The family reached out to Erin to see what they could do to help their baby get back on track. Erin makes a few tweaks to the routines and recommends using the Hatch clock - and gives her best tips on how to use it - for the family to try moving forward to help their little one start to identify some new sleep cues. Removing bottles/feeding from the sleep routines has been recommended too. They go over the steps for the 2-1 nap transition but are hoping the initial tweaks to the routine fix things before considering this as an option. We all tend to prepare for the questions that we will get asked during an interview.  But, you need to be equally prepared with questions to ask the hospital. 1.) Typical Medical Work-upsWhen to ask: anytime, can be a good ice breaker Who to ask: Medical Director/Owner, Associates, Techs How to ask: Examples of common cases:  vomiting/diarrhea dog, PU/PD cat, wellness visits Vomiting/diarrhea dog What you will learn from the response: Insight on the level of medicine, Is this a good fit for you? , What the average client are expecting 2.) Average Production for Veterinarians/Average Client Transaction When to ask: middle or end of interview, follow up after the interview (during offer) Hospital may need time to get the answer Who to ask: Hospital Manager/Administrator or Medical Director/Owner How to ask: Can you share what your next newest doctor is making in production per year?   For Average Client Transaction, need to take it into consideration with the schedule High-volume vs. low-volume What you will learn from the response: What to expect for earning potential (production) How open the hospital is to share 3.) Kidney disease is more common as dogs age. It is estimated that more than 1 in 10 dogs will develop kidney disease over a lifetime1,3, so it’s an important topic to understand. When healthy, the two kidneys efficiently: Filter the blood Process protein wastes and excrete them into the urine Conserve and balance body water, salts and acids Help to maintain normal red blood cells Kidney disease occurs when one or more of these functions are compromised or reduced. Unfortunately, it typically goes undetected until the organs are functioning at approximately 33% to 25% of their capacity. Dogs with moderate to severe chronic kidney disease (CKD) are prone to dehydration and you may notice that your dog is lethargic and has a poor appetite. Treatment options for advanced kidney disease are usually limited to treating the symptoms because dialysis and kidney transplants are not readily available for dogs. Protecting your pooch from kidney disease means you should be prepared to look for problems early. Causes of of chronic kidney disease in dogs The International Renal Interest Society or IRIS is a group of veterinary specialists studying  kidney disease in dogs and cats. They list several risk factors that make pets more susceptible to kidney disease, such as age or breed, and investigate reversible factors that initiate or accelerate kidney damage. Such factors include: Glomerulonephritis Pyelonephritis (kidney infection) Nephrolithiasis (kidney stones) Ureteral obstruction & hydronephrosis (stones causing a blockage) Tubulointerstitial disease (involving the kidney tubules) Leptospirosis Cancer Amyloidosis (protein problem) Hereditary nephropathies (genetic problem) Signs of chronic kidney disease in dogs One of the earliest signs of kidney disease in dogs is urinating and drinking more (polyuria/polydipsia or PU/PD). Often, dogs need to urinate at nighttime (nocturia) or have "accidents." There are many other causes of PU/PD, but kidney disease is one of the most serious concerns. Taking water away from your dog could make chronic kidney disease worse, so please don’t try to do this without your veterinarian’s direction. Having your dog examined promptly when you note a change in water intake and urine production is key! Dogs tend to be pretty sensitive to changes in their blood levels of waste so even mild to moderate changes may cause signs of illness. Other signs of chronic kidney disease include: Urinary incontinence (urine leakage) Dehydration Lethargy Reduced