daily sitting time—regardless of whether a person engages in moderate-to-vigorous physical activity—may be an independent health risk for cardiometabolic health conditions, certain cancers, and all-cause mortality. Our second CE, “Managing Stable COPD: An Evidence-Based Approach,” describes chronic obstructive pulmonary disease (COPD) risk factors, clinical manifestations, and diagnostic testing, and discusses how to put the revised Global Initiative for Chronic Obstructive Lung Disease recommendations into practice. In our next article, “Responding to Mass Shootings: Are Hospitals—and Nurses—Fully Prepared?,” nurses and physicians who have experienced mass shootings firsthand discuss the importance of disaster planning. “Mental Health Matters: Antidepressant Medications” provides a brief overview of the indications for use, adverse effects, and nursing considerations related to antidepressants. In addition, there's News, Reflections, Drug Watch, Art of Nursing, and more.

Acute and Chronic Effects of Smoking on Inflammation Markers in Exhaled Breath Condensate in Current Smokers

Background: Long-term cigarette smoking is associated with pulmonary inflammation, but the acute effects of smoking have been less well studied. Analysis of the exhaled breath condensate (EBC) can provide noninvasive markers that might be indicative of inflammation. Objectives: The aim of the study was to determine whether the pH, electrical conductivity and the levels of ammonium and interleukin 8 (IL-8) of EBC were altered in smokers and whether they changed after smoking a single cigarette. Methods: We included 19 healthy nonsmokers (controls), 29 asymptomatic smokers, 10 patients with stable chronic obstructive pulmonary disease (COPD) {[}Global Initiative for Chronic Obstructive Lung Disease stages (GOLD) stages II-III], and 10 patients with exacerbated COPD. In 13 smokers, EBC was also analyzed before and after smoking. EBC was obtained during 10 min tidal breathing with a cooled RTube (TM). pH was determined after deaeration with argon. Results: Acute smoking did not alter the pH or ammonium and IL-8 levels, but raised conductivity. As in COPD patients, the pH was significantly decreased in chronic smokers with a history of at least 10 pack-years compared to controls. Conclusions: EBC can be used to detect the acute and chronic effects of smoking. The increased conductivity of EBC after smoking suggests acute inflammatory effects. The reduced pH in chronic smokers shows cigarette-induced inflammation. Copyright (C) 2009 S. Karger AG, Basel
