Send us Fan MailWhat if modern medicine has become incredibly good at saving children's lives…but society still hasn't figured out how to help many of those children truly live afterward?Dr. Nicholas Holekamp, MD is the Chief Health Transformation Officer at Ranken Jordan Pediatric Bridge Hospital in St. Louis ( https://rankenjordan.org/about-us/leadership/nicholas-holekamp-md/ ), one of only a handful of pediatric bridge hospitals in the United States and the only one of its kind in Missouri ( www.RankenJordanFoundation.org ).For more than two decades, Dr. Holekamp has helped pioneer a radically different approach to caring for children with complex medical conditions through Ranken Jordan's innovative “Care Beyond the Bedside” model - a philosophy built around a deceptively simple but powerful idea: children heal better when they are active, engaged, social, and out of bed.Under Dr. Holekamp's leadership, Ranken Jordan evolved from a small 26-bed pediatric nursing facility into a nationally recognized 60-bed specialty hospital focused on helping medically complex children transition safely from intensive hospital care back to home and community life.A graduate of Dartmouth College and Saint Louis University School of Medicine, Dr. Holekamp completed his pediatric residency at Cardinal Glennon Children's Medical Center before joining Ranken Jordan in 2000. Along the way, he has become one of the leading voices in pediatric complex care and transitional medicine.Dr. Holekamp's work has combined clinical care, hospital transformation, and research - including published studies demonstrating how movement, play, environment, and family-centered care can improve outcomes for medically fragile children while potentially reducing healthcare utilization and costs.Dr. Holekamp has been recognized for both his medical leadership and community impact, including receiving the Missouri Foundation for Health's Dr. Corinne Walentik Leadership in Health Award and delivering a TEDxStLouis talk on the future of pediatric healing and healthcare transformation.Today, as Chief Health Transformation Officer, Dr. Holekamp is focused on sharing the lessons of Ranken Jordan with hospitals and healthcare systems across the country - challenging long-held assumptions about what healing environments for children should actually look like.#Pediatrics #ChildrensHospital #MedicalEthics #HealthcareInnovation #PediatricCare #Neurodevelopment #ChildrenWithMedicalComplexity #HealthcarePodcast #Medicine #PediatricMedicine #HospitalInnovation #CriticalCare #Bioethics #FamilyCenteredCare #HealthcareLeadership #MedicalResearch #VentilatorCare #Rehabilitation #PediatricRehabilitation #FutureOfHealthcare #HealthcareSystems #MedicalTechnology #ProgressPotentialPossibilities #HealthcareTransformation #PediatricICUSupport the show

What happens when a sharp-eyed cruise ship doctor spots a hantavirus amid a sea of hangovers and flu symptoms? In this high-stakes episode of the Prolonged Field Care Podcast, Dennis sits down with Dr. Ryan Maves — combat-experienced infectious disease expert and military medicine veteran — to break down the shocking recent Andes virus outbreak.Far from the next global pandemic, hantaviruses are a real, rodent-borne threat that has hit soldiers before (Korean War, anyone?) and can strike deployed units in austere environments. Ryan delivers hard-hitting insights on rapid diagnosis, the “off-script” decompensation that screams hantavirus, supportive care when there's no magic antiviral, and — most importantly — prevention strategies that actually work in the field. If you operate in rodent-infested buildings, set up in abandoned structures, or just want to trust your gut when a patient goes south fast, this episode is required listening. Real talk from the A-team who are currently managing these patients stateside.Key TakeawaysClassic presentation: Flu-like prodrome (fever, fatigue, myalgias, GI upset) for a few days followed by sudden shock, respiratory failure, and decompensation.Bedside diagnostic gold: Thrombocytopenia (low platelets) + hemoconcentration (elevated hematocrit) in a previously healthy patient = major red flag.Treatment reality: Purely supportive — fluids, pressors, oxygen, renal support. No silver-bullet antiviral; ribavirin has limited data at best.Prevention beats everything: Humans are dead-end hosts. Avoid aerosolizing rodent urine/feces/droppings (no dry sweeping!). Use bleach, N95 (or equivalent), gloves, and gown.Human-to-human spread: Extremely rare except with Andes virus (this outbreak strain). Still, treat unknowns with respect.Military relevance: Endemic in deployment zones worldwide; occupying previously rat-infested buildings is a classic risk. History tied directly to U.S. troops in Korea.Mindset: When things go “off script,” trust your clinical instincts over machines. The best tool in the field is still an experienced medic's gut.Chapters00:00 – Welcome back to the PFC Podcast00:26 – Introducing Dr. Ryan Maves & the cruise ship outbreak00:55 – Why this isn't the next pandemic… but still matters03:04 – Military relevance: hantaviruses in deployment zones03:51 – How the cruise ship doc nailed the diagnosis05:27 – Clinical syndrome & the “virus-y” prodrome07:04 – Key labs: thrombocytopenia + hemoconcentration explained09:42 – Disease progression and why young healthy people can still crash10:50 – History of hantaviruses (Korean War → Sin Nombre → Andes)12:21 – Who actually dies and why14:50 – Biocontainment units and the military experts on the case17:35 – Treatment in the field: supportive care only19:35 – Shock management: distributive + capillary leak20:55 – Prevention is king: rodent control & PPE tactics24:22 – Human-to-human transmission (Andes virus exception)27:31 – Infection control, differential diagnosis, and real-world precautions30:08 – Final thoughts: clinical acumen, zoonoses, and trusting your instincts32:32 – Closing & where to find more PFC contentGrab your N95 and hit playFor more content, go to www.prolongedfieldcare.orgConsider supporting us: patreon.com/ProlongedFieldCareCollective or www.lobocoffeeco.com/product-page/prolonged-field-care

