Join me for a summary looking at miniscrews, looking at where the answer to successful TAD placement lies, in research or clinical practice. The reasons for higher failure rates than others with TADs was explored through 3 key factors; insertion torque, site selection and root proximity. Evaluation of both scientific and clinical processes were described by Sebastian Baumgartel at the British Orthodontic Conference, as the Northcroft lecture. Is torque a factor in TAD success? Torque study – compression during insertion Motoyoshi 2006 · High torque – 60% · Low torque = 72% · Medium torque – 92% Understanding · Low torque = low compression, low primary stability - early failure as not engagement with screw · High torque = high compression, early success, but greater resorption after insertion, remodelling results in a resorption process · Medium = best of both = sufficient compression for primary stability, not high enough to cause resorption remodelling Ideal · Ideal torque range – 10 Ncm Shantavasinkal 2016 o Study of buccal tads · Sebastian's empirical experience between 10-25Ncm depending on site Rules: · Aim for medium torque · Target 10Ncm · Exceed 10Ncm on palate acceptable What is the best site for TAD insertion? Keratinised gingiva · Evidence - states no difference Lim 2009, Chen 2008, Park 2006, Cheng 2004 · Non Keratlised – depends on mobile or non mobile, with non-mobile higher success rate Viwattanatipa 2009 · 2mm apical to muco-gingival junction o zone of opportunity Target zones and site o No roots o Consistent cortical bone o More tolerant to higher torque o Attached gingiva with low mobile mucosa Is there ideal bone? · = if ideal torque = ideal cortical plate thickness § 1-1.5mm cortical plate thickness · CBCT can be overkill, using research sites for average sites Ideal site: – 1st premolar region (transverse) Sebastian 2009 – 2 mm away from mid-palatal suture o = creates ideal zone ‘Mx1' Evidence of site selection success · 98% Vs buccal 71% Houfar 2017 · 84% Trainee success Sebastian 2020 · Success of Sebastian anterior palate 100%, maxillary buccal lowest 85% Does root proximity influence TAD success? · Not just contact with roots, but proximity to root also causes failure Kuroda 2007, Asschericks 2008, Chen 2008 Understanding o Increase root and PDL proximity = bone stress increases = increase bone turnover = increase failure of TAD · 4mm interradicular distance needed (depending on size of tad) to achieve 1 mm clearance from roots · Most buccal sites have less than 4mm (resolve through diverging roots, or sites with no roots) What happens if TADs fail and we try again? – Secondary insertion success o 58% (reduced by 33%) Park 2006 o 44.2% (reduced by 36%) Uesugi 2017 o 58.1% buccal (reduced by 21%), 88.9% palatal (increased by 4%) Uesugi 2018 § Uesugi 2018 showed buccal failure increases for secondary insertion, but palatal insertion increases success For more education see Sebastian's TAD course: https://tadchallenge.com/tad-certification-course I have no financial interest

In our 13th instalment of the series, Japanese musician Yasuyuki Uesugi shares a 30 mins-long track specially produced for CLOT Magazine. Truths is a noise track that expresses the question of whether all the truth should be conveyed or how hidden it should be to live. Uesugi produces wall of sound kind of noise music, using almost a fully analogue workflow, u including a custom-made synthesizer made by JMT SYNTH (Japanese handmade synthesizer maker) the NSDQ, a 2VCO synthesizer equipped with multiple sub oscillators, delay effector, phaser effector, and a 16-step sequencer. Once the parameters on the instruments are set, Yusuegui doesn’t physically play, the noise changes and evolves due to the effects of the analogue instruments and spring reverb, recording all in one take and letting each passing moment decide where my sounds go The producer started being interested in noise music when he met the mind behind JMT SYNTH: I became fascinated by operating the analogue synthesizer and freely searching for noise sounds, which became part of my music style today. Uesugi gui productions as himself defines are “deeply experimental noise tracks that reflect ideas of the spiritual world, mental illness and the human brain”, and as he shares with us: "I use this NDSQ to create noise music that expresses my distorted spiritual world. I have a mental illness, so I express it in the form of noise". This is well reflected both in the music, harsh, abrasive - almost as if something was to be pulled from one’s brain-, but and also in the titles and track names in many of his prolific productions. He has self-released a bunch of EPs and albums and also published in labels such as Drvg Cvltvre’s New York Haunted. In times where the disconnection with a gone reality is pushing, when our minds are stressed and damaged, listening to Yasuyuki Uesugui’s productions can be a cathartic noise exorcism.

In the Sekogu period, Japan was divided into a large number of small provinces, each ruled by powerful daimyo and all at war with each other. Two of the most important daimyo were Takeda Shingen and Uesugi Kenshin. They seemed to constantly match each other. Takeda Shingen was known as the Tiger of Kai, while Uesugi Kenshin was the Dragon of Echigo. On a mountain plain known as Kawanakajima, the island between the rivers, they met in battle repeatedly. The Fourth Battle of Kawanakajima was their biggest and most well remembered engagement. Shingen had 20,000 men, while Kenshin held 18,000. Shingen took the first strategic move, sending a few thousand men toward the Uesugi forces to surprise them at night, forcing them into his main army's crane wing formation. Instead, Kenshin moved his troops toward the Takeda force first, with what was known as the winding wheel. This whirling formation kept fresh troops in the vanguard, and placed Kenshin in Shingen's tent. Shingen could only parry Kenshin's sword thrusts with his war fan. In the end, Uesugi Kenshin was driven back by Takeda bodyguards. The battle was astonishingly bloody, frustratingly inconclusive, and instantly legendary.
Back to the Dungeon i Dungeon of Mandom VIII.

Hello listeners! For the next few episodes we'll be airing what was originally meant to be a seperate podcast on the Samurai Archives podcast network. In these bonus episodes, your hosts will tackle things from their own perspective from outside academia in what is intended to be a comedic and free flow format where they will debate, attack, pontificate, and throw things under the bus in the name of entertainment, intellectual discussion, and philosophizing. Often in these episodes, in the name of debate and spurring discussion, strong, adamant or outright offensive opinions may be expressed, therefore it should be clearly stated up front that everything said in these bonus episodes reflect strictly and only the views of Chris and Forest, and should not be percieved to reflect the views of any other hosts or guests of the podcast. With that being said, please enjoy our bonus episodes. In this first bonus episode we examine the myths and mystique surrounding Takeda Shingen and the battles of Kawanakajima. The 4th battle of Kawanakajima is traditionally believed to be a victory for the Takeda clan, but the events that followed all point to a decisive victory by the Uesugi. As for Takeda Shingen himself, he is considered a hero of the Sengoku, loved by fanboys at home and abroad - but was he really the magnanimous Daimyo he's made out to be, or the 16th century version of a colossal douchebag? In this episode, we look at the Takeda clan during the 1560s. After Uesugi Kenshin effectively stopped Takeda Shingen in his tracks at the 4th battle of Kawanakajima in 1561, Shingen was forced to re-evaluate his goals in the region, which led to the 1560’s being a time of great change in the Takeda clan.
