The New Anti-Robocall Technology is Coming Online I know everybody I talk to hates these robo calls. They're happening all of the time. What are we doing about it? The FCC has made some changes and they just went into place. So we're going to talk about STIR/SHAKEN, right? Of course the government has to come up with some really cute acronyms and that's what's turned, shaken are, but here is what's happened. We have the ability to over TCPI P send phone calls. In fact, in my phone, I have a phone that's hooked up to T-Mobile and it is using wifi. [00:00:38] So when I'm making a phone call, it is going over my wifi in the office or at my house or wherever. It doesn't even use the regular salad or network, unless it has to why? Because it's all TCP IP. And then on top of it, sometimes with LTE and now always with five G you are using TCP IP. So your voice is converted into a digital signal. [00:01:10] Right there on your phone. And it is handled as data, just like pretty much any other data would be handled. Now our cell phones, of course, for a long time have been digital and have been in coding it, but they have not been using it as regular TCPI peop. So what the federal communications realized is since most of the tracks. [00:01:35] That we have on our phone networks nowadays is actually internet traffic. It's TCP IP. Why don't we add a little bit to the protocol? And they came up with this whole new way of being able to track the originator of a phone call right now, if you have a landline, this is not going to benefit you at all. [00:02:02] And we're going to talk about the different carriers here, but they don't have to comply these landline operators for another couple of years. Although the FCC right now is circulating a petition and is trying to make it so they have to comply. More quickly, but the nation's largest phone companies have met this deadline that was set by the FCC to put this new anti robocall technology in place. [00:02:31] But I want to warn you guys that does not mean that it is going to stop all of these robo calls anytime. Soon where it's not going to stop, but you might have noticed that at the end of June, there was a noticeable cutback in some of these robocalls. Now these are the scammers that are calling us and are pretending that they are a local phone number, right? [00:02:59] Their biggest trick is, yeah, I've got the same area code and first three digits, I'll call it a prefix as you do. So you should answer that phone. They announced at the FCC last Wednesday that quote, the largest voice service providers are now using stir shaken caller ID authentication standards in their internet protocol networks in accordance with the June 30th deadline set by the FCC. [00:03:26] This widespread implementation helps protect consumers against malicious. Spoofed robo calls and helps law enforcement track the bad. So it, I think is a very good idea. It's a step in the right direction. It is going to help a whole lot at T and T Verizon T-Mobile and us cellular have all put these in place. [00:03:54] In March, the FCC denied petitions for deadline extension from Verizon and us cellular are saying that they didn't meet the high standard of undue hardship. I think that makes sense. You've got to force these guys. Sometimes Verizon was saying that they had a small area of their fiber based home phone network that would not be able to meet it. [00:04:17] So again, it's landline. Versus the home phone that work, many of us have it. Comcast provides it as well, but basically if it's it IP based, they didn't have any sort of exceptions. So Verizon is now exchanging the stir shaken enabled phone calls with wireless carer carriers. It represents about 80% of the U S wireless industry. [00:04:45] Verizon said this week, so 80%. Yeah, you should have seen a bit of a drop more than 135 million calls a day are currently being exchanged between Verizon and the other carriers with the number of calls that are being exchanged using this tracking technology grow. It's also been deployed on IP enabled wire line phone networks operated by Comcast charter at and T Verizon and many others. [00:05:17] So we're pretty happy about this. We'll see what ends up happening in general, but some care companies that carry a lot of robo calls are not yet required to follow the stir shaken technology and the rules around. Because there's an exemption for carriers with a hundred thousand or fewer customers. So that's going to be a bit of a problem, but think about how you have to roll out technology. [00:05:46] Rolling it out via these big carriers and doing that first is seeing where the holes are in the technology. What's not working properly. That's the way to handle it. And then by having the big carriers do it, they're going to bear the brunt of the expense. Because always right. Those big screen TVs used to be five to 10,000. [00:06:12] I remember $15,000. That's a very big deal. And then it goes down and down. Now you can buy them at Walmart for 300 bucks. So having the big guys do at first makes a lot of sense. It's going to prove the technology, improve the technology, drive the costs down, and then you can bet that these smaller carriers are also going to be required to do