appetite Vomiting Weight loss Bad breath with a chemical odor Oral ulcers Pale appearance Testing your dog for chronic kidney disease Your veterinarian will often suggest these basic tests: Physical Examination— The first test your veterinarian is likely to perform will be a physical examination, and remember that taking your dog in for routine physical examinations is an excellent way to help protect him. Even normal physical exams may be useful later as a baseline of comparison. During the exam, your veterinarian might detect: Enlarged, painful kidneys Back or flank pain Changes in the prostate or urinary bladder A rectal examination can give more detail about the possibility of urethral, bladder or prostatic disease that might relate to kidney disease. Finding a large bladder in a dehydrated pet, or other, less specific, findings may also warn your veterinarian about a kidney problem, for example: Low body temperature Excessive skin tenting Dry gums (showing dehydration) Unkempt hair coat Other testing may include: Complete blood count (CBC)—The CBC may reveal anemia (too few red blood cells) or an elevation in white blood cells consistent with infection, stress or inflammation. Less commonly, blood parasites or circulating cancer cells are found. Chemistry profile with electrolytes Urinalysis with sediment exam—Examining a urine specimen, especially prior to any treatment, is a simple and economical way to gauge urine quality.   Urine culture with susceptibility Urine protein to creatinine ratio Infectious disease testing—Regular testing for heartworm infection and for vector borne diseases such as Ehrlichiosis andLyme can identify risk factors for kidney disease. Diagnostic imaging—Using radiography (X-rays) and diagnostic ultrasound to identify changes in the size, shape and architecture of the kidneys may suggest a specific blockage that needs emergency treatment. Blood pressure measurement—High blood pressure is common in dogs with chronic kidney disease. Kidney Sampling Management and monitoring of chronic kidney disease Sick pets with signs of kidney disease that include dehydration will likely require more intensive care in a hospital setting, while those that are happy tail waggers and are completely self-supporting will often be treated at home. Your veterinarian will work diligently to find a treatable cause of kidney disease and make individual recommendations for your dog.   General treatment goals for dogs with kidney disease include: Providing adequate and appropriate nutrition with a kidney friendly diet (always check with your veterinarian before making any diet changes). Ensuring excellent hydration. Balancing salts and acid-base levels. Aggressively treating any protein problems or high blood pressure, since these conditions tend to worsen kidney damage2. Keeping plenty of fresh water available—this is essential! Keep in mind that dogs with kidney disease are usually going to need more trips outside for bathroom breaks, so building this into the household schedule will be important. A friendly neighbor can help out, or a professional dog walker can come to give your pooch a midday break and help you avoid coming home to a puddle or two on the floor. He really can’t help it! Your pet’s condition, any other illnesses found and your personal goals for treatment will help determine the course of treatment and dictate how often you visit the veterinarian. Chronic kidney disease prognosis/advances Some dogs with chronic kidney disease will live years after diagnosis and have a good quality of life. Others will not be so lucky. Your veterinarian will evaluate after testing is finished. As a motivated pet guardian you can take an active role in your pet’s preventive care by planning for wellness exams and routine lab testing. Team up with your veterinarian to address any problems before they get worse. The discovery of new biomarkers for kidney function, such as SDMA, provides for early recognition of kidney disease before other blood values change and offers the prospect of earlier intervention and kidney care3. Ældre patienter med flere symptomer Middel-alderen er 9-11 år på diagnose tidspunktet, så