What does commanding troops in combat have to do with leading a hospital? More than you might think. In this episode of SoundPractice, host Mike Sacopulos sits down with Lt. Gen. Mark Hertling, U.S. Army (Ret) — former commander of U.S. Army Forces in Europe, CNN military analyst — to explore the surprising parallels between military and medical leadership. Soldiers and physicians share more in common than most realize. Both are defined by their profession first, and both operate in life-and-death environments where leadership is not optional. Hertling shares how he came to spend nearly a decade at AdventHealth developing physician leadership programs, what his doctoral research revealed about inter-professional training, and why getting doctors, nurses, and administrators in the same room may be the single most important thing hospitals can do. Inter-professional leadership training produces measurably better outcomes than siloed programs. He also discusses his newly released memoir, If I Don't Return: A Father's Wartime Journal, drawn from a handwritten journal he kept during Operation Desert Storm. Modern medicine is not shaped by science alone. It is shaped by ethics, trust, and the difficult decisions made when technology moves faster than society can understand it.In this episode of Research Renaissance, legendary bioethicist Dr. Art Caplan reflects on a career that helped build the field of bioethics from the ground up. From surviving polio as a child to shaping national policies on organ transplantation, informed consent, and end-of-life care, Caplan offers a deeply personal and historically grounded perspective on how ethical thinking became essential to modern healthcare. The conversation explores the lingering distrust born from COVID-19, the ethical blind spots of artificial intelligence, the environmental cost of data infrastructure, and why communication between science and the public may be the most urgent challenge ahead. This episode is both a history lesson and a call to action. Ethics is not abstract philosophy. It is practical problem-solving for real people, real patients, and real consequences.Key TakeawaysBioethics emerged to solve real clinical dilemmas, not theoretical debates.Policies such as informed consent, brain-death standards, and organ allocation were shaped by early bioethics work.Public trust in medicine declined significantly after COVID-19 due to shifting scientific guidance and poor communication.AI introduces ethical risks beyond autonomy and bias, including environmental strain, privacy vulnerability, and unclear liability. The U.S. healthcare system's structure, not just its technology, drives many ethical failures.Ethics must move from academic journals into communities through direct engagement and public dialogue.  Guest SpotlightArt Caplan, PhDOne of the founders of modern bioethics, Dr. Caplan has advised governments, medical institutions, and research bodies on issues ranging from organ transplantation policy to emerging AI ethics. His work bridges philosophy, clinical medicine, and public engagement.Topics DiscussedOrigins of bioethics as a disciplineHuman subject protections and informed consentEnd-of-life decision frameworks and hospice careVaccine hesitancy and post-pandemic mistrustEthical governance of artificial intelligence in healthcareEnvironmental implications of digital infrastructureStructural inequities in U.S. healthcare deliveryThe role of communication in rebuilding scientific trustIf you found this conversation valuable:Follow Research Renaissance for more conversations at the intersection of science, policy, and human health.Share this episode with colleagues working in healthcare, research, or ethics.Leave a review to help more listeners engage with these critical discussions. To learn more about the breakthroughs discussed in this episode and to support ongoing research, visit our website at tofflertrust.org. Hey friend! I am so glad you are here today, because this episode — this one right here — is one I have been genuinely excited to bring to you. Have you ever had that quiet, persistent feeling that God is calling you to something more — something beyond the bedside — but you did not know what to do with it? Maybe you have been faithful to your nursing career, you have given it everything, and yet there is this stirring in your spirit that will not go away. Like there is something else. Something bigger. Something you were built for that your nursing license alone cannot contain. If that resonates with you — stay right where you are. Because today's guest was made for this conversation. I have the honor of introducing you to Leniesha Norwood. Leniesha is a critical care nurse, a homeschool mom, an entrepreneur, a Holy Spirit-led coach, and the host of the Nurse to CEO Podcast. She brings over two decades of healthcare experience to everything she does — and she has spent that time not just caring for patients, but paying attention to what God was building in her through every single shift. She mentors nurses who feel that tension — the one so many of you know — between serving faithfully at the bedside and sensing God calling them to more.  In today's episode, Leniesha is unpacking something that I think is going to genuinely shift how you see yourself. Because here is the truth she is going to walk you through: you have been trained in far more than patient care. The skills you use every single day at the bedside; your ability to teach, to lead under pressure, to simplify complex information, to guide people through some of the hardest moments of their lives.... those are not just nursing skills. Those are Kingdom skills. Those are coaching skills. Those are the skills that build something that lasts beyond a 12-hour shift. Romans 11:29 says: "For the gifts and the calling of God are irrevocable." That means what God placed in you did not expire at the bedside. And today, Leneisha is going to help you start seeing that clearly. Now — I also want to make sure you know about something special she has coming up. On April 22nd at 11:30 AM Eastern, Leniesha is hosting the RN to CEO Shift Workshop — and it is going exactly where today's conversation is going, only deeper. She is going to walk you through how your nursing skills translate into coaching, mentorship, online business, and Kingdom-centered impact. Real examples. Practical steps. And the clarity you have been looking for about what your "next" could actually look like. You must be registered to attend or receive the replay. Go to rnskills.theconsecratedceo.com. I'll be there!
