In this episode, Max discusses two Challenger jet crashes involving dual engine failures: one in Naples, FL, in February 2024 and another in Nebraska in 1994. The incidents share some similarities. In the Naples crash, both engines experienced oil pressure warnings within seconds of each other before failing simultaneously. The preliminary report indicates the left landing gear touched down first, followed by the right, before the plane skidded into a concrete barrier. The cabin attendant facilitated passengers' safe evacuation through the tail compartment. The pilots, highly experienced, had a combined flight time of over 35,000 hours. The aircraft, with GE CF34 Series turbofan engines, underwent recent airworthiness inspections and was fuel-soaked post-crash, ruling out fuel exhaustion. Further examination revealed thermal damage to engine components but no clear cause for the simultaneous engine failures. Discussion with an Challenger jet pilot who listens to the show recalled a similar incident involving a Challenger aircraft that also had a dual engine flameout in 1994. That crash occurred during a positioning flight after passenger drop-off, after experiencing a dual engine flameout between FL370 and FL410. The aircraft sustained substantial damage during a forced landing at night in an alfalfa field. Analysis revealed water contamination in the fuel, leading to the engines' failure. Both engines were sent for examination, showing consistent findings with fuel samples. The NTSB attributed the crash to inadequate planning, decision-making, and preflight inspections following fuel contamination. Comparing the two crashes, both had simultaneous dual engine failures occurring around two hours into flight, and both had a yellow liquid in the fuel. At this point, the cause of the Naples crash is still unknown, though Cockpit Voice Recorder and Flight Data Recorder analysis should provide new clues. The episode underscores the importance of fuel contamination awareness, and the necessity for thorough preflight inspections and proper response to warning signs. Welcome to your weekly General Aviation news update, and this week we have 5 stories for you; small airplane crash, New Piper M700 Fury, FAA tells congress not to increase pilot retirement age, FAA advises to use analog NAVAIDs, and comments close for the 25 hour CVR rule. First up this week is a crash of a Grumman GA-7 Cougar. While we don't normally talk about small aircraft accidents, this one was recently rebuild by a popular YouTube channel called Rebuild Rescue. The aircraft departed Coatesville-Chester County Carlson Airport in Pennsylvania and crashed shortly after takeoff. Based on a map provided from Aviation Safety Network, the crash appears to have occurred less than 2 miles from the airport. The pilot and only occupant of the Cougar was killed in the crash. As usual, we're not going to speculate on the cause of the accident. Piper has unveiled their new M700 Fury, a single engine turboprop with a 301kts max cruise speed, 3400 foot per minute rate of climb, and take off and landing distances under 2000'. The M700 Fury comes with a Pratt and Whitney PT6A-52 Engine, Garmin G3000 Avionics Suite with HALO Safety System, Garmin PlaneSync, and Garmin Auto land. The Fury has a useful load of 2,220lbs, a maximum rage of 1852NM, and a maximum approved altitude of 30,000'. The new FAA Administrator Mike Whitaker has asked congress to leave the current mandatory retirement age in place until the agency has had more time to study the increase. The pilot retirement age is now part of negotiations towards a 5 year FAA reauthorization act. Without another extension or a reauthorization act, the FAA's reauthorization will expire on March 8th, 2024 . If you're unfamiliar, the NPRM aims to increase Cockpit Voice Recorders from 2 hours of recording to 25 hours. This would allow the FAA and NTSB to better investigate incidents. Reactions to the NPRM have been mixed, with a number of comments expressing concern that the 25 hours of CVR will be used to listen in on pilot conversations or used to punish pilots with certain viewpoints. ALPA addressed this in their comments, suggesting that there should be restrictions on the use of recording or transcripts for administrative or disciplinary proceedings including by both Federal Agencies and by a pilot's employer. Panglima TNI Marsekal TNI Hadi Tjahjanto, S.I.P. bersama Menteri Perhubungan (Menhub) Budi Karya Sumadi, Kepala Staf Angkatan Laut (Kasal) Laksamana TNI Yudo Margono, S.E., M.M., Kepala Badan Nasional Pencarian dan Pertolongan (Kabasarnas) Marsdya TNI Bagus Puruhito dan Ketua Komite Nasional Keselamatan Transportasi (KNKT) Soerjanto Tjahjono, menggelar konferensi pers terkait ditemukannya Flight Data Recorder (FDR) pesawat Sriwijaya Air SJ-182, bertempat di Dermaga JICT II, Tanjung Priok, Jakarta Utara, Selasa (12/1/2021). Terkait kronologis ditemukannya FDR oleh penyelam dari TNI Angkatan Laut, Panglima TNI menyampaikan bahwa pada hari ini, Selasa 12 Januari 2021, tepat pukul 14.00 WIB, Kasal menyampaikan informasi bahwa sesuai dengan perkiraan yang sudah ditentukan di wilayah yang sudah ditandai, telah ditemukan bagian dari FDR. Selanjutnya Panglima TNI memerintahkan kepada Kasal agar terus mencari FDR yang kemungkinan besar masih berada di sekitar wilayah yang telah ditandai. “Kasal menyampaikan bahwa bagian yang ditemukan justru adalah pecahan dan Underwater Beacon yang berfungsi memberikan sinyal,” ungkapnya. Sekitar pukul 16.40 WIB, Kasal kembali melaporkan bahwa FDR telah ditemukan setelah tim penyelam melakukan operasi pencarian di sekitar titik temuan awal pecahan Black Box. “Kasal melaporkan kembali bahwa FDR sudah ditemukan dan dilaporkan pula bahwa Underwater Locator Beacon ditemukan sebanyak dua buah” kata Panglima TNI. Selain FDR, untuk mengungkap penyebab jatuhnya pesawat Sriwijaya Air SJ-182 juga dibutuhkan Cockpit Voice Recorder (CVR) yang merekam pembicaraan pilot. Panglima TNI meyakini bahwa CVR bisa segera ditemukan di sekitar lokasi penemuan FDR Black Box Sriwijaya Air SJ-182. “Kami meyakini semua bahwa beacon yang ada di CVR juga ditemukan di sekitar itu, maka dengan keyakinan yang tinggi, Cockpit Voice Recorder akan segera ditemukan,” ucapnya. Menurut Panglima TNI, pasca ditemukannya Flight Data Recorder, proses evakuasi korban dan badan pesawat Sriwijaya Air SJ-182 akan tetap dilakukan. “Badan pesawat perlu diangkat guna proses penyelidikan penyebab jatuhnya pesawat dan dalam rangka melengkapi data yang diperlukan oleh KNKT,” ujarnya. Usai ditemukannya Flight Data Recorder, Panglima TNI secara simbolis menyerahkan secara langsung temuan Black Box atau kotak hitam Sriwijaya Air SJ-182 dari penyelam kepada Kabasarnas, yang kemudian dari Kabasarnas diserahkan kepada Ketua KNKT. “Saya mewakili seluruh prajurit TNI di lapangan, anggota Polri di lapangan, dan stakeholder terkait menyerahkan hasil temuannya FDR kepada penanggung jawab operasi evakuasi kecelakaan pesawat Sriwijaya Air SJ-182 yakni Kabasarnas,” kata Panglima TNI.