det er ældre patienter, der kan have andre sygdomme også. Anders oplever at med netop denne type patienter, kan man nemt komme til at ”blæse ejeren bagover”, med alt for mange oplysninger fra starten. Det kan altså være en fordel, hvis det er den samme dyrlæge, der møder patienten hver gang. Anders fortæller at langt de fleste (75 %) af hypofysær betinget Cushings Diagnose og symptomer Cushings er ikke en laboratoriediagnose, fortæller Anders. Det er en klinisk diagnose, vi skal stille på patientens symptomer. Hvis man har laboratoriefund, der ikke passer med de kliniske symptomer, må man træde lidt tilbage og eventuelt vente lidt. Patienten skal måske have tid til at udvikle sig, så vi senere kan lave en re-evaluering. Det klassiske symptom, hvor ejeren melder sig på klinikken, er PU/PD der er kraftig nok til at hunden tisse på gulvet eller skal ud hele tiden. Dertil kommer øget respirations, tab af muskulatur og ændre fedtaflejringen. Diverse hudproblemer og pelstab er også almindeligt. Paraklinisk ser min op til 90 % af patienterner har forhøjelse af alkatisk fosfatase og stress leukogram. Urinen vil ofte have lav vægtfylde og der kan ses proteinuri. UPC-ratioen, kan være 3-5 – her skal man dog være opmærksom af udelukke cystitis og nyreproblemer først, fortæller Anders. Diagnostiske tests for Cushings Urin Cortisol Creatinin Ratioen UPC-ratioen er meget sensitiv og ikke særlig specifik. Det betyder at testen slår ud ofte (og næsten altid når hunden har Cushings), men også giver udslag ved andre sygdomme. Denne test er god til patienter med svage symptomer, hvor man gerne vil _udelukke_ Cushings syndrom – vil kan altså kun bruge det negative svar. Er testen positiv, skal vi videre med andres tests. ACTH-stimulationstest ACTH-stimulationstest er ikke en særligt sensitiv test, men den er mere specifik. Det betyder at en del hunde med Cushings syndrom ikke vil vise sig på denne test. Anders udfører oftest denne test på de små hunderacer, men hvis testen er negativ, kan han også finde på at gå videre med LDDS-test. Low Dose Dexamethason Suppresions-test Low dose dexamethason suppresions-test er mest anvendelig på de store hunderacer, fortæller Anders, da de oftest har den adrenale form. LDDS-testen er nemlig mest sensitiv for denne type Cushings.

PDF og noter på SIVP.dk/40 Eva Spodsberg er cherfveterinær på Djursjukhuset Helsingborg Evidensia og resident i intern medicin. Hun ser derfor mange medicinske patienter i sit daglige arbejde. Katte med hyperthyroidisme ser Eva både som First Opinion og som Second Opinion-dyrlæge. Hyperthyreoidisme kaldes også hyperthyreose og thyrotoksikose, selvom der teoretisk set er mindre forskelle I betegnelserne.     Hyppig endokrin lidelse hos katte   Eva mener at det er vigtigt at vi som dyrlæger oplyser om de muligheder, der findes. Eva fortæller at hun i flere tilfælde har oplevet ejere været skuffede over at de ikke kendte til alle de behandlingsmuligheder, der findes til katte med denne lidelse.   Kort fortalt skyldes sygdommen en overdrevet produktion af Thyrosin (T4) og trijodthyronin (T3). Det skyldes i 98 % af tilfældende hyperplasi af kirtlen eller et benignt adenom. Kun i meget sjælden tilfælde er årsagen et malignt karcinom, oplyser Eva.   Selvom sygdommen oftest forekommer hos ældre katte, kan det fra tid til anden ses hos ynger katte, fortæller Eva. Median alder ved præsentation er ca. 13 år. Dog vil kun ca. 5% vil være under 8 år ved præsentationen.   Hyperthyroidisme er en kronisk progressiv sygdom og i starten kan symptomerne være ganske subtile. Symptomerne kan endda opfattes som positive af ejeren, da den ældre kat bliver mere livlig og taber sig.   Ca. 80 % af kattene polyfage, men nogle patienter kan have en nedsat appetit. Dertil kommer øget aktivitet og restløshed.   PU/PD i moderat grad er også et almindeligt symptom og vil ses i ca. 60 % af tilfældende, fortæller Eva.   Gastrointestinale symptomer med opkast og diarre (ca. 33%) forekommer også.   Tachycardi og/eller mislyd på hjertet er også et vigtige symptomer, men det kan være svært at vurdere på en nervøs kat i klinikken. Dertil kommer at ældre katte også ofte har hjertemislyd af andre årsager.   