Rincón de la Victoria celebra una nueva edición de los Paseos Primaverales, organizado por el Área de Sostenibilidad Medioambiental a través de Greencón, junto al Club de Montaña Rincón de la Victoria, con cuatro rutas, la primera de ellas, el próximo 12 de abril. El concejal de Sostenibilidad Medioambiental, Borja Ortiz (PP), ha destacado el éxito de participación de la iniciativa celebrada el pasado año con más de 200 personas de todas las edades. Se trata, añade el edil, “de concienciar sobre la conservación del medio ambiente, fomentar la práctica del senderismo responsable, y dar a conocer nuestro amplio y variado ecosistema”. El alcalde de Rincón de la Victoria, Francisco Salado (PP), ha valorado la importancia de la iniciativa “que promociona la vida saludable disfrutando de una forma atractiva y didáctica de nuestros paisajes naturales para sensibilizarnos sobre la importancia de conservar y proteger nuestro entorno natural y patrimonial”. En este sentido, el presidente del CMR, José Carlos Cañas, ha explicado que “serán paseos guiados para unas 50 personas por salida programada, de una duración aproximada de unas cinco horas, con itinerarios y localizaciones cargadas de belleza e historia”. Todas las personas interesadas en participar de esta actividad podrán inscribirse de forma gratuita en la sede del CMR en Calle Ronda, 44 (local 1D), en el siguiente horario: de 09.00 a 14.00 y de 16.00 a 20.00 horas de lunes a viernes. La Concejalía de Sostenibilidad Medioambiental en colaboración con la Asociación CMR ha organizado una programación de talleres medioambientales que se desarrollarán durante este año en el municipio. Se trata del taller de orquídeas, que se celebrará el próximo 6 de abril, el taller de kokedamas previsto para el 14 de junio, así como el taller de plantas culinarias y medicinales que se llevará a cabo en el mes de octubre. El edil de Área, Borja Ortiz (PP), ha explicado que, “se trata de acciones formativas muy esperadas y de gran aceptación en el municipio, y que tienen como finalidad difundir valores medioambientales a través de las actividades teóricas y prácticas impartidas por profesionales”. El alcalde de Rincón de la Victoria, Francisco Salado (PP), ha destacado “la importancia de estas iniciativas de sensibilización, educación medioambiental y concienciación en economía verde abiertas a la participación, que suponen una oportunidad única para conocer de cerca las especies autóctonas, sus secretos ecológicos y las claves para su conservación y cultivo”. El primer taller `Las orquídeas de Andalucía: conocimientos y cuidados´ se celebrará el domingo 6 de abril y será impartido por el curator internacional y una de las voces más autorizadas en el mundo de las orquídeas, Manuel Lucas. El presidente de la Asociación CMR, José Carlos Cañas, ha indicado que “en este taller los participantes aprenderán los mejores trucos para mejorar el estado de salud de la orquídea; cómo realizar mezclas de sustratos, limpieza de raíces y trasplantes, qué entorno es el más adecuado para que prosperen o trucos para inducir la floración”. El segundo taller será sobre Kokedamas, previsto para el 14 junio. “Se trata del cultivo de planta sin necesidad de macetas tradicionales para fomentar el reciclaje de materiales naturales como el musgo y la tierra de jardín”, indica el concejal. Por último, se celebrará en el mes de octubre el taller de plantas culinarias y medicinales, que abordará el beneficio de incorporar las plantas frescas a una alimentación saludable. Los talleres están dirigidos a personas adultas para un máximo de 20 plazas por acción formativa. The Heart Rhythm Society criteria for diagnosis of cardiac sarcoidosis require a biopsy-proven diagnosis of extra-cardiac sarcoidosis. If extra-cardiac sarcoidosis is confirmed, CS is highly probable when there is evidence of the following: (a) rhythm abnormalities in either ECG or Holter monitoring such as advanced AVB (Mobitz type II second-degree or third-degree AVB), sustained VT, and (b) LV dysfunction on echocardiography or CMR (LVEF

On January 25th, Pete Hegseth was sworn in as the 29th Secretary of Defense. He promised to eradicate wokeness from the military and has as his mission to revive the warrior ethos and restore trust in the military and to rebuild it by matching threats to capabilities and reestablish deterrence by defending the homeland. Returning to Crosstalk to discuss this issue was Elaine Donnelly. Elaine is president of the Center for Military Readiness, an independent, nonpartisan public policy organization that reports on and analyzes military/social issues. Since its founding in 1993, CMR has advocated for high, uncompromised standards in all forms of military training and sound priorities in the making of military/social policies.Elaine began by noting that the Biden administration made it clear from day one that they were going to pursue a very liberal social agenda/experiment in the military. They would take the transgender agenda to extremes that weren't even seen under President Obama.This brought the word "woke" to our vocabularies. She defined this as taking progressivism to extremes and posing it with coercion, even if it hurts the institution.Elaine brought her opinions concerning the Pete Hegseth confirmation hearings, how he's been doing on the job thus far, military readiness (including women in the military), Christian and pro-life groups targeted by the Army, access to the military by those with gender dysphoria, and much more including calls from listeners.

