Yes, living in a small town has some disadvantages - like, we didn't have a stoplight or a drive thru - I had to drive to larger community with my driving instructor to experience that! The one thing that I loved about a small town was the community! Being known, cared for and recognized was a great benefit of small town life! Two songs today for you - Miranda Lambert - Famous in a small town and John Mellencamp - Small Town "Famous in a Small Town" is a song co-written and recorded by American country music artist Miranda Lambert. It was released in April 2007 as the second single from her album Crazy Ex-Girlfriend. It was Lambert's second Top 20 hit on the U.S. Billboard Hot Country Songs chart. It was nominated for the Grammy Award for Best Female Country Vocal Performance in 2008. The song was written by Lambert and Travis Howard. The song was ranked number 40 on Engine 145's 50 Best Country Songs of 2007 "Small Town" is a 1985 song written by John Mellencamp and released on his 1985 album Scarecrow. The song reached #6 on the U.S. Billboard Hot 100 chart and #13 Adult Contemporary. Mellencamp wrote the song about his experiences growing up in a small town in Indiana, having been born in Seymour, Indiana, and living in Bloomington, Indiana, which, at the time of the release of the song, was larger. Case Summary Healthy and physically fit incarcerated 49M who presents with 2 weeks of fevers, myalgias, and SOB. His past medical history includes GSW to abdomen, psoriasis not currently on medications, prior tobacco and alcohol abuse. Transferred for undifferentiated shock on norepinephrine, and was found to be in sinus tachycardia to 110 bpm, hypotensive despite vasopressor infusions with labs showing a hyperinflammatory state, multi-organ failure, and eventually found to be COVID+. The patient quickly progressed into refractory cardiogenic shock requiring VA-EMCO, as well as Impella placement for LV unloading. The patient underwent endomyocardial biopsy with electron microscopy which was notable for COVID virions in the myocardium and was diagnosed with COVID myocarditis. Interestingly, his chest CT showed normal lung parenchyma and therefore presented as isolated cardiac involvement of COVID-19. The patient improved with tocilizumab, IVIG, and steroids.
