Writing patient notes is a simple (albeit boring) task that can have major implications if not done correctly. There are certain things that should always be included in patient notes, and others that should never be, and in today's episode Scott, Jake, and I share some examples of what's on each of those lists. Scott also gives us a rundown of the electronic medical records system that he uses, and with pharmacies heading towards a total ban on paper prescriptions, everyone will benefit from hearing his advice. Body dysmorphia is the disorder of imagined ugliness, a type of delusional OCD that can lead to severe depression, eating disorders, compulsive body alterations, and suicidality.  As a young child, Georgina was a "cute kid", not particularly self-conscious, and able to move through her world without undue pain. With the onset of adolescence, however, her self-image changed completely-- she suddenly found her face horribly ugly, she felt helpless and hopeless, and she frantically tried to find ways to cope. Tammy and Dr. Jeffrey Harris had been married for 11 years when Tammy was diagnosed with lupus. Doctors begin a treatment regimen but her health did not improve. Harris was wheelchair-bound, her weight dropped to just 70 pounds, and her hair and fingernails fell out. The couple traveled from Moscow, Idaho to New York in search of a holistic doctor, when Tammy suddenly collapsed in the lobby of the Lotte New York Palace hotel. She died six days later. An autopsy determined that Tammy Harris died from an overdose of the dietary supplement, Selenium. Selenium, a trace mineral is considered safe in small doses, but the city medical examiner found eight times the average amount of Selenium in Harris's system. An investigation revealed that Dr. Harris did not believe his wife had Lupus. He insisted that she suffered from mercury poisoning from working at a dental office several years prior. His receipt of this scholarship formulated the trajectory of his career as an oral surgeon. Dan's journey has involved treating war warriors as a GPR, completing a head and neck fellowship, and helping to develop the digital adaptation of the “jaw in a day” surgery. Today's guest is an oral and maxillofacial surgeon who specializes in alveolar cleft grafting, cleft orthognathic surgery, and distraction. Through these life-changing procedures, Dr. Chris Viozzi is doing his part to give back, and he does this not only through treating patients within the US, but also those outside of it during 10-day mission trips he was doing twice a year prior to the pandemic. Registered nurse Cristen Giangarra, 31, is among hundreds of women reporting rapes and assaults by their lift drivers. Giangarra called for a Lyft after a night of celebrating with friends. The ride to her home should have lasted six minutes but was instead was 51 minutes of terror. Giangarra says she fell asleep in the backseat and woke up to her driver raping her. Giangarra sued Lyft, saying the company cut corners on background checks and hired her driver, 54-year-old Larry Ward, despite his criminal history. An American Airlines executive was shot to death last year as he walked with his wife and their dog. Jennifer Faith told police that they were just outside their home when a masked man approached from behind, aimed at her husband's head, and fired several times. Then, she said, he knocked her down and tried to wrap duct tape around her hands and steal her jewelry. Now arrest has been made. Police say 48-year-old Jennifer Faith conspired with her high school boyfriend to kill her husband, Jamie Faith. Darrin Ruben Lopez, 48, is charged with murder. The investigation in the shooting of two people on the set of Alex Baldwin's movie 'Rust' continues. Director of photography Halyna Hutchins was killed and director Joel Souza injured. Reports say first assistant director David Halls picked up one of the three prop guns that had been set up by armorer Hannah Gutierrez and yelled “cold gun.” "Cold gun" is industry slang meaning the gun did not contain live rounds. Halls gave the gun to Baldwin who was using the gun to rehearse a scene. The search warrant said Baldwin was aiming the weapon at the camera when he fired, striking Hutchins and Souza.
