Motivation will always wash of, it could be in 5min an hour or a day, at some point it will leave you and this is when your habits will kick in. So it is worth asking yourself what habits have I set for myself, will they keep me on track for the life that I want live or are they going to take me back down the path that led me to where I am unhappy with myself and the life I have created.
In terms of thinking about your goals in life try to think of it in terms of how to I get here, instead of how do I get there. When you do this you lock in your mind a better belief in your ability to actually achieve the outcome you are after. Add some emotion into the mix and you are well on your way to being the person you dream of becoming.
It is always better to take action yourself than to wait around for someone to take action for you. Every successful change in life comes from a plan, not from a dream.

We only truly fail when we stop. There is a difference between failure and giving up. Change your outlook and never fail again.
Goals are great and I will always tell people to create and hunt them down, but somethings it gets to much for some people the distance between where they are right now and where they want to be is so far they self sabotage as they become afraid and overwhelmed. This is a great way to stop that from happening.