this fairly soon. [00:06:38] So the way the protocols are working is they are using tokens and to verify. The accuracy of the caller ID. So they're using public key cryptology. For those of you who know what that is using digital certificate, getting major us companies to adopt. It really was very big milestone. I'm very glad they finally did it by the way. [00:07:01] Stir, shaken. Ordered by Congress and FCC chairman as PI. You remember him? He did a lot of great things to help out communications, but he was only having voluntary compliance. And so it didn't lead to like widespread adoption. So this law really forced them into it. And they're now. So we'll see what happens. [00:07:28] Small carriers are exempt for now. Landlines. Aren't doing it. There's a lot of the TDM based stuff out there, but it's going to happen. There's also a gap in, at and T network right now. So we'll see. What ends up happening? 1. Microsoft, YouTube and Google Announce Their Advertising Revenue Figures - Google leads with a 32% increase in ad revenue, compared to Microsoft and their 17% increase in ad revenue. Meanwhile, YouTube is also growing with a 49% jump in ad revenue, followed by a 86% increase in daily views of YouTube Shorts. 2. LinkedIn’s Continued Growth Reaches 25% Revenue Increase - Since Microsoft acquired LinkedIn back in 2016, we only see brief overviews of the platform. Some of the impressive growth figures, besides revenue, include 43% increase in conversions; 29% increase in content sharing, and 80% increase in hours spent on LinkedIn Learning. 3. A Promising End Of Q1 For Facebook And A Questionable Future For Q2 - In terms of revenue and users, Facebook did exceptionally well and showed an increase in both metrics. A presentation on FB's Earnings illustrated a 2.86 billion increase in monthly active users (MAU), which is around 10% increase. However, despite this success in Q1, it is questionable what the numbers will be during Q2. Now that Apple has released iOS 14.5 and users will need to give permission to have their data and online activity tracked, there are speculations that the revenue of Facebook Marketplace will go down. 4. Pinterest Reveals What Users Are Planning, With a 96% Increase In Overall Searches - Some of the searched terms predict that people are preparing for a travel boom; looking for outfit inspirations; searching for party-related entertainment, and planning home renovations. Pinterest users all share 1 common characteristic - a positive attitude towards what is coming post-Covid19.5. Shopify’s Impact On The Economy - According to Deloitte’s report on Shopify’s Economic Impact, the platform has provided $2 billion in funding to entrepreneurs and 3.6 million jobs around the world. Moreover, the economy has received $307+ billion from Shopify merchants, and that makes them the 7th largest company in the world in terms of revenue. 6. TikTok Launches Lead Generation Ads - These ads will enable businesses of all sizes to create seamless interactions, gather audience information, and reach prospects in order to convert them into potential customers. Users will be able to provide their name, email and telephone number if they have an interest in a given product or service. Businesses will also be able to create fully customizable messages that are relevant to multiple customer segments. 7. How YouTube Creators Are Generating Revenue - YouTube’s efforts began 13 years ago when they launched their Partner Program. The statistics show that over 140K channels earned money from Paid Digital Goods on YouTube in December 2020, which is a 140% increase compared to December 2019. At the same time, creators on the platform earned over 4x as much revenue from channel memberships in 2020, compared to the previous year.8. Google Updates Customer Match Lists - Customer Match help advertisers build a stronger relationship with their customers and Google has introduced some updates to improve that feature. Advertisers can now see how much of their list is usable in real-time; and they can also keep their lists up to date with Recommendations.9. Google Gives Guidance on Apple’s ATT Policies - iOS apps submitted on or after 26 April must comply with Apple’s ATT policies and ask for permission when using certain information from other companies’ apps and websites. Here are steps that ensure a smooth transition, for advertisers promoting web-based conversion goals: 1) Understand the impact of ATT on Google Click ID, website and offline conversions 2) Take advantage of Google’s new innovations that can help with measurement on iOS 14 3) Expect performance fluctuations and monitor delivery and bid settings closely And this is how advertisers promoting iOS apps can ensure a smooth transition: 1) Evaluate whether Apple’s ATT prompt is right for your app 2) Implement Apple’s SKAdnetwork API 3) Consolidate iOS App campaigns and use only tCPI or tCPA bidding: 4) Get reporting insights for your iOS App campaigns 10. Google Reports An Increase In Search Spam Detection - According to data, there is an increase of 60% in spammy pages, compared to 2019. However, Google has improved in detecting spam and blocking it as a result, from detecting 25 billion pages of scam each day in 2019.

