In this episode: Dr. Chandarana joins us is today's episode to explain what endocrinologists do, and how they play a key roll in treating diseases like diabetes and thyroid disease. In this episode we will be focusing on the thyroid as it relates to pregnancy. As most of you know if you have been following me on social media, I was diagnosed with thyroiditis after having Miles. It was an uncertain time filled with so many varying symptoms that made me feel like my body was totally out of control. But once I was able to get some answers, we were able to take the correction action towards getting me better. What is the thyroid and what is it responsible for? The thyroid is a small gland in the body that is located centrally in the neck, and is responsible for synthesizing thyroid hormone. Thyroid hormone is necessary for the functioning of the body, controls metabolism in adults, and is critical for brain development in children. Some babies are actually born without a thyroid, so they would need to take thyroid hormone for proper growth and brain maturation. Early on in a pregnancy: Sometimes the pituitary gland can secrete a hormone called Beta ECG that is similar in structure to the thyroid stimulating hormone. When Beta ECG binds to the thyroid it can trick the thyroid into thinking it's being stimulated and causes a slightly overactive thyroid. Endocrinologist consider this to be normal early on in the pregnancy. Women might also experience some swelling in their neck (goiter) due to the thyroid being overactive. Endocrinologist want to be able to distinguish between these changes going on with what is considered “pathological changes” (these need to be treated/monitored). Some examples are: Underactive thyroid (hypothyroid): Some symptoms may be low energy, fatigue, muscle aches, brain fog, difficulty concentrating, weight gain, constipation, swelling of legs, cold intolerances, hair loss, irregular menses. Hashimoto's: Autoimmune thyroid disease, runs in the family Overactive thyroid (hyperthyroid): Some symptoms may be weight loss, anxiety, tremors, palpitations, increased sweating, heat intolerance, diarrhea, hair loss. Thyroiditis: Can be painless and also painful. If a woman experienced postpartum thyroiditis, they're more at risk for developing it again with subsequent pregnancies. Graves disease: overproduction of thyroid, will not usually get better without treatment. IN THIS EPISODE: I was referred to our guest speaker (Robin Forgione) by my Obstetrician, and thought it would be really beneficial to bring her on the podcast since she's helped me work through one of the hardest times in my life. She held a space for me where I was able to say whatever I was thinking, and had the knowledge, tools, and resources to properly support me. Remember when listening to this episode that if you are struggling with symptoms while pregnant or postpartum, you should seek the care and guidance from your doctors so they can choose the best plan of action for YOU. In this episode Robin will share with us the different types of clinicians that are available, and why if you're going through any postpartum issues, why it's more beneficial to seek the support from someone in a specialized field. I think it's important to note, that Dads may also be going through their own struggles bringing this new baby into the family. They too see counselors alone, or as a couple. Although Robin has seen clients with depression, the main theme Robin has seen with pregnant patients is anxiety. And a lot of the time it's because they've had a history of anxiety that may have contributed to it. As far as postpartum, the bulk of patients that she sees struggles with depression and anxiety, and some OCD. Although some women may not know this, if you do suffer from depression/anxiety or have a history of depression/anxiety there are medications that are safe to take while pregnant and nursing. Some women think they need to get off their meds, but that isn't always the case. Robin says she has actually seen women do better if they don't get off the medication once they find out they're pregnant. Again, this is all under the direction of YOUR doctor. Most important things Robin thinks a mom should have for support: Identify a support network that you can talk to Good dialogue with your partner FLEXIBLE MINDSET MANTRA: You're doing a REALLY good job. DEFINITIONS: Baby blues: Most new mothers — up to 80% — experience a wide range of emotions, including sadness and irritability, during the first few weeks after baby is born. Often referred to as the Baby Blues, this is a normal adjustment as pregnancy and postpartum hormones calibrate. Symptoms usually last 2-3 weeks and resolve without medical intervention. Learn more about the baby blues. Depression: women experiencing