Det er i øvrigt vigtigt i den kliniske undersøgelse at palpere efter en forstørret kirtel, mener Eva.     Diagnosen   Golden standard er fortsat at måle total T4, og i langt de fleste tilfælde vil værdien ligger over referencen. Dog vil nogle patienter tidligt i forløbet kun lige ligge i den øvre del at referencen.   I tvivlstilfælde kan man enten vente i nogle uger og gentage målingen eller man kan måle fri T4.   Ved nogle sideløbende eller konkurrerende sygdomme kan T4-målingen også være misvissende, forklarer Eva. Det er også derfor at man laver en fuld klinisk undersøgelse og samtidig tager en fuld blodprofil med biokemi og hæmogram.   En særlig gruppe at være opmærksom på er nyrepatienter med CKD (Chronic Kidney Disease).   Desuden er sekundær HCM (Hypertrofisk kardiomyopati) og sekundær hypertension på grund af den øget metabolisme almindelig, fortæller Eva.   I de biokemiske parametre kan man i langt de fleste tilfælde se en forøgelse af en eller begge leverenzymer. PCV kan også være øget.     Behandling af hyperthyreoidisme hos katte   Der findes 4 behandlingsformer, oplyser Eva: Oral tabletter, kirurgi, radio-jod-terapi og diæt.   Oral tabletterapi   Man vil typisk starte med de orale tabletter med Metimazole/Thiamazol, da man gerne vil vide hvordan katten reagerer på sin behandling inden man går videre.   Eva starter dem typisk med 1,25 mg/kat 2 gange daglig. Nogle kan klare sig med engangsdosering, men det er mere optimalt at dele de. 2,5 mg op i 2 doseringer.   Hos 90 % af patienterne vil man få en tilfredsstillende regulering af deres stofskifte og som udgangspunkt en billig løsning.   Eva anbefaler at katten i starten monitores med blodprøvekontroller med 2-3 ugers mellemrum. Vi vil nemlig gerne vide om katten kan tåle behandlingen og samtidig holde øje med nyrefunktionen.   En lille andel vil miste appetitten i starten af behandlingen og behandlingen øger risikoen for thrombocytopeni. Nogle katte udvikler desuden fasciale hudreaktioner under tabletbehandlingen, hvor de får sår og taber pelsen.   Desuden skal man være opmærksom på at katten ikke bliver hypothyroid.   Efter kaskadereglen kan man desuden overveje transdermale formuleringer eller humane præparater.   Kirurgi   Man bruger stort set ikke kirurgiske behandling af hyperthyroedisme mere. Det skyldes både et metoden er invasiv og har forskellige komblikationer. For eksempel skal man være opmærksom på ikke at fjerne parathyroidea, der ligger meget tæt på.   Desuden har nogle katte ektopisk thyroidea-væv i brystkassen, som man ikke ville kunne fjerne.   Hvis katten er ældre eller har andre sygdomme kommer der desuden hensyn særlig narkose, der måske kræver særlige kompetencer.   Radio-jod-terapi   Ved radio-jod-terapi gives katten et radioaktivt jod-præparat enten intravenøst eller subkutant. Joden sætter sig i alt det aktive væv og påvirker det lokalt. Stålingen til omgivelserne er minimale.   Af hensyn til det radioaktive stof, skal katten holdes i en specialenhed på dyrehospitalet, med særligt uddannet personale. Lige nu ligger omkostningerne i størrelsesordnen 12-15.000,-. En stor af jodbehandlede katte vil være kureret for sin sygdom.   Diæt   Ved behandling af hyperthyreoidisme med diet, gives katten særlig kost uden jod. Når katten har et underskud af jod, kan kirtlerne ikke danne den overdrevene mængde hormon.   Diætbehandling fungerer godt og efter 8 uger er de fleste af disse patienter i korrekt niveau thyreoidhormon.   Med denne behandling skal ejeren dog være meget opmærksom på at katten ikke kan komme til anden kost. Det kan altså ikke bruges til katte, der går ude. Man skal desuden være opmærksom på at der kan være en del jod i drikkevandet i nogle regioner.

Polyuria/polydipsia (PU/PD) is a common presenting complaint in small animal practice; a number of different disease states can disrupt normal physiology resulting in this problem. In this episode I discuss PU/PD with Dr Rosanne Jepson who is a Lecturer in Internal Medicine at the QMHA. Some of the things we discuss include what we mean by polyuria and polydipsia, background pathophysiology that can lead to this problem, and a rational approach to the PU/PD patient.
