Julia wurde mit einer seltenen Krankheit diagnostiziert: Sie ist eine chronische Oberzicke. Seit Wochen leidet sich unter Schlafmangel und so langsam lässt sie ihren Unmut gezielt an ihren Mitmenschen aus. In dieser Folge werden wir erfahren, wo diese Furie in kürzester Zeit so unterwegs war und wie unglaublich vielen Menschen sie mit ihrem Zicken-Dasein das Leben zur Hölle gemacht hat. The Marie Keating Foundation has launched a new awareness campaign urging women, especially those outside the screening age, to check their breasts regularly for any changes. Speaking to Ciara this morning was Professor Seamus O'Reilly Consultant Medical Oncologist in Cork University Hospital.

Ich spreche darüber, • warum Yoga heilsam wirken kann, ohne Therapie zu sein, • wie wir als Lehrende begleiten dürfen, ohne zu behandeln, • weshalb Präsenz manchmal wirksamer ist als Analyse, • und warum Heilung dort beginnt, wo wir aufhören, etwas verändern zu wollen. Für mich ist Yoga kein Werkzeug, um etwas zu reparieren, sondern ein Weg, sich zu erinnern – daran, dass in uns schon alles da ist. Wenn dich diese Frage auch beschäftigt – hör gern rein.

Photo: Andrew Downes Researchers at the University of Galway have revealed the results of a world-first study into how bowel cancer shuts down the immune system, and how this can be reversed to improve treatment. The findings have been published in the Journal for ImmunoTherapy of Cancer (JITC). Breakthrough for bowel cancer immunotherapy The research team showed how structural stromal cells switch off the immune system and how the body's own killer cells can be switched back on, opening up the opportunity for a completely new approach to optimising immunotherapy for patients. Colorectal cancer - commonly referred to as bowel cancer - is one of the world's most common cancers and the second leading cause of cancer death worldwide. Diagnoses in people under 50 have been climbing in recent decades, with recent global analysis showing these early-onset cases have almost doubled since 1990. In Ireland, the disease affects more than 2,500 people a year. Professor Aideen Ryan, Professor in Tumour Immunology at University of Galway's College of Medicine, Nursing and Health Sciences, said: "While immunotherapy has revolutionised care in cancers such as melanoma and lung cancer, it has shown very limited benefit in bowel cancer, leaving patients with advanced disease with few treatment options and poor survival outcomes." The breakthrough research discovery is the first to demonstrate that tumour stromal cells - the structural cells that support cancer growth - are directly reprogramming the immune system. They do this by switching off, or hijacking, two of our body's most important tumour-fighting cells - the natural killers (NK) and macrophages - rendering them unable to attack the cancer. Professor Ryan said: "The interaction between the cancer, our body's healthy cells and our defence mechanism is a complex one, but our research shows that the cancer is essentially creating an immune brake - it is blocking the body's natural response and fight mechanism." What has been discovered in relation to how the cancer interacts with the human body? Tumour stromal cells are the structural cells which allow the cancer to grow. They are coated in sugars called sialoglycans. These interact with receptors on the body's immune cells called Siglecs. This interaction causes the body's natural defence response to be switched off and unresponsive when immunotherapy is used, and therefore unable to attack the cancer. The research identified a specific enzyme that drives this process of 'switching off', as it produces the Siglec-binding sugars on stromal cells. When the researchers blocked this pathway using drugs called sialidases, they could show that the body's most important tumour-fighting cells - the natural killers (NK) and macrophages - reactivated. It showed that the tumours shrank and the spread of cancer, known as metastasis, was prevented. The multidisciplinary research was conducted in collaboration with colorectal surgeons and pathologists at Galway University Hospital, led by Professor Aisling Hogan and Professor Sean Hynes; as well as experts in colorectal cancer - Dr Philip Dunne, Queen's University Belfast and experts in targeting sialoglycans at Palleon Pharmaceuticals, MA, USA, who have developed sialidase drugs that disrupt the sialoglycan-Siglec interaction. Professor Ryan added: "Our research is a clear breakthrough in our understanding of bowel cancer and how immunotherapy could be more successful. This world-first finding shows that some of the bowel cancer cells are not just passive bystanders, they are actively reprogramming the body's immune cells, preventing them from doing their job. We have uncovered an entirely new checkpoint and by focusing on it we can reactivate the immune system and improve our body's innate ability to fight the disease, and even target metastasis." We speak with Alison Singer, the President of the Autism Science Foundation and the Autism Science Foundation of Canada. She says having one diagnosis hurts people like her daughter who is non-verbal and needs a lot of support — and that there should be a diagnosis of profound autism to help unlock research and support.

