ONCE UPON A GENE - EPISODE 167 Care Team Prescription - The Importance of Clinical Pharmacists with Chase Palmer Chase Palmer is a pharmacist on the board of Alström Syndrome International. In rare disease, we need all the help we can get, and he's motivated and he's jumped right in to share his voice. He's been dubbed the rare disease pharmacist– and I think it's going to stick.  EPISODE HIGHLIGHTS Can you introduce yourself and tell us how you became involved in rare disease as a pharmacist? In 2020 I started working with a nonprofit called Alström Syndrome International. We work with persons living with Alström syndrome, which is a rare genetic disorder that affects almost every cell and organ in the body, starting with blindness and deafness and leading to major organ failure. I was approached by ASI to become a member of the board and began meeting with patients living with rare disorders. I've been trying to learn more and more as time goes on to hopefully treat and cure Alström syndrome and other rare disorders.  What does it mean to practice as a clinical pharmacist? A clinical pharmacist is a hospital pharmacist or a pharmacist with a specialty. Some clinical pharmacists work in a specialization like cardio or infectious disease. They work with patients in a hospital, acute care setting or ambulatory care setting to monitor medications and understand side effects and interactions to provide the best quality of life possible. What approach can pharmacists be in helping to manage the care of kids with rare disease? For patients taking multiple medications, I think it's important to have a pharmacist on the care team who is monitoring drug interactions to ensure medications aren't negatively affecting organs and making sure they're safe and effective, making things better and not worse.  Do you have any advice for getting drugs covered by insurance companies? Insurance can be a major barrier and they may require patients to try alternative medications first. If that happens, make sure the insurance company's preferred medication is going to have the same positive result and not have any interactions with other medications or side effects. Sometimes a doctor can submit a pre-authorization and get medications covered that way.  Do you recommend that patient advocacy groups connect with pharmacists as board consultants? I think it's a good idea to have a pharmacist on the board and it can be really progressive and allows for multi-disciplinary care for rare diseases and genetic disorders. A pharmacist can help to manage medications, consult with other advocacy groups and other pharmacists and optimize care. I think they play an important role in moving rare disease forward and meeting the needs of patients. Naked Singularity, based on the novel by Sergio De La Pava, is about an idealistic young New York City public defender burned out by the system who decides to rob a multi-million drug deal of one of his clients.   Chase talks to Write On about adapting the novel for the screen. "The unifying voice of the film, I hope, comes from the book, which is the reason I wanted to do this. The author, Sergio De La Pava, is a public defender of 20 years; his wife, Susanna, is a public defender of 20 years, and he wrote this book that is a very well observed, funny, absurdist, kind of outraged novel that juggles in all of these different genres, but there's something very specific about his voice. It was like a new way into a discussion about social justice, and it was sort of a new way into seeing a story about a young public defender."   Chase goes on to discuss the challenges of adapting: "With [adapting] a book, the first thing is 'What's the spirit of the book?' and trying to really get into your head, like what do you love about it? And what's the spirit of it, before even getting into the nuts and bolts of what characters are you going to cut and consolidate and plot points? On this episode of The Harold & Maudecast's Justice and Doom Now Playing Movie Reviews, Sarah Mason and Jake Essoe review IT. A new screen adaption of Stephen King's novel "It" has become the unlikely savior of the Summer box office shattering records with a $123m opening weekend. The novel which was originally published in 1986 was first adapted into a television miniseries in 1990. Set in a small town in Maine in the 1980s, the story follows the interactions of seven children who in addition to being stalked by a bully and his gang, are being hunted by a killer being who appears [mostly] in the form of a clown named Pennywise. The book weaves in and out of time periods alternating between the children's story and their reunion as adults. In this film it's all about the kids--a narrative choice which seems chosen solely to capitalize off the success of Stranger Things. Unfortunately, this film does not share the same charm and carefully crafted nostalgic atmosphere. The unknown and underwhelming cast of kids in IT (which does include one of the actors from Strangers Things, Finn Wolfhard), struggles to carry the film when Pennywise is not on the screen. Bill Skarsgård, who takes over the role of Pennywise from Tim Curry (in the 1990 series), gives a career-defining performance. Terrified of clowns or not, it was hard not to crave his presence on screen. The bond between the kids, the self-named "Losers Club", which is so beautifully developed in the book and the 1990 miniseries, seems thrown in only as necessary for their joint encounters with Pennywise and bully Henry Bowers. DIrector Andrés Muschietti (MAMA) took the helm after Cary Fukunaga (True Detective) left for creative differences. Fukunaga along with Chase Palmer and Gary Dauberman are credited with the script. Can't help but wonder what those creative differences were and if Fukunaga's vision would have been more fulfilling. A sequel is already in the works which will undoubtedly feature the grown ups. If you have not read the book, love experiencing shared scares, and are NOT hopelessly afraid of clowns you will enjoy seeing IT in the theater. We give IT a 7.2 for Bill Skarsgård's performance and some visually intriguing oh-so scary creatures. For more reviews visit out website at www.thehmcnetwork.com
