What is frailty? Kate Callahan relates a clear metaphor on today's podcast. A frail person is like an origami boat: fine in still water, but can't withstand a breeze, or waves. Fundamentally, frailty is about vulnerability to stress. In 2021 we talked with Linda Fried about phenotypic frailty. Today we talk with Kate Callahan, Ariela Orkaby, & Dae Kim about deficit accumulation frailty. What is the difference, you ask? George Kushel probably explained it best in graphical terms (in JAGS), using the iconic golden gate bridge as a metaphor (Eric and I get to see the bridge daily driving or biking in to work). Phoenotypic frailty is like the main orange towers and thick orange support cables that run between towers. Damage to those critical functions and the bridge can collapse. Deficit accumulation frailty is like the hundreds of smaller vertical cables that connect the thick orange support cables to the bridge itself. Miss a few and you might be OK. But miss a bunch and things fall apart. Resilience is the ability of the bridge to withstand stress, like bridge traffic, wind, waves, and the occasional earthquake (hey it's California!). Frailty research has come a long way. We're now at a point where frailty can be measured automatically, or electronically, as we put in the title. Kate created an eFrailty tool that measures frailty based on the electronic health record (EHR) data. Ariela created a VA frailty index based on the EHR of veterans. And Dae created an index using Medicare Claims. Today we're beginning to discuss not just how to measure, but how to use these electronic frailty indexes to improve care of patients. We should not get too hung up on battles over frailty. As Kate writes in her JAGS editorial, “If geriatricians wage internecine battles over how to measure frailty, we risk squandering the opportunity to elevate frailty to the level of a vital sign. Learning from the past, a lack of consensus on metrics impeded the mainstream adoption of valuable functional assessments, including gait speed.” To that end, modeled after ePrognosis, Dae and Ariela have launched a new tool for clinicians that includes multiple frailty measures, with guidance on how to use them and in what settings. How can national and local leadership work together on solutions to our country’s biggest problems? Recorded in front of a live audience at the Music Hall Loft in Portsmouth, NH, Billy Shore welcomes Senator Jeanne Shaheen (D-NH), Black Trumpet chef and owner Evan Mallett and Dondero Elementary School principal Kate Callahan for a lively discussion that considers issues from both national and local perspectives. Outspoken about the nation’s opioid crisis, Shaheen is leading efforts to get resources to local communities. “We’re not going to jail our way out of this epidemic - we’ve got to help people get the treatment they need to be successful,” she explains. Callahan sees first-hand the effects of the crisis on kids. “Housing has become more transient because of the opioid crisis. [Some students] come from a homeless shelter,” she says. Mallett helped start a job fair for people with addiction-based criminal records. “The idea is to introduce restaurants and farmers to candidates who need a second chance, and to hopefully restore their dignity through employment,” he says. Known for bringing civility and sophistication to public affairs, Shaheen is troubled by today’s political climate. “We have to be able to listen to each other and figure out how we can compromise and work together. We’ve got to make sure that our leaders are as good as our people in this country,” she stresses. Mallett believes one of his roles as a chef is to help facilitate civil discourse. “I feel like food is the ultimate democratizer and the ultimate way to have people sit down and have a conversation. Delivering the food and witnessing that exchange is a truly meaningful part of what I do,” he says. The Brain Injury Alliance of Connecticut and Trinity College joined together to offer survivors of brain injury a free day of relaxation, friendship and fun at the BIAC Healthy Living Retreat for People with Brain Injury. This event was held on Friday, May 29th,at Trinity College in Hartford. Survivors were able to spend the day participating in workshops in Tai Chi, Art and Music Therapy, Yoga, Massage, and Diet and Nutrition and enjoyed a musical performance by Kate Callahan. Kate Callahan was named the Connecticut State Songwriter of the Year and was named was named one of 2014's Top 100 Unsigned Artists by Music Connection Magazine. The Boston Globe describes her as "An award-winning singer who rose from a calamitous injury to claim her creative life, Kate Callahan has garnered an appreciative audience with her easygoing vibe and inspirational, at times mystical lyrics.”
Nominated for a 2014 New England Music Award, Kate Callahan, Connecticut's “Best Singer-Songwriter 2013” has three albums including her recent release Two Doors, which was featured on WNPR's Where We Live. Callahan's music feels soulful and she's unapologetically optimistic in concert. WNPR host Colin McEnroe praised a recent performance, saying, “Kate Callahan is the only performer I've ever seen who occasionally seems to be channeling something vaster and far more ancient than herself, something more easily understood by Emerson than by any modern person. Kate's credits include appearances at the Falcon Ridge Folk Festival Emerging Artists Showcase, the New Haven Festival of Arts and Ideas, and the CT Folk Festival. She has opened shows for folk legends Judy Collins and Noel Paul Stookey (Peter, Paul, and Mary), troubadours Aztec Two-Step, John Gorka, Mustard's Retreat, and the late Bill Morrissey. She has also shared stages with folk-rock artists Rachael Sage and Regina Spektor. Stookey calls Callahan "real" and says her lyrics are "Zen-like."
