C'est parti pour Roland Garros...! La première journée des internationaux de France a eu lieu ce dimanche sous un temps automnal. Et David Goffin s'est fait sortir d'entrée de jeu par la pépite, Jannick Sinner, on entendra la réaction du jeune italien dans ce podcast. Le tournoi est aussi déjà fini pour Kirsten Flipkens et Greet Minnen. Ca passe en revanche pour Elise Mertens.  Rafael Nadal et Novak Djokovic donnent leurs impressions à Paris.  Il y avait également deux tournois sur terre battue en guise de répétition générale avant Roland Garros: Strasbourg et Hambourg: qui a brillé, qui a déçu ? Muchas veces tienes la suerte de pertenecer a determinados grupos sociales más o menos selectos como un club de padel, running, etc. En mi caso, tengo la suerte de pertenecer al "club de la dermatitis seborreica", un club más o menos selecto (al que pertenecemos unos 2 millones de personas sólo en España), del que muchas veces no puedes renegar porque lo llevas en la cara... Aquí no se aceptan chaqueteros! Leer más... ¿Qué es la dermatitis seborreica? Es una enfermedad inflamatoria, crónica (no mata, pero mortifica), y normalmente se localiza en las zonas más grasas de nuestra piel (y no me refiero a la tripa), si no a las zonas de la piel que secretan más grasa (como son la piel de la cara y el cuero cabelludo, principalmente) y que coinciden con los pliegues de cara y cuerpo. La composición de este sebo es lo que hace que unas personas desarrollemos esta dermatitis y otras no. Esta composición anormal de la grasa cutánea es lo que hace que proliferen ciertos microorganismos llamados levaduras (Malasezzia furfur) y provoquen picor, enrojecimiento, etc. ¿Cómo se manifiesta la dermatitis seborreica? Se produce un aumento de la descamación y seborrea en el cuero cabelludo, y a nivel de cara aparecen unas lesiones escamosas, enrojecidas y grasosas, y no rasposas al tacto. Los brotes de dermatitis seborreica suelen coincidir con épocas de vida sendentaria y estrés, y se suele estar mejor en los meses de verano, en los que pasamos más tiempo al aire libre. Muchas veces viene acompañada de blefaritis (se inflaman los párpados) y/o conjuntivis. Muchas veces, la dermatitis seborreica es un anticipo de una psoriasis que puede aparecer más tarde, por lo que es bueno que el dermatólogo nos haga un seguimiento. ¿Qué formas de dermatitis seborreica existen? Hay 3 principales: la facial, la pediátrica y la del cuero cabelludo. La dermatitis seborreica pediatrica (costra láctea): Aparece en los primeros días de vida, afectando principalmente al cuero cabelludo, pliegues, cara y tronco. En la cara y el cuero cabelludo las lesiones son aceitosas, con costras sin base inflamatoria. Mientras que en el cuerpo son lesiones enrojecidas de menor tamaño. La costra láctea es benigna y autolimitada. El tratamiento de la costra láctea consistirá en: - Usar aceites emolientes (oliva) o jabones suaves (sin jabón) que permitan eliminar las escamas. - Un tratamiento que reduzca la inflamación (emulsión de keluamida), evitando en lo posible el uso de corticoides. Higiene y buenas costumbres En general, el paciente con dermatitis seborreica mejora con la exposición al aire libre, el ejercicio y la exposición gradual y moderada al sol. Se recomienda un lavado frecuente de la cara o cuero cabelludo con jabones suaves y si el médico lo indica con jabones de tratamiento (como el ketoconazol). Debemos evitar las lociones que contengan alcohol, ya que pueden empeorar el estado de la piel. Tratamiento y mantenimiento de la dermatitis seborreica: Dermatitis seborreica facial: Será clave disminuir la población de levaduras con fármacos tratantes (clotrimazol, ketoconazol, miconazol, ciclopirox, etc). A veces basta con la aplicación del gel jabonoso. También puede ser útil el tratamiento con corticoides de muy baja potencia durante los primeros días. Todo esto lo debe evaluar el médico, claro. Debemos evitar cosméticos que aporten grasa a nuestra piel, al principio experimentaremos una ligera mejoría y posteriormente empeorará notablemente el estado de nuestra piel. Como tratamiento de mantenimiento de la dermatitis seborreica facial tendremos en cuenta de que se trata de un proceso crónico y que debemos ser constantes. Con activos calmantes (enoxolona, keluamida), y que controlen el exceso de levaduras (piroctona olamina). Dermatitis seborreica capilar: Cuando la dermatitis seborreica se ceba con nuestro cuero cabelludo afecta seriamente a la salud de nuestro pelo, provocando picor, enrojecimiento del cuero cabelludo, descamación (caspa grasa) e incluso muy frecuentemente la caída del cabello en corros. En este caso es habitual el empleo de champús con antifúngicos (ketoconazol o ciclopirox olamina) e incluso corticoides topicos en loción capilar para disminuir la reacción inflamatoria. Los champús con sulfuro de selenio, zinc piritiona o keluamida también son eficaces calmando el brote y normalizando el cuero cabelludo. Las breas minerales (coaltar) o vegetales (enebro, pino, etc) son eficaces por disminuir el picor. Destaca el ictiol pale por su seguridad, se puede emplear durante largos periodos de tiempo. El aceite de arbol de té (melaleuca alternifolia) tiene efectos antifúngicos y es eficaz frente a la levadura Pityrosporum. Puede ser útil a una concentración del 5% en forma de champú. Tiene una actividad anticaspa, seborreguladora y calmante sobre el cuero cabelludo. Los queratolíticos (como el ácido salicílico, la urea o el ácido glicólico) ayudan a desprender el sebo cutáneo. También se pueden emplear algunos corticoides tópicos de baja potencia, si el médico lo estima oportuno. Si el brote es intenso aplicaremos una loción o aceite capilar tratante en el cuero cabelludo por la noche y aplicaremos un champú de tratamiento a la mañana siguiente. A la hora de lavarse el cabello: No hacerlo con agua muy caliente, más bien tirando a fría. Es mejor 2 enjabonadas con poco champú que una con mucho y que se vaya por el desagüe sin hacer su trabajo. No emplear champús muy agresivos. Aclarar bien el jabón. Secar bien tras el lavado, dejarlo húmedo puede gustar demasiado a nuestras queridas levaduras y hongos.
