Upon first clinical presentation it looked certain that Jackie Ellis was suffering from ICI myocarditis, a rare but deadly condition. Upon further evaluation Dr. Joshua Mitchell determined this was not in fact the case. This allowed her oncology team to continue her on the life saving cancer therapy she needed. This is cardio-oncology care at its best. The goal is to mitigate cardiovascular complications in order to keep treating the cancer!

Epidemiology of Myocarditis   Guest: Leslie Cooper, M.D. Host: Sharonne Hayes, M.D.   Approximately 1,000,000 cases of myocarditis are diagnosed annually with an incidence of approximately 16/100,000.  The incidence is highest in men between 15 and 45 years of age.  However, after age 60 years, women have higher mortality than men. The overall mortality rate is 0.4/100,000.  The rates of diagnosis are double in high SDI regions compared to low STI regions, most likely due to the availability of the pivot all diagnostic tests, cardiac MRI and heart biopsy.  The incidence rates are stable while the death rates have decreased from 1990 to 2023.  Finally, the causes of myocarditis or changing with more cases of immune checkpoint inhibitor related myocarditis which can affect between 0.2 and 1.1% of patients receiving these cancer therapies.   Topics Discussed: How does the epidemiology of myocarditis help inform clinical practice for individual patient management? Dr. Jyotsna Shah is an immunologist and molecular biologist with over 40 years of experience in diagnostic tools for tick-borne diseases. This podcast highlights three updates: Moderna's Spikevax COVID-19 vaccine is now fully FDA-approved for high-risk children 6 months to 11 years, though rare myocarditis remains a concern. A large study found first-trimester TMP-SMX antibiotics increase congenital malformation risks compared to β-lactams, reinforcing β-lactams as safer. Finally, a trial showed no cardiovascular benefit from taking blood pressure medication at bedtime versus morning dosing.

Dr. Kirk Milhoan is a pediatric cardiologist and senior fellow at the Independent Medical Alliance. He has been treating children with myocarditis and other cardiovascular issues associated with COVID-19 and the COVID-19 vaccines.“Four years later, five years later, I'm seeing this constant and dramatic change in who I'm seeing coming to see me. They're complaining their heart doesn't beat normally. And it beats fast for no reason at all,” says Dr. Milhoan. “Specifically after the second dose of the new platform for the COVID vaccine, we were seeing an increase in myocarditis in children that we've never seen before with any vaccine product in children.”In this episode, we dive into the apparent rise of cardiovascular conditions in children and how to better address and understand them.“We need to return the idea of a patient-doctor relationship,” says Dr. Milhoan. “You're not a consumer. We've made this too marketing-oriented. We need to go back to: ‘I'm a physician who cares for you because I have compassion for you.
