Healthcare quality professionals are integral leaders in improving quality and safety while preventing harm. Unfortunately, hospital leaders report high turnover in these positions. Frequently, when a new quality leader is appointed, the previous quality leader has already left the organization creating a knowledge gap. The Iowa Hospital Association’s Quality Residency Program is designed to provide knowledge, support and networking opportunities that will position quality leaders for success. This is a two-year program that will include in-person learning sessions, coaching calls, pre- and post- learning assessments and interactive discussion among quality leaders. Jennifer Nutt, senior director for nursing and clinical services and Kathy Trytten, vice president of information and quality services with the Iowa Hospital Association will share information on the Quality Residency Program.

Interviewing IHC's 'Fan Favorite' poster winner - Van Diest Medical Center. In 2010, Van Diest Medical Center (VDMC) implemented a multidisciplinary Readmissions Team which meets monthly to retrospectively review the readmissions. A need for a proactive versus reactive approach to manage patients with unmet healthcare needs emerged. The Team hypothesized that, by pre-identifying patients with high utilization of unplanned care, VDMC could manage and prevent progression of illness putting patients at risk for a 30-day readmission. A workgroup -- Continuity of Care -- was chartered to manage patients with frequent unplanned visits that have unmet primary care, social, financial and mental health needs. An informatics tool that concurrently flags high utilizers of acute, unplanned care, such as ED visits, observation stays and inpatient admissions was built to identify patients in need. Bi-weekly meetings are held to network on unmet needs and optimize management in the outpatient setting. Patient-specific interventions are implemented to address identified needs, and adjusted as necessary. The Team collects and audits data bi-weekly. Current data exhibits a reduction in unplanned encounters, inpatient days, and cost of care. This supports the Team’s hypothesis that a proactive approach has the capacity to make an impact on population health, chronic disease management and promote a cost effective care management solution. In this episode we interview Sarah Derr, Pharm D and Medication Management Lead at the Iowa Healthcare Collaborative to announce a new resource available to hospitals, clinics, pharmacies and communities throughout the state of Iowa. In this episode we will interview Kevin Gabbert from IDPH regarding the new mandatory reporting of an Opioid Overdose requiring Naloxone Administration made active on July 1, 2018. Kevin will provide Compass HIIN with relevant background information on this mandate as well as a general overview of requirements. We will end with Kevin sharing what has been learned from reporting so far, addressing commonly asked questions and concerns and giving insight as to how the state plans to use the data.
