Guest Heather McNemar experienced a taste of world travel while in high school, when her father took a sabbatical and the family visited many spots, including Siberia for a slightly extended stay. Heather was drawn to the language and the culture and knew she would study both in college. Later, as she was making her way into the world, she didn't know exactly how she would use her skills but knew she wanted to help people. She got a job helping newly settled immigrants to the U.S. get jobs and realized that good language skills were key to their success. She began taking classes to teach English as a Second Language and landed a spot as a Peace Corps volunteer in Kazakhstan. Jon McNemar is a Sports Performance Coach at Duke University. He joined the Blue Devils in July of 2021 and works directly with the field hockey, track & field sprints-jumps groups, as well as men's tennis team. McNemar previously worked at Jacksonville University where in 2015 he worked his way from intern to graduate assistant before being promoted to a full-time assistant strength coach in 2018. He also spent part of the 2018 season as an intern with the Florida State University football program. McNemar was an offensive lineman and four-year letter winner on the Fairmont State University football team. He was awarded the “Iron Falcon Award” which is given to the strongest and most dedicated offensive/defensive lineman in the weight room. Abstract: Motivation: Microbes perform a fundamental economic, social and environmental role in our society. Metagenomics makes it possible to investigate microbes in their natural environments (the complex communities) and their interactions. The way they act is usually estimated by looking at the functions they play in those environments and their responsibility is measured by their genes. The advances of next-generation sequencing technology have facilitated metagenomics research however it also create a heavy computational burden. Large and complex biological datasets are available as never before. There are many gene predictors available which can aid gene annotation process though they lack of handling appropriately metagenomic data complexities. There is no standard metagenomic benchmark data for gene prediction. Thus, gene predictors may inflate their results by obfuscating low false discovery rates. Results: We introduce geneRFinder, a ML-based gene predictor able to outperform state-of-the-art gene prediction tools across this benchmark by using only one pre-trained Random Forest model. Average prediction rates of geneRFinder differed in percentage terms by 54% and 64%, respectively, against Prodigal and FragGeneScan while handling high complexity metagenomes. The specificity rate of geneRFinder had the largest distance against FragGeneScan, 79 percentage points, and 66 more than Prodigal. According to McNemar's test, all percentual differences between predictors performances are statistically significant for all datasets with a 99% confidence interval. Conclusions: We provide geneRFinder, a approach for gene prediction in distinct metagenomic complexities, available at github.com/railorena/geneRFinder, and also we provide a novel, comprehensive benchmark data for gene prediction --- which is based on The Critical Assessment of Metagenome Interpretation (CAMI) challenge, and contains labeled data from gene regions -- avaliable at sourceforge.net/p/generfinder-benchmark . Acil tıp hekimlerinin gerek servislerinde gerekse de servis/hastane dışı vakalarındaki en önemli uğraş alanı; hava yolunu açmak ve güvenli bir şekilde devamlılığını sağlamaktan geçer.  En çok kullandığımız ve en iddialı olduğunu düşündüğümüz endotrakeal entübasyon yöntemi bazı durumlarda çok da kolay değildir. Toplumda sık kullanılan “ağzının içine bakmak” deyiminin acilci için anlamı; “bu hasta kolay mı entübe olur yoksa zor mu?” desek çok mu abartmış olurum.   [box type="info" align="aligncenter" class="" width=""]ZOR HAVAYOLU deneyimli bir doktorun, hastasını maske ile havalandırmada veya trakeal entübasyonda ya da her ikisinde birden karşılaştığı zorluk olarak tanımlanmaktadır. Rakam arttıkça zorluk da doğru orantılı olarak artıyor.   11 ay boyunca, genel anestezi alacak olan, 16 yaş ve üzeri ardışık hastalar çalışmaya dahil edilmiş. Dişi olmayan hastalar, ağız açıklığını sağlayamayanlar, laringeal kitlesi olanlar ve boyun hareketi kısıtlı olan hastalar çalışmadan çıkartılmış.  Entübasyon sürecinde yer almayan iki anestezist, hastaları ÜDİT ve Modifiye Mallampati Testine (MMT) göre değerlendirmiş. Hastaların preoperatif değerlendirilmesinde yer almayan diğer anestezistler ise, entübasyon esnasında entübasyon zorluğunu Cormack-Lehane sınıflamasına göre değerlendirmiş. Entübasyon öncesi ve entübasyon esnasındaki değerlendirmeler karşılaştırılarak söz konusu skorların doğruluğu açısından analiz edilmesi hedeflenmiş.  300 hasta çalışmaya dahil edilmiş. 17'si (%5.7) entübasyon esnasında zor havayolu olarak değerlendirilmiş. MMT sınıf III veya IV olanlar ve UDIT sınıf III olanlar zor entübasyon olarak kabul edilmiş. 300 hastadan MMT sınıf III-IV olan hasta sayısı 108 iken, ÜDIT sınıf III olan hasta sayısı 45 olarak tespit edilmiş. Bu iki sınıflamanın McNemar testiyle özgüllükleri (%95 GA; 66.8 [60.9-72.2]'ye 88.7 [84.3-92])ve doğrulukları (%95 GA; 67.7 [62-72.9]'a 88 [79.9-93.6]) arasında ÜDİT lehine istatistiksel olarak anlamlı fark saptanmış (p0.05).

