The U.S. Surgeon General says Americans are confronting an epidemic of loneliness. About half of U.S. adults report experiencing loneliness, and that was before the COVID-19 pandemic. Loneliness has physical health consequences, and impacts work performance, academic achievement and our overall mental health. Additionally, the Surgeon General has warned that the stress of caring for children has an impact on our mental health. Many members of Generation X may be caring for children or again parents or both, and the stress of caregiving has impacts on mental health as well. In this episode of Further Together, hosts Michael Holtz and Matthew Underwood talk to Jennifer Reynolds, senior manager of health communication, marketing, and promotion and Kristin Mattson, senior health education specialist. Both work in ORAU's Public Health and Healthcare Program. Throughout this episode, the hosts and guests discuss the problems, contributing factors, their lived experience, and where ORAU has done work to better understand and provide solutions. The chair, Kate Charlesworth opens by explaining that the healthcare system has a big problem – we are, in effect, producing our own patients. We use huge amounts of resources; produce vast amounts of waste and have a big carbon footprint. Globally, if the healthcare system was a country, it would be the fifth biggest polluter on the planet. The irony is then of course that we're therefore contributing towards the climate crisis which is harming human health. We've seen that with storms, fires, floods, and all the associated impact they're having on our health. We have a huge job ahead of us – to decarbonise or to get to a net zero health system. Nick Watts, doctor, and chief sustainability office for the National Health Service in England says that he wants to discuss three things: why the NHS cares about climate change, what we can do about it and exactly what that change needs to looks like. He then goes on to explain the steps that the NHS is taking to reach net zero by 2045, and the exact steps that we need to take in order to do the same. His three key messages are: The climate crisis is a health care crisis. The time for talking about stuff is over, the only thing that matters is what we are going to do about it. Dr Dominique Cosco is an associate professor of Medicine and Director of the Internal Medicine residency program at Washington University School of Medicine/Barnes-Jewish Hospital. In addition, Dr. Cosco is the Director of Teacher Development for the Academy of Educators at Washington University School of Medicine. Dr Cosco completed her Medical school from Tulane University and Residency in Internal Medicine from Emory. During her time as a faculty member at Emory University, she served as an Associate Program Director of the Emory University Internal Medicine Residency Program and directed the Global Health Distinctions Program. She is widely recognized for her role as Director of the Coaching Program at Emory and as Director of the TEACH (Teaching Educators Across the Continuum of Healthcare) Program in the Society of General Internal Medicine. She has also held significant educational and leadership roles in Academic Alliance for Internal Medicine (AAIM) and the Association for Program Directors in Internal Medicine (APDIM) where she is the Chair for the program planning committee. Because of her exceptional skills and expertise, she has received a number of honors and awards for her educational contributions and citizenship. As physicians, we each have individual strengths. And while they should be recognized, Dr. Dominique Cosco reminds us that we should be continually looking for ways to improve and refine those strengths. Today, she talks about embracing new opportunities for growth that we encounter throughout our career in medicine. Dr. Cosco realized early on that growth and learning never stop. And in medicine, leaning into the ways we can build on our strengths will take us from good…to great. Pearls of Wisdom: 1. Even in our strengths, we have an opportunity to grow and refine them. Recognize your unique strengths but understand that you will continually have opportunities to improve those strengths. That will take us from good to great. 2. We have to be flexible, yet willing to make small decisions. As we make small decisions along the way, keep our minds open to the big picture, so we can continue to navigate the ship in the right direction. 3. It's not about how smart you are, it's about how well you play on a team. To lead well, you have to know how to lead from within.

She worked as an administrative resident at the John’s Hopkin’s Bayview Medical Center focusing on Quality and Process Improvement. She worked at the Dana Farber Cancer Institute and Brigham and Women’s Hospital over 5 years and helped multidisciplinary teams apply lean principles to improve efficiency and safety of their clinical and business processes.