Pearls and Quotes - Cardiomyopathies The presence of an S4 and a rapid y-descent in the jugular venous pulsation on exam should clue you to the presence of a restrictive filling pattern. Restrictive filling doesn't necessarily mean restrictive cardiomyopathy and is more commonly due to dilated or ischemic cardiomyopathy. The five main topics of counseling that every hypertrophic cardiomyopathy (HCM) patient should understand: (1) Prognosis, (2) Family Screening, (3) Risk of Sudden Death, (4) Treatments, and (5) Physical Activity. Remember 1/3: In clinical trials of cardiac myosin inhibitors for HCM (mavacamten), about a third of patients had a tremendous improvement in symptoms, another third had some improvement, and the final third had no improvement or had to discontinue the drug due to negative inotropy. When counseling patients about septal reduction therapy, consider the patient's age. For younger patients, surgical myectomy at an experienced center offers a higher success rate and greater durability with lower rates of pacemaker placement when compared to alcohol septal ablation. Historically, the conclusion that it was higher risk to be an athlete with HCM was unfortunately generalized to mean that it was high risk to exercise for patients with HCM. “And we turned a generation of HCM patients into HCM cardiometabolic syndrome patients, which is actually a worse combination.” Notes - Cardiomyopathies What is the initial approach to evaluating a patient with new or suspected cardiomyopathy, including hypertrophic cardiomyopathy (HCM)? A history and physical exam, including a thorough past medical and family history, is always the first step and helps determine the patient's risk for potential underlying etiologies, including genetic cardiomyopathies, hypertrophic cardiomyopathy, or those related to treatments of previous cancer. In terms of ECG findings, pay attention to QRS voltage (high or low) and the presence of any arrhythmias. TTE should be obtained in all patients and is often sufficient to diagnose many underlying cardiomyopathies, including HCM. Cardiac MRI (CMR) is helpful as an adjunct when TTE alone is inconclusive or imaging quality is poor. CMR can help provide a better idea of chamber sizes and wall thickness, and late gadolinium contrast enhancement (LGE) can also be helpful if present in a specific pattern, though often HCM patients may have non-specific patterns of LGE. Invasive hemodynamics assessment is reserved for patients with discordance between non-invasive testing and the clinical impression. Comparison of FDG-PET/CT and CT for Treatment Evaluation of Patients With Unresectable Mesothelioma

BUFFALO, NY- July 15, 2024 – A new #research paper was #published in Oncotarget's Volume 15 on June 20, 2024, entitled, “Comparison of FDG-PET/CT and CT for evaluation of tumor response to nivolumab plus ipilimumab combination therapy and prognosis prediction in patients with unresectable malignant pleural mesothelioma.” Malignant pleural mesothelioma (MPM) is an aggressive neoplasm and affected patients have low survival rates. In this new retrospective study, researchers Kazuhiro Kitajima, Kozo Kuribayashi, Toshiyuki Minami, Hiroyuki Yokoyama, Akifumi Nakamura, Masaki Hashimoto, Takashi Kijima, Seiki Hasegawa, Hayato Kaida, and Koichiro Yamakado from Hyogo Medical University and Kindai University Faculty of Medicine examined the effectiveness of fluorodeoxyglucose positron emission tomography (FDG-PET) criteria, i.e., immunotherapy-modified PET response criteria in solid tumors (imPERCIST), with morphological computed tomography (CT) criteria, i.e., modified response evaluation criteria in solid tumors (mRECIST), to evaluate patients with unresectable MPM undergoing nivolumab plus ipilimumab combination therapy as first-line treatment regarding response and prognosis prediction. “Results for malignant pleural mesothelioma (MPM) patients following first-line treatment with nivolumab plus ipilimumab obtained with immunotherapy-modified PERCIST (imPERCIST), shown by [18F] (FDG-PET/CT), and modified RECIST (mRECIST), shown by CT, were compared for response evaluation and prognosis prediction.” Twenty-six patients (23 males, 3 females; median 73.5 years) with histologically proven MPM and no curative surgery received nivolumab plus ipilimumab combination therapy. FDG-PET/CT and diagnostic CT scanning at the baseline, and after 2–4 cycles (2 in three, 3 in 17, 4 in six patients) were performed. Therapeutic response findings evaluated using imPERCIST and mRECIST were compared. PFS and OS analyses were done using log-rank and Cox methods. Results: imPERCIST indicated nine progressive metabolic disease (PMD), eight stable metabolic disease (SMD), four partial metabolic response (PMR), and five complete metabolic response (CMR) cases. mRECIST showed nine with progressive disease (PD), nine stable disease (SD), seven partial response (PR), and one complete response (CR). Although high concordance was noted (κ = 0.827), imPERCIST correctly judged a greater percentage with CMR (15.4%). Following a median 10.0 months, 15 patients showed progression and eight died from MPM. A 37-year-old Caucasian man with a history of tobacco smoking and hypertension who presented with chest pain and elevated troponin was admitted for non-ST elevation myocardial infarction (NSTEMI). Ischemic evaluation with an invasive coronary angiogram