    Kicking off 2022 like an EXPERT Part 5

    We are almost done getting ready for the year so hang tight. I have a couple more things we need to plan for 2022. Next, we need to plan our Environment of Care rounds for the year. Most healthcare facilities conduct formal rounds in clinical areas twice per year, and non-clinical areas once per year. This is always subject to change if there are issues or if you are expecting a regulatory visit. In my experience, if I don't plan these at the first of the year, I wake up and half the year is gone! Let's get these dates on the calendar. I would schedule an entire day for this activity, but you might only need half a day. Here are some of the common areas you want to monitor: Nursing units Kitchen Laundry Facility Biohazard and Trash Areas Specialty Areas- Dialysis, Procedural Areas, etc Therapy Gyms Outdoor Areas Diagnostic Imaging Laboratory Materials Management (Storage) These are some of the most common areas, but this is not an all inclusive list. It gives you a general idea, but you need to examine your own facility to determine how you want to designate the different areas. By the way, if you need a tool to help guide your EOC rounds, I have one ready to go here. If you need more assistance with conducting the rounds, I have an entire course here.     Kicking Off 2022 Like an EXPERT Part 4

    We have covered a lot already, but stay with me. We have more to cover to ensure you have a successful year! Next on the list is new guidelines. Are there any guidelines that have changed in 2022? One group that changes their definitions each year is the National Healthcare and Safety Network (NHSN). If you use their definitions, you want to review the new manual that comes out each year. Other groups update their guidelines throughout the year, but it's possible you may have missed something. I recommend skimming through these websites and others that are relevant to your facility to make sure you know all the new guidance for the year. NHSN CDC CMS OSHA Your State Regulations and Reportable Diseases Once you review all of the regulations, make note if there are areas you need to add to your Infection Prevention Plan. I like to keep a running list of new guidance along with where I am in the implementation process. This helps me stay on track with making sure that I am in compliance with all regulations. You also want to include any accreditation groups such at The Joint Commission to your list to review for new standards. I know it seems like a lot of reviewing, but remember many of these have not changed so it's just a matter of confirming that you have any new information. Also, after you do this once, the following years will be much easier because you already know where to go. Win!     Kicking off 2022 like an EXPERT Part 3

    Let's recap. So far we have covered your annual infection prevention program and annual training. Now let's move onto the disinfectant list review. Each year it's important for you to review the current list of disinfectants and obtain approval through the Infection Prevention Committee or whatever committee oversees the Infection Prevention Program. Word of caution. Make sure that the items on the list are the actual products being used by the cleaning staff. I have been surprised in the past by what I actually found on the cleaning cart. My best advice would be to take the list and do rounds with the Environmental Services leader to determine if the products match. Here are a couple of things to look for when doing rounds and talking to staff: EPA approval number on the label Appropriate contact time being used Correct PPE being used Right Disinfectants being used for the right conditions, i.e. bleach for C.diff Only products on the list being used Right disinfectant being used for the right surface, i.e. electronics, etc You may think of other items to look for when you start this process so add them to your list. If you start a list of things to observe this year, this process will be a breeze next year. As an added bonus, I included a helpful resource related to selecting disinfectants for your facility. Your list doesn't have to be fancy. You can create a simple list with the name, active ingredient, and contact time. Easy peasy!     Welcome to Part 2 of the series. Last week we talked about what you need to get done first. Hopefully, you have completed those tasks so we can continue to move forward. Next on the list is annual training. Do you know what annual training is required for your facility? While each facility has their own regulations to follow, I have made a list of the common types of training that are needed on an annual basis. OSHA Bloodborne Pathogens Training Fit Testing and Training for N-95 Respirators Hand Hygiene Influenza Vaccination and mitigation strategies COVID-19 Vaccination and mitigation strategies Device associated infections- CLABSI, CAUTI, Pneumonia Surgical Site Infections, if applicable High Level Disinfection, if applicable Multidrug Resistant Organisms Isolation Cleaning and Disinfection While this is not an all inclusive list, it covers the basics. If you are unsure what training is required, check your state regulations, accrediting agencies (TJC), and federal regulations (OSHA, CMS).     Let me officially welcome you to 2022! I am excited to start a new year, but first we need to do some preparation. I have talked about this in multiple emails and podcasts, but I feel the need to bring it up again because it is one of the most important strategies for getting your Infection Prevention Program on the right track. Not to mention it is a requirement for many facilities.     Conducting A Semi-Annual Review