We are at an all time high of mental illness diagnoses. Some argue the reason is better diagnostic methods, others because we are just increasing in mental un-health. The discussion in this episode is in regards to the prevalence of mental diagnoses and the impact on individuals. In  Brene Brown's bestselling book, Atlas of the Heart, she outlines 87 different emotions that are in and of themselves, normal and healthy. But when do we go over the line from normal to abnormal? From feeling some depression to being clinically depressed? Having anxiety or an anxiety disorder? Do we need or want an official diagnosis so we can get needed help? Or do we want to deny and boycott any such limiting label? I've brought on an expert to discuss it. Sarah Fay is an author, educator, and activist on the issue and has a book titled, Pathological: The True Story of Six Misdiagnoses. Sarah from a young age displayed some unhealthy behaviors and was diagnosed, labeled, and treated. Over the next 30 years she went on to have multiple clinical diagnoses and labels for Anorexia, Major depressive disorder, Anxiety disorder, Attention deficit hyperactivity disorder, Obsessive compulsive disorder, Bipolar disorder, and has also dealt with alcoholism, being a compulsive exerciser, and many bouts being suicidal. You name the drug, she's been prescribed it and taken it. Today…she is well, though still accepting of a couple diagnoses and medications, but has much to educate and warn us about regarding our current culture which has nearly half of all Americans being given an official, clinical diagnosis during their lifetime. “Chemotherapy-induced alopecia does cause a lot of stress. It's associated with lower quality of life. Scalp cooling may really help improve quality of life. Some studies have shown that women in the scalp cooling group felt less upset about losing their hair and less dissatisfied with their appearance compared to the women in the control group that didn't receive any scalp cooling. So a lot of these studies are showing it does have a very positive impact on psychosocial feelings and side effects in relation to overall cancer treatment,” ONS member Jaclyn Andronico, MSN, CNS, OCN®, AOCNS®, clinical nurse specialist at Memorial Sloan Kettering Cancer Center in New York, NY, told Jaime Weimer, MSN, RN, AGCNS-BS, AOCNS®, manager of oncology nursing practice at ONS, during a conversation about chemotherapy-induced alopecia and scalp cooling. Highlights From This Episode “Some of the classes that are high risk for chemotherapy-induced alopecia can include antitumor antibiotics, such as doxorubicin, epirubicin; antimicrotubule drugs, such as taxanes like docetaxel, paclitaxel; alkylating agents such as cyclophosphamide. The lower risk alopecia-causing chemo agents are the antimetabolite classes, which a lot of people know as gemcitabine or fluorouracil. We also are seeing patients experience some degree of alopecia with a drug called sacituzumab govitecan, which is an actual antibody–drug conjugate.” TS 2:09 “Scalp cooling is approved for solid tumor patients. Patients receiving chemotherapy agents, as we discussed before, with that high incidence of chemotherapy-induced alopecia really should be considered for scalp cooling as long as they don't have certain contraindications. Some contraindications do exist for these populations. Those are patients that have cold agglutinin diseases, cryoglobulinemia, cryofibrinogenemia, and any cold sensitivity issues. Patients also with abnormal liver functions are not suggested to receive scalp cooling because their liver function is associated with the metabolism of the drug agent. It's also not recommended for patients with hematologic malignancies who are higher risk for cutaneous metastatic disease or failed chemotherapy and even reduced survival rates.” TS 9:23 “Overall, scalp cooling has a good tolerance, but it's important to be aware that scalp cooling can be uncomfortable for some, and it isn't always tolerated by some patients. Patients have reported side effects such as headaches, dizziness, chills, cold sensations, scalp pain, head discomfort, and even claustrophobia. Among these, the most common is the cold feeling and headaches. So when caring for patients that undergo scalp cooling, the nurses really should recognize the patient's feelings and help relieve that discomfort with position changes, prophylactic painkillers such as [acetaminophen] or [ibuprofen] if they're allowed to take that, additional warm blankets. Even antianxiety medications can really help, especially if that claustrophobia feeling is there. So collaborating with that team, the doctors, the nurse practitioners, just to be aware that if the patient's not comfortable during this treatment of scalp cooling, they should have those things on board prior to starting.” TS 12:23 “A lot of primary education points for nurses to review with patients is explaining the financial reimbursement process and assistance options—collaborating with that financial department within your institution. Also discussing the efficacy of the scalp cooling and the tolerability of it as well. I don't know if patients are always aware of the feeling of the cold—really warning them, letting them know what they're going to experience while in the chair attached to that cooling machine.” TS 18:55 “Nurses, we're at the forefront of scalp cooling, and we manage a lot of this area that comes with the service. They are managing, the coordination of the care, the education. They're assisting with that financial discussion and collaborating with the financial department. The symptom management—they're helping make the patients feel more comfortable with the symptoms of scalp cooling. They are the real deal here, so they are the best in helping with the situation. Nurses should be really familiar with the efficacy, again, and the tolerability, the contraindications, the side effects, and the costs and even the access for scalp cooling.” TS 20:57
