In this episode of Flying Magazine's "I learned about flying from that" podcast, host Rob Reider speaks with Tim Kurrek about a truly challenging incident that occurred over 20 years ago. Tim, a longtime instructor with extensive tailwheel and glider experience, was on what should have been a routine flight in IMC in his trusted Cessna 180 when the engine suddenly and unexpectedly quit. With no autopilot and the gyros spooling down, Tim was immediately thrust into a situation requiring proficient hand flying and quick thinking. After declaring the engine failure and contacting NorCal, he began troubleshooting while maintaining his best glide speed. However, battling heavy rain and downdrafts, Tim realized his descent rate was higher than expected, and he wouldn't make the initial airport NorCal vectored him towards, which was surrounded by suburbia. This critical realization forced a rapid decision and a change of course towards what he hoped would be more suitable terrain. Breaking out of the clouds at a low altitude, he faced the daunting task of selecting and landing on an unconventional off-airport site under challenging muddy and crosswind conditions. The landing, while successful in preserving life and aircraft integrity, presented its own immediate set of problems with the aircraft sinking into the mud. What followed involved navigating interactions with a gruff local farmer, law enforcement, and the FAA, all while trying to understand the mysterious engine failure. Ultimately, the cause of the engine issue was identified, and the challenging process of recovering the aircraft from the field, with the farmer's reluctant assistance, became another chapter in the story. Tim shares the powerful lessons learned about the importance of aircraft knowledge, maintaining pilot proficiency, critical situational awareness, and the absolute necessity of pre-planning flights, even IFR ones, with VFR charts to understand the ground below. His experience vividly illustrates astronaut Frank Borman's quote: "A superior pilot uses his superior judgment to avoid situations which require the use of his superior skill".

In this episode, Max discusses two Challenger jet crashes involving dual engine failures: one in Naples, FL, in February 2024 and another in Nebraska in 1994. The incidents share some similarities. In the Naples crash, both engines experienced oil pressure warnings within seconds of each other before failing simultaneously. The preliminary report indicates the left landing gear touched down first, followed by the right, before the plane skidded into a concrete barrier. The cabin attendant facilitated passengers' safe evacuation through the tail compartment. The pilots, highly experienced, had a combined flight time of over 35,000 hours. The aircraft, with GE CF34 Series turbofan engines, underwent recent airworthiness inspections and was fuel-soaked post-crash, ruling out fuel exhaustion. Further examination revealed thermal damage to engine components but no clear cause for the simultaneous engine failures. Discussion with an Challenger jet pilot who listens to the show recalled a similar incident involving a Challenger aircraft that also had a dual engine flameout in 1994. That crash occurred during a positioning flight after passenger drop-off, after experiencing a dual engine flameout between FL370 and FL410. The aircraft sustained substantial damage during a forced landing at night in an alfalfa field. Analysis revealed water contamination in the fuel, leading to the engines' failure. Both engines were sent for examination, showing consistent findings with fuel samples. The NTSB attributed the crash to inadequate planning, decision-making, and preflight inspections following fuel contamination. Comparing the two crashes, both had simultaneous dual engine failures occurring around two hours into flight, and both had a yellow liquid in the fuel. At this point, the cause of the Naples crash is still unknown, though Cockpit Voice Recorder and Flight Data Recorder analysis should provide new clues. The episode underscores the importance of fuel contamination awareness, and the necessity for thorough preflight inspections and proper response to warning signs. Oil leaks don't just happen. When they do, they should trigger a close look at the engine. John Goglia and Todd Curtis share this advice after digging into the fatal crash of a Cessna Centurion after the owner and his passenger made unscheduled repairs on the oil system.The experienced pilot and passenger lost oil in fight and landed to add more. Surprisingly, there is no evidence that they did any kind of inspection of the engine. Witnesses present when they took off again say they did not do an engine runup. They also reported hearing odd engine noises as the plane took off.Todd and John discuss how the decisions made by the pilot were the primary cause of this plane crash. Every engine is going to be a little different, but there are generic steps for troubleshooting any partial or complete engine failure. All piston engines need fuel, air, and a source of ignition, so you need to check each of these items. For fuel, switch tanks, turn on any auxiliary fuel pumps, and adjust the mixture. If you have carb heat and a manual alternate air system, try applying these. For ignition, try different positions on the mag switch to see if that helps. The mixture on turbocharged engines is handled differently. In all cases, you should refer to the engine failure in flight checklist for your aircraft. The newer phraseology results in the same service provided by ATC; they'll still ask for souls on board, and fuel remaining, and you can still deviate as needed, but you don't need to say the words "I'm declaring an emergency."
