The COVID-19 pandemic forced Americans inside their homes and restricted access to critical services, including health care. However, telehealth technology that was previously only used by a fraction of patients turned out to be a reliable way for all Americans to access health care services from the safety of their homes. A new bipartisan report by the Progressive Policy Institute (PPI) and Americans for Prosperity (AFP) found that over the duration of the pandemic, the costs of telehealth care fell, suggesting that in a post-pandemic world telehealth could help increase access to medical care without increasing costs. The report, authored by PPI Director of Health Care Arielle Kane and Americans for Prosperity Health Policy Analyst Charlie Katebi, found that at the outset of the pandemic, telehealth use skyrocketed, as did costs for those who utilized it versus those who didn't. But as time went on, the average telehealth patient spent less on health care services than the average in-person patient. In the final month of the study, telehealth patients spent 26% less than the average in-person patient. Further, when excluding the first three months of near complete lockdowns, telehealth patients had lower overall health care utilization. Capturing State Legislatures State capture refers to the idea that a set of organizations, businesses, and movements can capture a political office and dictate its agenda, decisions, and resource allocation to benefit their interests. Capturing state legislatures is especially effective because state governments – as opposed to the federal government – have control over significant aspects of our daily lives: taxes, minimum wage, health insurance, and administering elections.  The Troika Three powerful conservative organizations, commonly referred to as the troika, work in tandem to capture state legislatures: the American Legislative Exchange Council (ALEC), the State Policy Network (SPN), and Americans for Prosperity (AFP). ALEC works with lawmakers directly to pass legislation it often writes and provides. SPN is a network of think tanks that works outside of government, creating reports, legislative testimony, and polling that champion conservative bills often created by ALEC. AFP operates like a political party with national, state, and local offices, all aimed at electing conservative lawmakers around the country. Public Policy Changes Politics Public policy can and does change politics. The troika has successfully promoted the adoption of so-called “right-to-work” laws, which weaken labor unions. These laws make it more difficult to unionize, collect dues, and support pro-labor candidates for office. In fact, they are a direct response to the unionization of public sector workers and their successful organizing, specifically the National Education Association in the 1960s-70s. Once anti-labor policies were in effect, it became easier for conservatives to continuously win elections and cement their political power. This libertarian/conservative political advocacy group is funded by David H. Koch and Charles Koch and has over 3,200,000 members in 36 states. https://youtu.be/lsY892pYetYAfter the 2009 inauguration of President Barack Obama, AFP helped transform the Tea Party movement into a political force. It organized significant opposition to Obama administration initiatives such as global warming regulation, the Patient Protection and Affordable Care Act, the expansion of Medicaid and economic stimulus. It helped turn back cap and trade, the major environmental proposal of Obama's first term. AFP advocated for limits on the collective bargaining rights of public-sector trade unions and for right-to-work laws, and it opposed raising the federal minimum wage. AFP played an active role in the achievement of the Republican majority in the House of Representatives in 2010 and in the Senate in 2014.
