From a global perspective, there are four main models of health care, each of which takes a different approach both legislatively and in practical terms of how it affects a populace's ability to obtain health care coverage and services. In 1929, Blue Cross established the first employer-sponsored health coverage in Dallas as a partnership between the Baylor University hospital and its patients. One of the largest health care acts in American history, commonly referred to as the Medicare and Medicaid Act, was approved by President Lyndon B. Johnson in July 1965. After intense debate, lawmakers passed the Patient Protection and Affordable Care Act (PPACA) or Obamacare, which was signed into law on Mar. 23, 2010.

A fare le domande, Riccardo Alcaro, coordinatore delle ricerche dell'Istituto Affari Internazionali. Chi risponde è Mario Del Pero, illustre americanista e Professore di Storia Internazionale presso SciencesPo a Parigi. Oggi parliamo del 44° presidente degli Stati Uniti, il Democratico Barach Hussein Obama, in carica dal 2009 al 2017. Obama ha libertà di governare sul piano domestico solo per il biennio 2009-10 in cui i Democratici controllano tanto la Camera quanto il Senato, dove peraltro hanno una supermaggioranza di 60 senatori (fino alla morte di Ted Kennedy). Dopo che i Repubblicani torneranno a controllare la Camera (dal 2011) e poi il Senato (dal 2015), di fatto Obama riuscirà a fare poco in politica interna, e tutto quello che farà (in particolare sul fronte delle regolamentazioni ambientali e della protezione dei migranti entrati negli USA come minori o neonati) sarà soggetto a cause legali, a volte perse dell'Amministrazione. Nei due anni in cui può governare Obama ha due priorità: continuare e anzi accelerare la gestione della macro-crisi del 2008 e avanzare un'agenda di governo di ispirazione democratica. L'azione dell'Amministrazione Obama si traduce soprattutto nella promozione di tre grandi leggi approvate dal Congresso: La prima è il grande stimolo fiscale (del valore di circa 900 miliardi di dollari; la seconda legge è il Wall Street Reform and Consumer Protection Act, la riforma del mercato finanziario nota come Dodd-Frank; l'ultima grande legge fatta varare da Obama è naturalmente il Patient Protection and Affordable Care Act, ovvero la grande riforma sanitaria nota come Obamacare. In politica estera Obama abbandona la retorica della Guerra Globale al Terrore, ma intensifica l'uso di droni per colpire sospetti terroristi, nonostante le controversie legali ed etiche per le vittime civili. In Europa rilancia la collaborazione con l'UE, che vede come un pilastro dell'ordine globale a guida americana e opera il reset con la Russia di Putin (ottenendo aiuto sull'Afghanistan, Iran e New Start); ma non prevede – e non si oppone con sufficiente durezza all'invasione russa dell'Ucraina del 2014 che risulta nell'annessione della Crimea e la destabilizzazione del Donbas. Le difficoltà in Medioriente ed Europa gli impediscono di dare maggiore sostanza al famoso pivot to Asia. The Affordable Care Act (ACA) is the comprehensive healthcare reform signed into law by then-President Barack Obama in March 2010. Formally known as the Patient Protection and Affordable Care Act and commonly referred to as Obamacare, the law includes a list of healthcare policies intended to expand access to health insurance to millions of uninsured Americans.1 The law expanded Medicaid eligibility, created health insurance exchanges, mandated that Americans purchase or otherwise obtain health insurance, and prohibited insurance companies from denying coverage due to preexisting conditions The ACA was designed to reform the health insurance industry and help reduce the cost of health insurance coverage for individuals who qualify. Ambassador Emanuel became the 31st U.S. Ambassador to Japan in December 2021, prior to his current role he served as the 55th Mayor of the City of Chicago.About the Speaker:Rahm Emanuel was confirmed in a bipartisan vote as the 31st United States Ambassador to Japan on December 18, 2021. Previously, Ambassador Emanuel was the 55th Mayor of the City of Chicago, a position he held until May 2019. During that time, he made the critical choices necessary to secure Chicago's future as a global capital.As Mayor, the Ambassador added four years to a student's education. He increased the school day by 75 minutes and added more than 200 hours to the school year, marking the largest single increase in educational time by any city and taking Chicago from having the least educational time of any large school district in the country to being on par with its peers. All Home Care Matters was privileged to welcome Mr. Ian Kremer, J.D. as guest to the show. Ian is the Executive Director of the LEAD Coalition (Leaders Engaged on Alzheimer's Disease). Ian N. Kremer, JD, has worked on federal, state and local dementia policy since 1996. Since 2012, Kremer has served as Executive Director of the LEAD Coalition (Leaders Engaged on Alzheimer's Disease: http://www.leadcoalition.org), the uniting voice of over 200 member and allied organizations. The LEAD Coalition accelerates transformational progress in care and support to enrich the quality of life of those with dementia and their caregivers, detection and diagnosis, and research leading to prevention, effective treatment, and cures. The LEAD Coalition has helped to secure historic funding increases for the National Institutes of Health (NIH), expand Medicare services for people with dementia and protect dementia-relevant components of Medicaid and the Patient Protection and Affordable Care Act, expand the role of people with dementia and their care partners in medical product development, build a nation-wide network of dementia-friendly communities, and worked with a dozen federal agencies to overcome health disparities, clarify regulatory pathways, combat elder