was negative. He was treated as NSTEMI and scheduled for outpatient cardiac MRI (CMR). The patient came back 2 months later with right arm weakness and confusion and was found to have an embolic stroke. Labs showed positive troponin with a flat trend and hypereosinophilia. Transthoracic echocardiogram (TTE) showed obliteration of LV and RV apex with thrombus and reduced LV systolic function. CMR was consistent with myocarditis with biventricular thrombus. The patient was started on corticosteroids and warfarin. Hypereosinophilia workup was positive for PDGFRA alpha rearrangement. He was diagnosed with primary hypereosinophila syndrome. Imatinib was initiated. The patient was followed up with the hematology clinic, achieved a complete hematologic response with normalized cell count, and remained free from any cardiovascular event at the 8-month follow-up. Case Media Pearls - Apical Obliteration with Biventricular Thrombus Cardiac MRI is a valuable test for patients presenting with myocardial infarction with non-obstructive coronary arteries (MINOCA). Obliterated apex with apical thrombus on TTE with hypereosinophilia should raise high suspicion for eosinophilic myocarditis. Initiation of corticosteroids is the first-line treatment for eosinophilic myocarditis, which is associated with lower mortality in patients with myocarditis. For other potential complications, such as heart failure, intracardiac thrombus, arrhythmia, and pericardial effusion, the standard of care for each disorder is recommended. Hypereosinophilia can be seen in parasitic infections, vasculitis, asthma, allergy, hematological malignancies, and as a primary disorder. Show Notes - Apical Obliteration with Biventricular Thrombus What is the differential diagnosis for patients with elevated troponin and nonobstructive CAD? The occurrence of acute myocardial infarction (AMI) without significant CAD was reported 80 years ago. However, the term MINOCA (myocardial infarction with non-obstructive coronary arteries) has only been used recently to describe these patients. It involves ischemic and nonischemic etiologies. First, overlooked ischemic etiologies need to be ruled out by reconciling the angiogram images such as spontaneous coronary artery dissection (SCAD) and plaque disruption. Intracoronary imaging, such as intravascular ultrasound (IVUS) or optical coherence tomography (OCT), may be applied to evaluate for SCAD and subtypes of plaque disruption when indicated.  The investigation continues with nonischemic causes such as stress cardiomyopathy, myocarditis, pulmonary embolism, demand ischemia from sepsis, anemia, chest trauma, heart failure exacerbation, arrhythmia, and stroke. The diagnosis of MINOCA is established when it fulfills the following criteria: First, it is AMI by the Fourth Universal Definition; Second, less than 50% of stenotic lesion on angiogram; Third, there is no alternate diagnosis.

Pearls and Quotes - Cardiac Tumors Keep it simple when approaching an intracardiac mass; start with transthoracic echocardiography (TTE) and use transesophageal echocardiography (TEE) or cardiac magnetic resonance (CMR) based on the clinical context. Use TEE when suspecting valvular vegetations or thrombi & CMR for intracavitary cardiac masses. Cardiac tumors can manifest with a variety of symptoms; however, they are more commonly diagnosed as an incidental finding! When faced with the dilemma of selecting the most suitable imaging modality for evaluating a cardiac mass, consider the following hierarchy: begin with TTEas the first choice, followed by CMR. If the patient cannot undergo CMR, the next step is cardiac computed tomography (CT) or Fluorodeoxyglucose F18 positron emission tomography (FDG-PET). TEE is especially useful for the evaluation of small, highly mobile cardiac masses! Imaging cannot substitute a tissue diagnosis of cardiac masses. However, in cases of advanced malignancy, it may not always be necessary. Show notes - Cardiac Tumors Segment One: A big “picture” Approach to Cardiac Tumors Let's start with an overview of cardiac masses Neoplastic vs non-neoplasticNeoplastic lesions can be further classified into Primary Cardiac Tumors (PCT's) & Secondary Cardiac Tumor (SCT's)A majority of PCTs are benign (up to 90%!); however, rarely, they may be malignant. SCTs are more common than PCTs, and, by definition, they are malignant tumors. Now, let's look at the tools you can use to aid with the diagnosis of cardiac masses… Step 1: Investigate the cardiac mass initially with TTE. Step 2: Collect clues through history-taking & examination.If suspecting valvular vegetations (as in infective endocarditis!) or left atrial appendage thrombus, characterize the mass further with TEE.Consider the possibility of metastatic cardiac tumors in patients with a known malignancy, as they are more common than primary cardiac tumors. In cases where it is uncertain if the mass is a cardiac tumor or thrombus, use CMR to differentiate the two entities. Some findings on TTE that support the presence of a thrombus include left ventricular dysfunction with segmental wall motion abnormalities and/or apical aneurysm as these result in local pockets of stasis (think: Virchow's triad) Step 3: Put it all together! Think about whether a tissue biopsy will be needed. If yes, determine whether a negative margin or open biopsy will be required. Segment Two: Symptoms, Symptoms, Symptoms!