How do you break down the barriers to better mental health in a pediatric setting? Dr. Julie Peek from Terrace Pediatrics in Nashville, TN, joined our Best Practices hosts for our latest podcast episode discussing behavioral health integration. Terrace Pediatrics has been treating generations of patients since the 1960s, but only since joining TCPi has the practice worked toward enhancing care in a way that gives providers a broader picture of the influences on a young patient’s health. The practice began by incorporating post-partum and adolescent depression screenings, but the providers were left figuring out what to do with the results of those screenings. Working with TCPi offered Terrace Pediatrics the opportunity to partner with a local chapter of the Mental Health Co-Op to bring a mental health professional into the clinic. Behavioral health integration, Dr. Peek advises, has to begin with a physician advocate, with clinical involvement and leadership, and with open lines of communication for a practice to successfully enhance their care in treating the whole patient. Siloam Health faced the hurdle of improving efficiency and effectiveness of the patient flow process. Claire Reagan, Nurse Manager at Siloam Health in Nashville, TN, joined Dr. Thomas Spain, primary care physician and Director of Practice Transformation at the Midsouth Practice Transformation Network, and Kirkland Ahern-Jones, Director of Operations for the Midsouth PTN, to discuss the effort the practice made to implement team huddles and the difference huddles made in the practice’s workflow and experience as part of their TCPI work. By initiating a clinical team huddle, Siloam Health moved to the next level of team based care. Taking the time to establish the huddle habit and to encourage providers to collaborate about patient care, Siloam Health improved both the team and the patients’ experiences and further developed relationships between patients and the entire team. Team huddles led to a 20% improvement in colon cancer screening over baseline, moving the practice performance above the national MIPS benchmark in just 18 months. The practice improved dramatically on mammogram screenings, showed a 15% over baseline in administration of Pneumovax to elderly and diabetic populations, and better incorporated behavioral health in patient care. With the implementation of daily huddles, Siloam Health is able to work as a more organized team to serve each patient’s unique needs. The Best Practices podcast explores the exemplary work done by practices in the MidSouth Practice Transformation Network over the last three years. Throughout the episodes, the podcast explores quality improvement efforts, but what are these continuous process improvement methods? Kirkland Ahern-Jones, Director of Operations for the Midsouth PTN, interviews her cohost, Dr. Thomas Spain, primary care physician and Director of Practice Transformation at the Midsouth Practice Transformation Network, to explore the continuous process improvement methods and why they are needed. The efforts of the MidSouth PTN were underpinned by the knowledge that to thrive in a value-based payment environment, practices needed to learn how to continually improve and how to use data to drive that improvement. There are a number of continuous process improvement methods, but the two most common in healthcare are Lean Process Improvement and the IHI Model for Improvement. Each method includes preparation practices need to make before they undertake improvement and before they begin making changes. Continuous process improvement methods are essential when striving to enact change, because these methods provide a feedback cycle that enables practices to learn from the changes being put in place and to allow what they have learned to influence the changes made in the future. The IHI Model for Improvement is simple and universal. The Institute for Healthcare Improvement provides a wealth of resources online, and their Model for Improvement begins with two parts. The first part is a trio of questions that aid a practice in setting aims, establishing measures, and selecting changes. A practice’s aim should be quantifiable; it should be specific, measurable, attainable, relevant, and time-frame bound. How a practice will measure to know a change is an improvement is a step that is often left out, but it is pivotal to the use of data to drive improvement. The second part of the first step of the IHI Model for Improvement is the Plan-Do-Study-Act (PDSA) method for testing change. For those interested in quality improvement, we invite you to explore the continuous process improvement methods. Dr. Spain’s best advice for those just beginning their healthcare improvement journey: Start small. High value healthcare is safe healthcare, and studies estimate upwards of 90% of patients seen in an outpatient setting have medication errors in their record. Dr. Amanda Mixon from Vanderbilt University Medical Center in Nashville, TN, joined our Best Practices hosts to discuss the quality improvement efforts and patient safety intervention methods of medication management. It is important for practices to understand that every time an additional medication is prescribed, there is an inherent risk. Medication management should be patient-centered, and patient education plays a pivotal role. The patient should be educated on why he or she is taking the medication, on how the medication should be taken, and on when the practice should be notified if problems arise. The three main steps of medication management in an inpatient setting can be translated to an ambulatory setting. The first critical step in the beginning of the process is obtaining a medication history for the patient. The next step is one of reconciliation: practices should take the medication history and compare it to what the provider wants to change. Finally, the provider needs to both document changes to the medication list in the medical record and communicate any changes to the family and caregiver. The best data collected thus far in estimating the magnitude of medication errors and the downstream repercussions still underestimate the problem because it is underreported. Studies show that roughly two-thirds of patients being discharged from inpatient care to outpatient care are at risk for having medication errors. Time and staffing present the biggest challenges in this transformation effort, but Health Information Exchanges, investing in an on-site pharmacist to monitor and intervene, and a concerted de-prescribing effort can play a critical role in medication management. Lane Stiles, Director of Patient Education for Vanderbilt University Medical Center, joined our Best Practices hosts to “SHARE the Knowledge” about shared decision making. Shared decision making is a patient engagement strategy that focuses on creating collaborative conversations between the patient and provider. The current healthcare climate places patients in a reactive role, but shared decision making is an invitation to patients, offering them a reason to actively participate. When patients are involved in their healthcare and own their decisions, studies have shown that patients are more likely to follow a care plan, to take their medications as prescribed, to be satisfied with their care, and to accept outcomes, even adverse ones. This approach to engage patients solves numerous downstream issues and allows providers to personalize and communicate risk. Shared decision making places the patient at the center of their healthcare team. Shared decision making is a simple concept, one that makes sense and is a rational patient engagement strategy, but it requires practice for providers and teams to do it well.
