Again and again, we learn the secret to stroke recovery is repetition. It's about doing the same movement or behavior again and again -- tens of thousands of times. In a typical session with an OT or PT, a patient might do the same exercise 30-60 times, which is a good start. But what if a therapist could crank that up to 1,000 reps an hour, or one every four seconds? Now you've got some interesting possibilities for recovery. Bionik, Inc makes devices and software that do just that. This week I talk with CEO Rich Russo about the InMotion Hand and InMotion Arm devices and how they work in conjunction with a therapist to help patients recover. From the Bionik website: Mr. Russo Jr. has over 15 years of finance and accounting leadership experience and is a Certified Public Accountant. From March 2017 through November 2020, Mr. Russo was the Vice President of Finance and United States Chief Financial Officer, of IcarbonX, a privately held digital health management company specialized in artificial intelligence and health data, and a predecessor PatientsLikeMe. While there, he was responsible for, among other things, the merger of three companies, fundraising, and the ultimate dissolution of certain affiliated companies. From 2007-2016, Mr. Russo held various key leadership roles for Nasdaq-listed companies in life sciences, pharmaceutical and medical device industries. From September 2015 to October 2016, he served as Corporate Controller for Pieris Pharmaceuticals, Inc., a clinical stage biotechnology company, and prior to that, he had roles at Juniper Pharmaceuticals, a woman's health company focused on developing therapeutics, and Cynosure, a medical device company focused on aesthetic treatment systems. In each of these roles, Mr. Russo was responsible for all finance activities and SEC reporting, including partnering closely with management to ensure effective and efficient financial procedures throughout the organizations. Mr. Russo started his career in 2005, where he served as an auditor at Pricewaterhouse Coopers in the assurance group. Mr. Russo is a graduate of Bridgewater State University in Bridgewater, MA, where he graduated from a dual degree program, receiving his Bachelor of Science in Accounting and his Masters in Management and Accounting. Other Inpatient Solutions The Bionik system is one for hospitals and rehab units. The rapid reps help in partnership with the rehab professional. In that respect, it's similar to devices from Restorative Therapies. I talked with the team at Restorative Therapies in episode 92. You can find that episode here.  The key difference is that Restorative Therapies uses Functional Electric Stimulation (or FES) to activate a patients muscles. The Bionik solutions provide physical assistance to help the patient complete motions. They are different ways to stimulate the brain, increase repetitions, and drive the neuroplasticity that is so key to recovery. They are also both intended for use in a hospital or rehab facility with the help of a trained therapist. The other devices I talk about often, like those from sponsor Motus Nova and previous guests with Neofect and Racoon Recovery are for at home use, as a supplement to therapy provided at a medical facility, or as an alternative when those services are not available, for whatever reason. The point of all these solutions is the same -- drive patient recovery through increased movement and repetitions to neureoplastically teach to brain how to access that limb once again. Hack of the Week Wear comfy socks. It's such a simple thing, but the right socks can make a big difference in how you feel. The right socks can wrap your feet well and wick away perspiration. They can protect your foot from rubbing against an AFO or the heel of your shoe. The wrong socks will keep you too hot or too cold. The wrong size will leave you with an uncomfortable wrinkle you walk on all day. If they're too slippery, you've got an additional safety hazard to contend with. The hospital socks they gave me in the hospital were terrible. The had the no slip dots, which was great, but they kept falling down and rotating around my foot. Part if it was they were likely the cheapest that met minimum standards. The other part is that I have large feet for my height (size 12). So my partner ordered me better hospital socks from Amazon,* which helped. For air travel after stroke, I have made a change to my wardrobe. I now wear knee-high compression socks.* They do a good job of preventing swelling in my feet and legs during long flights. And that helps to prevent DVT or deep vein thrombosis, which is where a clot forms in the legs and causes problems there, or breaks loose and lodges elsewhere in the body. That's how Ted Baxter had his stroke. I talked with Ted back in Episode 34. Relatively speaking, good socks are still fairly cheap. Try different ones until you find the socks that are best for your feet and life style. Good socks are worth it. Links Where do we go from here? Scott Kim is the Co-Founder and CEO at Neofect. Neofect provides technologies that work to provide stroke victims with rehabilitation devices that can increase their range of motion. These devices include smart gloves and ankle braces. In this episode, Scott Kim discusses how Neofect technology is assisting stroke victims.    Top three takeaways:  The user receives a smart-glove and a tablet with the Neofect App.  Each therapeutic session consists of eight different “training games” that allow users to regain mobility lost from a stroke injury.  This technology allows users to achieve rehabilitation results from their own homes.      [0:00] Ladan introduces the episode and the guest, Scott Kim.     [2:40] Scott Kim describes the beginnings of Neofect and their devices.    [5:40] The smart glove works by measuring six targets of motion guided by the therapist coach.    [7:50] An initial range of motion for each activity is captured as a baseline, then progress is determined over time through improved performance.    [10:55] The therapists at Neofect report a 10% improvement in the range of motion of users.    [15:00] While the main target with the device is stroke victims, the technology can be applied to other injuries in the future.    [18:00] The company recently launched Neofect Connect, which gives users daily reading and training games.