    Can you believe it? We are halfway through the calendar year! How in the world did we get here already? I don't know about you, but this year is flying by. Maybe it has something to do with the pandemic that has taken up most of our time over the past year and a half! Regardless of the reason, it's time to start looking at your infection prevention data again. Why? Because you don't want to wait until the end of the year to find out that you have a problem and didn't meet your goals! I know what you're saying....I look at my data every month so why should I look at the last 6 months? While it's important to look at it monthly, you need to review the trends over several months. It's also a good idea to look at your numbers collectively to see if you are on track to meeting your annual goals. I prefer to do a semi-annual (January- June) report every year to see where I am in terms of the goals for the program. The other benefit to doing a semi-annual report is that is makes completing your annual report much easier because you already have the first 6 months done. After you complete your report, don't forget to present it to leadership and frontline staff so that they also know how the facility is performing. This might be hard to think about right now, but you will thank me at the end of the year! Speaking of reports, if you took my IP Mentor Course, you can use the Annual Report that was included as a template for the semi-annual report. Just change the dates and only look at 6 months instead of 12. Not familiar with the IP Mentor Course?     What to Cover in Orientation

    Are you responsible for new employee orientation? In my experience, most IPs have at least a section of the new employee orientation process. That being said, do you know what to cover? If not, I am going to share with you the basic topics you should cover for orientation. Before I give you the list, be sure you check your state, federal (OSHA, CDC, CMS), and any accrediting agency (TJC) requirements. Often times, there are specific education requirements for Infection Prevention in healthcare settings. This is the list I have used in the past, but you may need to add to it if you have additional requirements. Of course, I had to add COVID-19 to the list! OSHA Bloodborne Pathogens Isolation Hand Hygiene Device Associated Infections COVID-19 PPE Respirator Training if using N-95 respirators Tuberculosis Multidrug Resistant Organisms (MDRO) Exposure Procedures Influenza Cleaning/Disinfection Hopefully, this will help you develop your ongoing list for training.     3 Steps to Implement Your COVID-19 Plan

    Have you created a COVID-19 plan yet? Over the past couple of weeks we have discussed the new OSHA ETS and creating a COVID-19 plan since that is part of the requirement. That brings me to the next step, implementation. It's one thing to create a plan and another to implement the plan. Sometimes implementing is harder than creating! I have listed 3 steps to help you implement your plan this week. 1. Break the plan down into pieces (Testing, Isolation, Admission, etc) 2. Determine the audience for each piece 3. Create educational content for each piece and distribute I know this might sound like a lot to do right now, but you don't have to make it complicated! Break it down and start educating. If I were you, I would delegate some of this education if possible. After all, you are surround by smart people. You might as well use some of them to help you!     Implementing the OSHA Emergency Temporary Standard

    We have now reached the 14 day point since the ETS was published in the Federal Register (June 21st) where most the OSHA requirements should be in place. Here is the language from OSHA about implementation: Employers must comply with most provisions within 14 days, and with provisions involving physical barriers, ventilation, and training within 30 days. OSHA will use its enforcement discretion for employers who are making a good faith effort to comply with the ETS. Here are some resources for implementation: OSHA ETS Checklist SPICE Training Session OSHA ETS Fact Sheet Is your workplace covered by the ETS? There have been a lot of questions about aerosol generating procedures. Listed below is the definition according to OSHA. It's slightly different from CDC. AGPs are defined as medical procedures that generate aerosols that can be infectious and are of respirable size. Under the ETS, only the following procedures are considered AGPs: Open suctioning of airways; Sputum induction; Cardiopulmonary resuscitation; Endotracheal intubation and extubation; Non-invasive ventilation (e.g., BiPAP, CPAP); Bronchoscopy; Manual ventilation; Medical/surgical/postmortem procedures using oscillating bone saws; and Dental procedures involving: Ultrasonic scalers, High-speed dental hand pieces, Air/water syringes, Air polishing, and Air abrasion.     The OSHA Emergency Temporary Standard is official!

    As of June 21st, the OSHA Emergency Temporary Standard (ETS) is part of the Federal Register. This means that employers must comply with most provisions within 14 days of the publication. There are certain provisions that have 30 days for implementation. Here is the link to the ETS page on OSHA, and the complete standard.     Make these changes now or stay behind

    I recently heard this saying- "The only person who likes change is a wet baby". It's so true. If we are honest, none of us really like change. We like the results of making a change, but the change itself is usually hard. Over the past several months, we have seen lots of changes. Some good, some not so good. As an Infection Preventionist, it's imperative that we learn to roll with the changes because that's part of what we do. Guidelines, recommendations, and requirements are constantly being updated to reflect the most recent studies and science. The sooner you get on board, the better. So how can you respond to all the change from this past year? Embrace them and move forward! I want to help you do this.     Connect the dots

    Did you know one of your most important roles as an IP is to connect the dots for everyone? It's true. You are the expert with the knowledge about the infection data. It's important that you use this knowledge to connect the dots for leaders and frontline workers. I know what your are thinking. How do I do this? Well, it's actually as easy as pointing out how the infection is related to behaviors and how it impacts patient/residents. Here are some key points to get you thinking: If you have an increase in C.diff infections. Was your hand hygiene compliance low too? If so, connect those dots! Did you have an increase in CAUTI? Was Foley catheter care documented on those patients? If not, connect those dots! Did you have a new case of COVID-19? Was that person vaccinated? If not, connect those dots! Did you have a needle stick? If so, was that person using a safety needle or device? If not, connect those dots! As an IP, we are always looking for ways to help other understand their role in Infection Prevention as a whole. This can be done by connecting the dots for them.     Finding Infections

