    Today's episode is a special one. I got to sit down in person with Tristin Colley, formerly known as Tristin Van Ord, for a conversation that felt personal, honest, and really meaningful. Tristin is a professional marathoner, a two-time Olympic Trials Marathon qualifier, and currently pregnant with her first child, a baby girl due this April. She made her marathon debut in 2019 running 2:40 and has steadily worked her way down to a 2:25 personal best, a testament to her patience, belief, and long-term commitment to the sport. In this episode, Tristin and I talk about the many transitions she is navigating right now. From stepping away from the ZAP Endurance program following its recent closure, to moving with her husband Andrew to Raleigh, to entering motherhood while still holding big goals as a professional runner.     As BMIs and weights increase across the US population, there have been increased calls for universal screening for existing DM at entrance to prenatal care, if under 20 weeks. Others, including the ACOG, prefer to screen early those with additional risk factors (like prior GDM HX, prior macrosomia, BMI >30, PCOS, first degree relative with diabetes, or age >40). In July 2024, the ACOG released its publication, “Screening for Gestational and Pregestational Diabetes in Pregnancy and Postpartum”. In this guidance, it states, “At this time, there are insufficient data to support the best screening modality for pregestational diabetes in pregnancy, but consideration can be made to use the same diagnostic criteria as for the nonpregnant population (A1c value 6.5 or higher, or fasting plasma glucose value 126 mg/dL or higher, or 2-hour plasma glucose value 200 mg/dL or higher during a 75-g OGTT, or random plasma glucose value 200 mg/dL or higher in patients with classic hyperglycemia symptoms)”. However, a new proposed protocol has been published in AJOG for early screening for DM in pregnancy. This also describes the differences in diagnosis and care for Standard GDM diagnosed at 24-28 weeks, vs a diagnosis of pregestational DM diagnosis made prior to 20-weeks vs “early” GDM also diagnosed under 20 weeks of gestation.     There is much to consider about the intersection of perimenopause, ADHD, and postpartum. With more and more people choosing to have babies later in life, there is the likelihood that the start of perimenopause may coincide with the perinatal period. Those with ADHD have another variable thrown into the mix. We are tackling these topics with our expert guest today. Please join us! Kara Cruz is a Licensed Marriage and Family Therapist, Perinatal Mental Health Specialist, private practice owner, clinical supervisor, women's mental health advocate, and educator. Her areas of focus are reproductive mental health, ADHD, complex trauma, and mood and anxiety conditions. In addition to psychotherapy with individuals and couples, Kara also facilitates continuing education and mental health training for professionals. One of her favorite projects is her Therapist Group Bootcamp, where she teaches and coaches fellow therapists on building their own wellness and support groups. Kara is an EMDRIA-certified therapist who leads workshops, including her signature group for women, “It Begins With Us.” She is the co-owner/co-founder of Women's Support Circle, a space for community, resource sharing, and support for women worldwide. She created the Wellness for Women Healers Community, a space for healers of all professions and backgrounds who share the common goal of promoting women's healing and wellness.     The postpartum period is one of the most dramatic hormonal transitions in a woman's life, yet it is rarely explained in a clear or compassionate way. In this episode, Teresa and Jamie walk through what is actually happening inside the body and brain after birth, why so many women say they don't “feel like themselves,” and how much of this experience is rooted in biology , not personal weakness or failure. They begin by unpacking the role of prolactin, the hormone responsible for breast milk production. While prolactin supports bonding and nurturing behaviors, it also suppresses ovulation and lowers dopamine, the brain's motivation and reward chemical. This helps explain why many postpartum women feel more emotionally flat, less driven, or disconnected from activities they once enjoyed, even while deeply loving their baby. Next, they discuss the massive drop in estrogen and progesterone that occurs within the first 24–48 hours after delivery. During pregnancy, these hormones are higher than at any other time in a woman's life. Their sudden withdrawal can contribute to mood swings, anxiety, tearfulness, sleep disruption, and the baby blues, and in some women may play a role in postpartum depression or anxiety. Progesterone's calming effect on the nervous system and estrogen's support of serotonin help explain why this shift can feel so emotionally intense. The conversation then turns to dopamine and how its suppression during breastfeeding affects motivation, pleasure, focus, and emotional resilience. Teresa and Jamie emphasize that it is possible to feel grateful, bonded, and deeply loving toward your baby while also struggling internally , and that these experiences can exist at the same time. They also explore why postpartum is uniquely demanding when you combine hormonal changes with sleep deprivation, physical healing, and identity shifts. Together, these factors create one of the most neurologically and hormonally vulnerable seasons of a woman's life. The episode then transitions into the topic of fertility after birth. Many women are told that breastfeeding prevents pregnancy, but ovulation always occurs before the first postpartum period, meaning pregnancy is possible before cycles return. Prolactin levels vary widely depending on nursing frequency, nighttime feeds, pumping, stress, and individual biology, making fertility unpredictable during this time. Finally, Teresa and Jamie discuss how natural family planning and fertility awareness methods can be used postpartum, but require special education and realistic expectations. Cervical mucus patterns can be irregular, estrogen may rise multiple times before true ovulation, and the body may have several “practice runs” before releasing an egg.
