Clinton is joined by Dr Nicole Higgins, president of the Royal Australian College of General Practitioners who are calling for changes to after-hours care so that more patients across Australia can get the care they need, when they need it. It comes following the College's submission to the Department of Health and Aged Care's Review of After Hours Primary Care Policies and Programs, which is investigating general practice care after-hours services and what needs to change. In May 2023, the Albanese Government announced new changes to after-hours care through Primary Health Networks, or PHNs, and Healthdirect. However, RACGP President Dr Nicole Higgins said that further enhancements were needed. “More must be done to help patients get the after-hours care they need,” she said. “Funding drives results. By supporting GPs to provide this care, we can expand what we do, and keep more patients out of hospital. By allowing GPs to claim after-hours rebates from 6pm on weeknights and 12pm on Saturdays for services delivered in the practice, we can enable more practices to remain open longer." "In my home of Mackay, there was previously an after-hours clinic that provided 24 hour care. However, after the Government made changes to the funding model, it's now operated by emergency doctors, costs patients three times as much, and closes at 10pm. This episode of Psych Matters recorded at the 2023 RANZCP Congress in Perth is about remote supervision for psychiatry trainees. Inequitable access to psychiatric care has been highlighted as a significant shortcoming for both the Australian and New Zealand mental health care system for some time. The college developed a rural psychiatry roadmap 2021 to 2031, a pathway to equitable and sustainable rural mental health services, and has made a number of proposals for change, emphasizing how building the rural psychiatry workforce will require the development of dedicated rural psychiatry training pathways to fellowship.The roadmap focuses on expanding opportunities for aspiring psychiatrists to live, train and practice rurally, as well as optimising the support available for those who take up these opportunities. One of the key priority areas for the roadmap has been to develop the remote supervision regulations which have been endorsed by the board and will be implemented over the coming years. Remote supervision is for newly created and established training posts that have not previously been provided by existing resident psychiatry supervision. The principal supervisor does not reside in the location and is primarily off site. Remote supervision is not designed to replace existing supervision arrangements but rather extend the training opportunities into locations which may have been unable to offer a training post due to meeting requirements of the current supervision policies.This conversation is with Associate Professor Jill Benson and Associate Professor Mat Coleman, who were involved with an expert advisory group in the development of the remote supervision guidelines and new regulations.Associate Professor Jill Benson AM is an Adelaide-based GP who has mostly worked with refugees, Aboriginal people and doctors. She is also an academic and medical educator and has written the Remote Supervision Guidelines for the RACGP as well as RANZCP. Associate Professor Mathew Coleman is a consultant psychiatrist with the WA Country Health Service, Clinical Director for the Great Southern and Midwest Mental Health Service and Clinical Academic with the Rural Clinical School of WA. He is a qualified child and adolescent, and addiction psychiatrist and has experience and qualifications in health service management. On the 1st of November 2023, the Australian Government will release new guidelines for screening prostate cancer with the Prostate Specific Antigen (PSA) blood test. These guidelines are based upon the Clinical Practice Guidelines (CPG) published in 2016 with new items to be included in the Medicare schedule. These items are based upon the Prostate Cancer Foundation and the Cancer Council of Australia guidelines that have been endorsed by the NHMRC, RACGP, and USANZ. We speak to Professor Ken Sikaris who has been at the forefront of these new recommendations. Our special guest is Professor Ken Sikaris who is a chemical pathologist and Director of Chemical Pathology at Melbourne Pathology. The RACGP says GPs could play a larger role in ADHD diagnosis and treatment to improve affordability and access to care for patients. Dr Bindiya Sethi started in a lead clinical role establishing a WHO pilot project against tuberculosis in India in a district of over 200k population immediately after her internship. When the choice is between writing a script for pain killers and a 45 consultation about weight management, which one do you choose?According to Associate Professor Kade Paterson, University of Melbourne, scripts for pain killers and referrals to orthopaedic surgeons are unnecessarily common for patients with osteoarthritis (OA).Professor Paterson says everyone who has osteoarthritis should be offered some sort of therapeutic exercise that suits them, and his fitness focus is backed by evidence.“We see very positive outcomes from the three approaches - exercise, weight management and education. All have been shown to be clinically effective at reducing both pain and function,” Professor Paterson says.Professor Kim Bennell is director of the Centre for Health, Exercise and Sports Medicine at University of Melbourne. She says that the kind of language clinicians use with OA patients is important. Focusing on the person, rather than the joint, is shown to be clinically effective in improving a patient's willingness to take up exercise, she says.Resources: OA treatment resources from the Centre for Health, Exercise and Sports Medicine.“Using language that talks with optimism about the effective, different treatments out there,” is a small change that is relatively easy to make says Professor Bennell.General practitioner Dr David King also advocates for non-drug and non-surgical treatments to be prescribed first. South Australia's long covid clinic loves GPs and keeps them close.Dr Angela Molga is a clinical pharmacologist and geriatrician at the long covid clinic at Royal Adelaide Hospital.“We engage the GPs very early on, from the moment we receive the referrals. The patients are kept updated on the length of the waitlist and we also send them out resources specifically around self-rehabilitation,” she says.Dr Molga says the average age of the patients who were seen in the South Australian clinic last year was 47 years old.“These were previously healthy people. Little contact with the healthcare system, but now have multiple chronic issues. They have to change their lifestyle significantly, and then this also impacts their mental health,” she says.Also on The Tea Room medical reporter Cate Swannell shares the nitty gritty details of the long covid parliamentary inquiry report.Although long covid fails to get a mention in the federal budget Cate says the long covid parliamentary inquiry is pushing for $50 million. She says the RACGP has welcomed the recognition of GPs in the report.“They also have pointed out that many recommendations reflect what the RACGP has been calling for, for a long time. He has also had an advocacy role in integrative medicine with a prior position on the ACNEM board, and a current position on the Australian Integrative Medicine Association (AIMA) advocacy committee, and is a founding board member for the Australian Chronic Infectious & Inflammatory Disease Society (ACIIDS) and the International Society of Environmentally Acquired Illness (ISEAI).