Background: To determine if black-blood 3 T cardiovascular magnetic resonance (bb-CMR) can depict differences between symptomatic and asymptomatic carotid atherosclerotic plaques in acute ischemic stroke patients. Methods: In this prospective monocentric observational study 34 patients (24 males; 70 +/- 9.3 years) with symptomatic carotid disease defined as ischemic brain lesions in one internal carotid artery territory on diffusion weighted images underwent a carotid bb-CMR at 3 T with fat-saturated pre- and post-contrast T1w-, PDw-, T2w- and TOF images using surface coils and Parallel Imaging techniques (PAT factor = 2) within 10 days after symptom onset. All patients underwent extensive clinical workup (lab, brain MR, duplex sonography, 24-hour ECG, transesophageal echocardiography) to exclude other causes of ischemic stroke. Prevalence of American Heart Association lesion type VI (AHA-LT6), status of the fibrous cap, presence of hemorrhage/thrombus and area measurements of calcification, necrotic core and hemorrhage were determined in both carotid arteries in consensus by two reviewers who were blinded to clinical information. Background: Retention of mouth-to-mouth, mouth-to-mask and mouth-to-face shield ventilation techniques is poorly understood.Methods: A prospective randomised clinical trial was undertaken in January 2004 in 70 candidates randomly assigned to training in mouth-to-mouth, mouth-to-mask or mouth-to-face shield ventilation. Each candidate was trained for 10 min, after which tidal volume, respiratory rate, minute volume, peak airway pressure and the presence or absence of stomach inflation were measured. 58 subjects were reassessed 1 year later and study parameters were recorded again. Data were analysed with ANOVA, textgreekq2 and McNemar tests.Results: Tidal volume, minute volume, peak airway pressure, ventilation rate and stomach inflation rate increased significantly at reassessment with all ventilation techniques compared with the initial assessment. We initially performed a mutation screen of the coding region of the MC4R in 808 extremely obese children and adolescents and 327 underweight or normal-weight controls allowing for a case-control study. A total of 16 different missense, nonsense, and frameshift mutations were found in the obese study group; five of these have not been observed previously. In vitro assays revealed that nine [the haplotype (Y35X; D37V) was counted as one mutation] of the 16 mutations led to impaired cAMP responses, compared with wild-type receptor constructs. In contrast, only one novel missense mutation was detected in the controls, which did not alter receptor function. The association test based on functionally relevant mutations was positive (P = 0.006, Fisher's exact test, one-sided). We proceeded by screening a total of 1040 parents of 520 of the aforementioned obese young index patients to perform transmission disequilibrium tests. The 11 parental carriers of functionally relevant mutations transmitted the mutation in 81.8% (P = 0.033; exact one-sided McNemar test). These results support the hypothesis that these MC4R mutations represent major gene effects for obesity.

Die Qualität der Gefäßdarstellung war bei den Arterien in der Angiographie besser als in der Computertomogaphie und dies um so mehr, je kleiner die Gefäße waren.
