In this episode I am once again joined by Dr Ben Joffe, anthropologist and scholar practitioner of Tibetan Buddhism. This episode is the third part of a series in which we explore Ben’s PhD dissertation about the ngakpa of the Himalayas, or as Ben calls them ‘wizards and socially engaged yogis’. In this episode we explore the realms of gender and sexual orientation, in both textual and social contexts. Ben discusses the complex situation of female spiritual practitioners in the Tibetan Buddhist tradition, looking at terms such as ‘khandroma’, ‘dakini’, and more. We learn about the role of the sangyum, or sexual consort, and find out why Ben’s teacher and co-author Dr Nida Chenagtsang dedicated their book on sexual yoga to abuse victims, particularly those who have been abused in religious contexts. Es wurden zwei heterogene Patientenkollektive mit unterschiedlichem onkologischen Therapieansatz - Strahlentherapie allein vs. Radiochemotherapie - bzgl. ihrer Lebensqualität, ihrer medizinischen und soziodemografischen Daten miteinander verglichen. Hierbei zeigte sich, dass die Patienten mit einer Radiochemotherapie älter waren, einen niedrigeren Karnofsky-Index hatten und sich in einem fortgeschritteneren Krankheitsstadium (bzgl. der TNM-Klassifikation) befanden. Zudem wurden sie zu ca. 90% stationär behandelt. Patienten mit einer Strahlentherapie allein wurden hingegen in einem annähernd ausgeglichenen Verhältnis stationär und ambulant behandelt und zeigten einen sig. höheren Anteil an weiblichen Patientinnen. Bzgl der soziodemografischen Daten fand sich nur ein geringer Unterschied beim höchsten Schulabschluß. Trotz der unterschiedlichen Ausgangssituation fanden sich nur geringe Unterschiede in der Lebensqualität beider Gruppen. So waren die Patienten mit Radiochemotherapie etwas höher belastet (FBK), hatten phasenweise (zu Ti2) höhere Depressionswerte (SDS) und zeigten zu Ti2 und Ti3 ein geringeres körperliches Wohlbefinden (FACT-G). In den Fragen zum Betreuungsbedarf (BB) fanden wir bei diesen Patienten auch ein verstärktes Interesse für eine pflegerische Betreuung zu Hause. Keine Unterschiede zwischen beiden Gruppen sahen wir in der Lebenszufriedenheit (FLZ), in der Krankheitsverarbeitung (FKV) und in der sozialen Unterstützung (SSS) der Patienten. Physische Parameter wie der Karnofsky-Index konnten also nicht ohne weiteres als Maß für die Lebensqualität von Patienten genommen werden, da hohe Werte hierbei nicht zwingend mit einer hohen Lebensqualität korrelierten und vice versa (Mackworth et al. 1992). Zudem wurde von Patienten die Toxizität einer Hochdosispolychemotherapie nicht unisono mit einer geringeren Lebensqualität gleichgesetzt (Coates 1987, Richards 1992). Trotzdem fanden sich in der Literatur auch Hinweise auf den Einfluß medizinischer Parameter beispielsweise auf die Krankheitsverarbeitung (Buddeberg 1996, Faller et al. 1997) oder die soziale Unterstützung (Dakof und Tailor 1990, Ell et al. 1992). Die Lebensqualität der Patienten mit einer kombinierten Radiochemotherapie war insgesamt nicht schlechter zu bewerten als die der Vergleichsgruppe.