abuse, and improve cognitive impairment detection and diagnosis, clinical care, and access to home and community-based services. Kremer was an external reviewer for the 2021 National Academies of Science, Engineering, and Medicine report, “Meeting the Challenge of Caring for Persons Living with Dementia and Their Care Partners and Caregivers: A Way Forward.” Kremer also has served on steering and advisory committees for a wide variety of organizations and projects including the International Collaboration for Real-World Evidence in Alzheimer's Disease in the US (ICARE-AD-US) Study, the Gerontological Society of America Workgroup on Cognitive Impairment Detection and Earlier Diagnosis, the PCORI Dementia Research Methods project, the Brain Health Partnership, the Alzheimer's Disease Partnership for Evidence and Value (AD EVAL), and Dementia Alliance International. The Government Accountability Office (GAO) is recommending expansion of the Medicaid Recovery Audit Contractors (RACs) program, to include auditing of payments made to managed care organizations (MCOs), according to a GAO report released in June.Launched in 2011 as a consequence of the Patient Protection and Affordable Care Act (PPACA) by the Centers for Medicare & Medicaid Services (CMS), the Medicaid RACs were purported to be aggressive in their recouping of alleged Medicare Fee-for-Service (FFS) overpayments. She saw the Opioid epidemic as a crisis and wanted to help so she joined the NSC which provides resources to workplaces for mental health. Youturn Health is partnering with NSC to provide a comprehensive program to organizations that will impact workplace impairment. Jenny Burke advances the National Safety Council's mission of eliminating preventable deaths in our lifetime by leading NSC advocacy initiatives. These strategic programs, involving safety on the road and in homes and communities, fatigue, transportation, and prescription drug overdose, raise awareness and educate audiences to keep each other safe. Jenny has raised awareness of these initiatives through such successful national efforts as benchmarking employer data on the opioid epidemic and its impact on the workplace and creating cost calculators to illustrate the effect of opioids or fatigue on an employer's bottom line. Additionally, she oversees the development of educational programs that tackle cutting-edge issues, including deaths and injuries related to fatigue and addiction. Jenny previously served as a senior legal analyst at Wolters Kluwer, specializing in Medicare, Medicaid, food and drug law, and policy and healthcare compliance. Jenny co-authored and managed the publication of several books and white papers on areas of focus including the Patient Protection and Affordable Care Act, Medicaid expansion, contraception coverage, prescription drug abuse, and telemedicine. She is a licensed attorney in Illinois since 2002 and clerked for two years in the Illinois Appellate Court, First Division. MICRA, the Medical Injury Compensation Reform Act, was put in place in 1975. Since then, stakeholders have been battling over its cap on noneconomic damages for medical malpractice. Assembly Majority Leader Eloise Gomez Reyes brought the stakeholders together with AB 35, ending the decades long fight. He is also the Director of the Health Care Program at the National Bureau of Economic Research, and the former President of the American Society of Health Economists. A member of the Institute of Medicine, the American Academy of Arts and Sciences, the National Academy of Social Insurance, and the Econometric Society. He has published more than 175 research articles, has edited six research volumes, and is the author of Public Finance and Public Policy, a leading undergraduate text, Health Care Reform, a graphic novel, and Jump-Starting America: How Breakthrough Science Can Revived Economic Growth and the American Dream (with Simon Johnson). In 2006 Dr. Gruber received the American Society of Health Economists Inaugural Medal for the best health economist in the nation aged 40 and under. During the 1997-1998 academic year, he was on leave as Deputy Assistant Secretary for Economic Policy at the Treasury Department. And from 2003-2006 he was a key architect of Massachusetts' ambitious health reform effort (aka Romneycare), and became an inaugural member of the Health Connector Board, the main implementing body for that effort. During 2009-2010 he served as a technical consultant to the Obama Administration and worked with both the Administration and Congress to help craft the Patient Protection and Affordable Care Act (aka Obamacare). In 2011 he was named “One of the Top 25 Most Innovative and Practical Thinkers of Our Time” by Slate Magazine. In both 2006 and 2012 he was rated as one of the top 100 most powerful people in health care in the United States by Modern Healthcare Magazine.

But there's one list you'll certaintly want your hospital to avoid.A report from Johns Hopkins University has revealed that many of America's top 100 hospitals are using predatory tactics to pursue lawsuits against patients. WASHINGTON, DC – The Ripon Society hosted a health care policy forum with three doctors currently serving in the U.S. House of Representatives, who not only dissected the Patient Protection and Affordable Care Act that was signed into law last year, but presented what they see as the “Top 10 Reasons” why the law needs to be replaced. The doctors taking part in the event included Congressman Michael C. Burgess (TX-26), Congressman Dan Benishek (MI-1), and Congresswoman Nan Hayworth (NY-19). Burgess, who also serves as Vice Chairman of the Energy & Commerce Committee's Subcommittee on Health, opened the forum by pointing to one of the unintended consequences of the 2010 legislation – namely, that it makes innovation and new product development even more difficult than it already is.
