An experienced pilot with multiple ratings lost control of his Mitsubishi MU-2 aircraft and crashed after a missed approach. Greg Feith, Todd Curtis and John Goglia look at the facts surrounding this incident that killed six.ADS-B information broadcast by the plane shows a missed approach followed by an unusual second attempt. Rather than flying the published missed approach, air traffic controllers were directing the pilot to take a series of right turns for a second approach. The pilot was taking several members of his family to a holiday celebration, so the pilot may have chosen to fly a non-standard missed approach because it would take less time than the published approach.Greg shares firsthand experience and knowledge of the performance of MU-2 aircraft. Several years ago, he evaluated MU-2 models for safety issues and found that many previous accidents in these models were due to issues with the pilot, not the aircraft.This aircraft was being flown by a single pilot in IFR conditions. Flying single-pilot IFR in actual IFR conditions is a challenge for any pilot under normal circumstances. Temperatures were just above freezing at neighboring airports and there was snow in the area at the time of the crash. Icing and the presence of five passengers and their baggage may have degraded the aircraft's performance.The pilot's familiarity with the destination may be a factor. A review of flight records showed this was the first time the aircraft had flown to this airport in the previous six months. Scott Kartvedt became a Navy fighter pilot in 1991 at the controls of the AMAZING F/A-18 Hornet. He flew with the Blue Angels for three years as the #7, #6, and then the #5 solo pilot, then returned to the fleet to serve in combat in Iraq in 2003 in 2005. He because a commanding officer, serving two combat tours in Afghanistan in 2009 in 2010, and ran the Navy's F35 program in 2011 before being selected as the Navy's first stealth fighter squadron commanding officer. In all, Scott served during five combat deployments, flying 91 combat missions, accumulating over 6,300 flight hours and 658 carrier arrested landings on 11 aircraft carriers. After retiring from the Navy, Scott hasn't slowed down in the slightest. He currently flies for the Patriots jet demonstration team, was a stunt pilot in Top Gun Maverick, and was the aviation safety supervisor in Mission Impossible 8. His sons are both Naval aviators, following in their father's footsteps. ADS-B traffic data enhances situational awareness with collision detection and wake turbulence tracking. Additionally, archived weather data helps pilots review conditions during debriefs. Gen. Lloyd “Fig" Newton began his Air Force career in the heat of battle, flying 269 combat missions in the F-4D Phantom during the Vietnam war. Following the war, he served as the first African American member of the Elite United States Air Force Thunderbirds Demonstration squadron. General Newton served as the Air Force congressional liaison officer with the U.S. House of Representatives in Washington DC and was Director of Operations, United States Special Operations Command. General Newton is a command pilot with more than 4,000 flying hours in the T-37, T-38, F-4, F-15, F-16, C-12 and F-117 stealth fighter. Tom retired in May 2024 after 34 years of service flying for a commercial airline. He served in the Air Force prior to the airlines as an active duty and reserve pilot over a fourteen-year period. He flew the C-141 Starlifter during that time and conducted special operations missions in various conflicts as well as during Desert Storm. In his airline career, Tom flew the Boeing 727, L-1011, Boeing 767, MD-88, and A-320. Both Tom and Erik still fly together in General Aviation aircraft today!

A fatal midair collision involving a Cessna 172 illustrates several aviation safety concerns related to pilot training in and around airports with commercial operations. The incident involved a student and instructor on board performing touch-and-go landings and a Dash 8 departing on a passenger flight.While the event happened in Kenya, the circumstances that led to this accident could happen in any location where airliners and small training aircraft operate out of the same airport. In this case, the midair collision occurred only about 500 feet above the ground and 1500 feet below a broken cloud layer in an area with over 10 km of visibility.The Cessna 172 was performing touch-and-go landings on from one of the airport's runways. Shortly after the Dash 8 departed from an intersecting runway, the stabilizer of the airliner collided with the Cessna. The Cessna crashed, killing both on board. Todd Curtis and John Goglia analyze the preliminary report, which came out three months after the accident. They examine the transcript of ATC communications around the time of the accident. Anyone who operates aircraft in similar circumstances could learn something useful from studying this event.Immediately after the collision, the transcript of the ATC communications stated that during 97 seconds, there was "unrelated transmission from other traffic." He has a diverse background which provides him with the ability to work with a wide variety of personalities and backgrounds. He has been a Certified Flight Instructor since 1975. He has had an opportunity to fly and teach in a number of regions of the country which provides him with an understanding and appreciation for weather and challenges that other parts of the United States offer to pilots.   He specializes in training in Columbia/Cessna/Corvalis aircraft and other Technically Advanced Aircraft as well as transition training for all Garmin GPS units, G1000, Avidyne, Aspen and Bendix King equipped aircraft. The crew's use of noise-canceling headsets and the challenging new airport approach added to their workload, leading to mistakes. Trescott uses the SHELL model (Software, Hardware, Environment, Liveware) to analyze the accident, emphasizing how the interaction of these elements can lead to errors. He discusses the effects of high workload on pilot performance, including attentional narrowing and task shedding, and offers strategies to manage it, such as starting descent planning earlier and effective CRM practices. Recognizing high workload is crucial, but pilots often don't notice it in real-time, necessitating proactive measures and specific training. Trescott concludes by highlighting the dangers of overconfidence and complacency, even among experienced pilots, and stresses the importance of developing strategies to catch and correct mistakes to enhance flight safety.
