“ipsa scientia potestas est” This is FOAMed. Thus there are a lot of people that are nothing short of obsessed with airways. It’s as if humans were primarily a giant "Pass the Parcel" game with a super difficult intubation inside. This will allow us individually to be the airway super-hero that we dream we could be. But we don’t have super powers. None of us do. We do have some information though and information is power. NAP-4 out of the UK was the product of information collected by the College of Anaesthetists in the UK. Before that was published Toby Fogg had already started collecting airway data at the Royal North Shore Hospital in Sydney, Australia and beyond. Toby spoke to Minh Le Cong on his PHARM podcast 5 years ago. This rather helped get the word out and he followed up with a post on Lifeinthefastlane. https://lifeinthefastlane.com/do-you-know-if-you-really-own-the-airway/ 5 years later we caught up with him to find out how it is all going. Toby is still collecting the data. Some people collect stamps, some collect beer glasses. Toby collects airway data. He's been at it for about 7 years now with the cooperation of a significant number of hospitals around the country. He has been able to see the changing trends in Australian airway management from the publication of the NAP-4 audit to emerging practices such as Delayed Sequence Intubation, Apnoeic Oxygenation and the increasing use of ketamine and rocuronium for RSI. He has also had to learn the intricacies of setting up a database for potentially sensitive information. In the background Luna Park was in full swing with music playing and rides running, which reflected the rollercoaster ride that Toby has undertaken. Sounds like a laugh. After our talk with Minh Le Cong from the PHARM podcast, we decided it was appropriate to discuss the POKER Trial.  No, it is not about cards, but rather about if propofol mixed with ketamine is better than propofol alone. In addition to this podcast, there have been a few other blogs such as St. Emlyn's and REBEL EM that reviewed this study. At this time, we believe that this is the only podcast to create an audio discussion in detail on the subject (we may have missed someone and we apologize). For that reason, we wanted to make sure there was a way you could listen to a detailed analysis of this study soon as it is still a relatively recent publication.

Video laringoskop son derece pahalı ve günümüz sağlık sisteminde yöneticilerin acil servislere almak istemediği cihazların başında geliyor. Cep telefonlarımızın bile mükemmel videolar çektiği zamanlarda olmamıza rağmen basit bir video laringoskop ucundaki kamera sisteminin bu kadar pahalı olması akıl alacak gibi değil. Geçtiğimiz senelerde özellikle mali durumu yetersiz yerlerde en azından eğitim amacıyla kullanmak için ev yapımı video laringoskop konusu FOAMed dünyasında gündeme gelmişti. İlk defa bir ambulans hekimi olan Dr. Malcolm Russell tarafından bildiğimiz inşaat boroskopu ile bir deneme yapılmış ve bu denemeyi youtube'dan izleme şansı bulmuştuk. Bu boroskop 100 USD civarında bir maliyete sahipti ve o zaman ülkemizdeki fiyatı da 300-400 TL arasındaydı. https://vimeo.com/62716322 Ama teknoloji hızla ilerledi ve ucuzladı. Dr. Minh Le Cong (@ketaminh) ve Dr. John Hinds (SMACC'de tanıştıktan bir ay sonra kendisini ne yazık ki elim bir motorsiklet kazasında kaybettik) geçen sene ucuz yollu video laringoskop yapmak için türlü denemeleri kendi bloglarında paylaşmışlardı. Ben de konuyu yakından takip eden biri olarak 15 gün önce son derece ucuz (8,84 USD yani 30 TL'nin altında!), su geçirmez, 5 mm kalınlığında ve 2 metre uzunluğunda, ışık miktarı ayarlanabilen bir USB endoboroskop görünce hemen satın aldım ve ilk denemeyi yaptım. Elime dün geçen endoboroskop kaliteli bir kabloya sahip. Mac ve Windows bilgisayarlar cihazı takınca direk USB 2.0 kamera olarak tanıyor. Mac'de Quicktime içerisinden USB camera diye seçerek hemen görüntü alabiliyorsunuz. Windows 7 ve 8 için ise bilgisayarınıza endoskop ile gelen bir cd'deki programı kurmanız gerekiyor. Malzeme kalitesi orta seviyede, öyle yıllarca dayanacakmış gibi bir hali yok aslında (bu sebeple ben de sipariş verirken 2 tane vermiştim).  Işığı çok güçlü, görüntü 480p, 0,3 megapiksel. Yani selfie çekerken kullanılan iphone ön kamerasına denk bir kalitesi var. Laringoskop ucuna bağlarken birkaç santim geride tutmak gerekiyor, bir de üzerinden üstü neresi altı neresi belirtmediğinden kameradan bakıp tam üste gelecek şekilde sabitlemelisiniz. Sabitleyince ise aşağıdaki görüntü alınıyor. Zaten alışık olduğunuz Mac blade kullanmak en büyük avantaj, sadece ucuna kamera eklemiş oluyorsunuz. Hasta üstünde denemek gibi bir durum söz konusu olmadı ancak üstünde çektiği elektrik son derece düşük voltajda, ayrıca su geçirmez, yıkanıp, alkol bazlı sterilizasyon sıvılarında bekletilebilir olup olmadığını test etmek gerekiyor. Ancak şu haliyle video laringoskopu olmayan kliniklerin maket eğitimlerinde rahatlıkla kullanabilecekleri, 30 TL'lik bir video laringoskop sahibi oldunuz. Hayırlı olsun.

Minh Le Cong and Chris Zammit banter about the use of ketamine in patients with traumatic brain injury (TBI).  Then, we delve into core content pearls on concussion/mild TBI including diagnosis, treatment, and complications using Tintinalli 254 but to be honest, Rosenalli is lacking on this topic so we've turned to the ACEP clinical policy, AAN guidelines, Ontario Pediatric Guidelines, and the  AAP guidelines on the topic.