It’s been awhile since I checked in with the folks at Neofect so in this episode I chat with CEO and co-founder Scott Kim. If you follow the technology and gear around rehab, you’ve probably heard of Neofect. They make a product called the Smart Glove. I talked about it with OT Lauren Sheehan a couple years ago at http://Strokecast.com/Lauren. You can listen at that link. The Smart Glove is a plastic exoskeleton with very sensitive sensors that you wear on your stroke affected arm. Then you play video games with that arm. The sensors detect the slightest motion and translate that motion into in-game activities. It encourages you to do more therapy by making it more engaging. It makes getting those thousands of reps in that much easier. We talk about that, enhancements to the product, new products, and some of the decisions a CEO has to make Bio Scott Kim is the co-founder and CEO of Neofect USA, a digital health company that creates customized digital rehabilitation solutions for patients with neurological and musculoskeletal injuries. Scott met his business partner, Hoyoung Ban, at the Darden Business School at University of Virginia and they quickly connected over personal experiences with the healthcare system and rehabilitation. Scott was born with spina bifida, so after having major surgery on his back as a child, he spent tons of time in rehabilitation. After moving to the U.S. for college, Scott started to notice the gaps in the healthcare system — and that gave him an idea of how he could contribute and make the process more engaging. Before starting Neofect, Scott worked as a management consultant, a product manager in the software industry and for mobile gaming companies such as GREE, Z2Live (which is now Activision Blizzard) and 505 Games. He founded a couple startups before using all his experience to create Neofect in 2010. Scott now leads a national team from Neofect’s U.S. headquarters in San Francisco. About Neofect Neofect’s creates gamified rehabilitation solutions for patients recovering from stroke and other neurological and musculoskeletal conditions. This started with the Neofect Smart Glove and has progressed to include the Neofect Smart Board, Neofect Smart Pegboard, Neofect Smart Kids, Neofect Cognition, Neofect Smart Balance, and the NeoMano robotic glove for functional assistance. When Scott Kim (MBA ’11) was undergoing grueling and lengthy rehabilitation after childhood surgery for spina bifida, little did he know that years later he’d be heading Neofect, a med-tech startup that brings game-based physiotherapy to millions of patients worldwide. What launched Kim on an entrepreneurial career path was a class taught by Darden Professor Saras Sarasvathy and a serendipitous encounter with a fellow Korean-born Darden student Hoyoung Ban, whose family members had struggled with stroke recovery. Inspired by Sarasvathy, Kim and Ban set out to revolutionize what they saw as an archaic approach to physiotherapy. In 2010, they launched Neofect, which has since raised $17 million in a Korean IPO. Scott called in from his hometown of San Francisco (and was appropriately sporting a Warriors jersey) to tell them about his journey to success in creation of the Rapael Smart Glove. As with other innovators and entrepreneurs they have spoken with previously - Scott outlines the difficulties and barriers he faced and how he dealt with them. Background: Scott is the CEO of NEOFECT USA, and one of the founders of its parent company, NEOFECT, a rehabilitation technology company headquartered in South Korea. He wore several different hats before joining Neofect – a management consultant and a product manager in the software industry. Scott worked for mobile gaming companies such as GREE, Z2Live (which was acquired by King.com, a "Candy Crush" maker that was later acquired by Activision Blizzard) and 505 Games, and founded a couple of startups. NEOFECT went on an IPO in November 2018, currently traded on the KOSDAQ. Scott was born with spinal bifida and has undergone surgery and understands the long process of rehabilitation. Spina bifida is a birth defect that occurs when the spine and spinal cord don't form properly. It falls under the broader category of neural tube defects. His prior experience in the gaming software industry has also contributed to his ability to create the games and software.

Today's Guest I met Lauren Sheehan through Olivia, the awesome OT who co-starred with me in an ad for Swedish. Swedish had recently acquired a Rapael Smart Glove to work with patients and wanted to get my thoughts. I met Lauren a few weeks later to talk more about her work and the possibilities of Neofect's tool. Neofect did send me a unit to try out and provide feedback on. So far, it's promising. I'll provide a more thorough review once I really put it through its paces. This week, I interview Lauren. We talk about why she got into OT, the idea of the "art" of life, and the nature of gamification in therapy. One of the key takeaways from Lauren's story is how she came to work with Neofect -- building relationships and talking with folks. The more folks you can engage with on a regular basis -- online or in the real world -- the more opportunities that become available to you. Maintaining those relationships doesn't need to be about what they can do for you though; it's about keeping interesting people in your life. Because I maintained a good relationship with my inpatient therapy team, I met and got to work with Lauren, for example. That wasn't a plan. It was the out growth of life. Plus, staying engaged in a social life is good for brain health. So who is Lauren Sheehan? Lauren has been a practicing occupational therapist for over 10 years.  Her dream to join a technology company was realized when dreaming about the possibility of doing something “outside the box” after spending the last decade in outpatient neuro clinical practice and most recently in administration and management roles.  Lauren has served in various roles on her state occupational therapy associations and is a proponent of being an active member, particularly in advocating for occupational therapy through legislation and contact with elected officials.  She has enjoyed planning and organizing Washington state’s “Hike the Hill” event for the last three years.  She has also served as the AOTA Representative Assembly Member for the state of Washington.  Lauren believes that OT professionals are poised to be product designers, user experience experts and consultants as it relates to technology solutions that meet the needs of individuals with disabilities. Neofect is also launching a Kickstarter for a new product on October 30, the day after World Stroke Day.   Hack of the week: Wear an apron I've started wearing a kitchen apron around the house because it has big pockets. Carrying stuff is a challenge these days. With hemiparesis, I lost access to the pockets on the left side of my body. Plus, my working hand needs to hold the cane, handrails, phone, or other stuff as I go room to room. An apron with big front pockets is a great solution for hauling more stuff around with me. And tying the apron strings is great OT.