depression during pregnancy or the first year postpartum might have feelings of anger, sadness, irritability, guilt, lack of interest in the baby, changes in eating and sleeping habits, trouble concentrating, and thoughts of hopelessness and sometimes even thoughts of harming the baby or herself. Learn more about depression including risk factors, symptoms and treatment options. A woman with anxiety may experience extreme worries and fears, often over the health and safety of the baby. Some women have panic attacks and might feel shortness of breath, chest pain, dizziness, a feeling of losing control, and numbness and tingling. Learn more about anxiety, including risk factors, symptoms and treatment options. Psychosis: Individuals experiencing psychosis sometimes see and hear voices or images that others can't, called hallucinations. They may believe things that aren't true and distrust those around them. They may also have periods of confusion and memory loss, and seem manic. This severe condition is dangerous so it is important to seek help immediately.Postpartum psychosis is a very serious emergency and requires immediate medical help. In this episode: Kelly starts off this episode with telling us about how her journey with yoga and meditation started. In the beginning she was more into the physical aspects of yoga and admits she was a “shavasana skipper”, but when she started doing it with her mom after she was diagnosed with Stage 3 breast cancer, she started getting more into the “softer side” of yoga. Yoga has been a way for her to escape those hard times that have come up in her life. We live in such a loud society, with tons of stimulation, and issues beneath the surface keep getting pushed down, and we force ourselves to stay busy so we don't have to deal with those issues. A lot of people don't start meditation because of the misconception that they have to turn off their mind and sit in silence and that feels hard for them so they don't start. Separate yourself from the idea that there's good and bad feelings, and that there are just feelings. In today's episode I interviewed a friend named Chelsea who I met at a 5k race when we first moved to North Carolina. Chelsea talks about how she and her husband moved to North Carolina for the military, but she also had the dream of starting her own career and opening up 5 gyms. In 2018 she gave birth to her first son River at 22 weeks and was able to spend a couple of hours with him before he passed on. She now has a 1 year old son named Axel. Chelsea expresses how hurtful it is when people are too scared to talk about her experience with her and will eventually just change the subject. She gives some tips on how to have that conversation with people. Chelsea was diagnosed with “incompetent cervix” after her first pregnancy, and with this crucial information she was able to take the right precautions with her second pregnancy. She expressed that with her second pregnancy she put her walls up and didn't want to bond with the baby until she was absolutely sure the baby was going to actually be a part of their lives after having experienced the loss of her first child. She set up milestones to “look forward to” and at 28 weeks she was able to let those walls down and bond with Axel. In women with a prior spontaneous preterm birth and who are pregnant with one baby, and have shortening of the cervical length less than 25 mm, a cerclage prevents a preterm birth and reduces death and illness in the baby.[1] The treatment consists of a strong suture sewn into and around the cervix early in the pregnancy, usually between weeks 12 to 14, and then removed towards the end of the pregnancy when the greatest risk of miscarriage has passed. The procedure is performed under local anaesthesia, usually by way of a spinal block. It is typically performed on an outpatient basis by an obstetrician-gynecologist. Usually the treatment is done in the first or second trimester of pregnancy, for a woman who has had one or more late miscarriages in the past.[2] The word "cerclage" means "hoop" in French, as in the metal hoop encircling a barrel.[3] There is no evidence that cerclage is effective in a multiple gestation pregnancy for preventing preterm births and reducing perinatal deaths or neonatal morbidity. Definitions of cervical weakness vary, but one that is frequently used is the inability of the uterine cervix to retain a pregnancy in the absence of the signs and symptoms of clinical contractions, or labor, or both in the second trimester.[1] Cervical weakness may cause miscarriage or preterm birth during the second and third trimesters. It has been estimated that cervical insufficiency complicates about 1% of pregnancies, and that it is a cause in about 8% of women with second trimester recurrent miscarriages.