    Last week I discussed the key components of an Infection Prevention Program. This week I am going to talk about finding infections, more commonly referred to as surveillance. The key to surveillance is consistency. You need to find a published set of definitions for hospital acquired infections and use the same criteria every single time. This is the only way you can effectively monitor for changes and make comparisons. The most common types of definitions are NHSN(National Healthcare Safety Network) and McGeer Criteria. Here are some key points about performing surveillance: Review microbiology reports, re-admission diagnoses, repeat surgery diagnoses, and history & physical reports Review medical record to determine if the criteria are met Get a second opinion if you are unsure about criteria Calculate HAI rates using surveillance data Establish or adopt benchmarks for HAI data Report data to IP Commmittee Share trends with leaders and frontline staff It's important to watch for changes. The NHSN definitions are reviewed and updated every year in January. If others are helping you complete surveillance, make sure that the person is following the criteria consistently so that you data is accurate. It's also important to present your definitions to the Infection Prevention Committee. If you don't have a committee, I recorded a podcast about developing one.     Stop it from ever happening

    We want to stop bad outcomes from ever happening, right? Last week I talked about building the foundation of your Infection Prevention Program. This week I am going to point out the key components of the program so you can start identifying the gaps that will lead to bad outcomes for your residents and patients. Depending on the tool that is most relevant for your facility, the terms may be a little different; however the overall concepts and components are the same. I encourage you to go through this section and complete it for your hospital, but here are some key concepts. Establish a hand hygiene program and monitor for compliance. If you need a hand hygiene program that you can implement immediately, look no further! Hand hygiene program Monitor compliance with CDC guidelines to prevent healthcare associated infections- CAUTI, CLABSI, SSI, etc. Train staff on the proper use of PPE and monitor for compliance. Establish a cleaning and disinfection program and monitor for compliance. Educate staff on injection safety and monitor for compliance. Ensure that point of care testing is done appropriately. Monitor the process for device reprocessing. There are other components identified as well, but these are the main concepts. Complete this entire section for your facility. This will help you identify gaps in your program. Once you identify the gaps, you can start working on solutions.     Last week I talked about doing an assessment of your Infection Prevention Program. Did you have a chance to download the tool from CDC? If not, no worries! Go here to download your copy of the assessment. Pick the one that works best for your facility. We will be starting with the first section that addresses the infection prevention program and infrastructure. This is where it all starts! I encourage you to go through this entire section and answer the questions, but here are some of the main points. You need to have a designated Infection Preventionist with dedicated time for the program. This is essential! You need support for the program from administration and/or leadership. I talked about the best strategy to get this support in a podcast last month. Listen here. You need policies and procedures that are based on current guidelines and requirements from the state, OSHA, CDC, CMS, TJC, etc. You need an annual evaluation, risk assessment, plan, and goals for your program. I have templates on my website. You can find them here. I know this sounds like a lot of work, but it is essential to have a good foundation for your program.     I know, I know! You are tired of hearing about COVID-19. Me too! Unfortunately for us, it's not going anywhere yet. For that reason, it's essential that we are prepared for another outbreak and able to maintain our Infection Prevention Program. The bad news is we don't know how long this is going to last. The good news is there is an excellent tool to help you get your program up to par. Have you hear of an ICAR? If not, it stands for Infection Control Assessment and Response. It's a tool that CDC created to help facilities, hospitals, and specialty clinics assess their current program so that they can focus on the things that matter most. Over the past several months, I have been working with the CDC Foundation. This is one of the tools that we have used to help facilities identify what is missing in their program. I think it is very effective and useful for an IP. Go here to download your copy of the assessment. Pick the one that works best for your facility. Over the next few weeks, we are going to walk through the various components of this tool. I am going to give you tips and tricks for the different sections.     Are you checking the CDC, your state health department, OSHA, and CMS websites daily? If not, you need to start this habit today. If you are an APIC member (I highly recommend), they also have weekly updates on their website. It's so critical that you stay on top of current recommendations for your facility to build trust with staff and promote the safest practices for patients. If you don't have time to physically check all the websites, sign up for their emails if available. This will save you time and effort plus you will get the most accurate information. Here are some key updates from the past couple of weeks in case you missed them. CDC Updates for Vaccinated Healthcare workers CDC Guidance for Fully Vaccinated People Resuming J&J Vaccine CMS Nursing Home Visitation CMS Updates I know it's tough to think about anything other than COVID-19 right now, but we still have a job to do.
