Why You Should Listen:  In this episode, you will learn about chronic kidney disease and how to address the Kidneydemic with Renology. About My Guest: My guest for this episode is Dr. Robin Rose.  Robin Rose, MD began a journey into holistic healing in her teens in the mid-sixties beginning with nutrition and botanical medicine and yoga and meditation.  Over the years her journey included time living in India and working with both village doctors and healers.  She became a health food chef and inspired many to change their habits.  Before long she was enrolled in an RN program and then a family nurse practitioner program.  Aware that the education wasn't complete, she attended University of Arizona College of Medicine; while also serving as a medical student board member for the American Holistic Medical Association.  During residency, she continued learning Chinese medicine, acupuncture, osteopathy, energy medicine, and herbal medicine.  Her main practice was in Ashland, Oregon where she served a community eager to integrate many healing modalities into the conventional setting, including innovative care in the hospital.  In the past decade after her own alarming health challenges with kidney cancer and advanced kidney disease, she became agile in regenerative medical approaches to kidney care.  She created a new specialty called Renology; a new concept of "Kidney Success" not Kidney Failure.  When she discovered peptides and especially bioregulator peptides, she led the brigade to new heights of seeing success in a field that had not embraced this kind of care.  Her recent book "Renology Peptides" is a nearly 800 page text on how to achieve this renewal of health.  Her current intention is to raise awareness of the role of kidney in wellbeing and how we can all celebrate this success.  Key Takeaways: What is the purpose of the kidneys? My guest today has a long history of working and playing in some of the most rugged and remote parts of NSW. So, with a few days up his sleeve, a bit of research and the right kind of skills and experience behind him, he set off for another steep n' deep trip through Kanangra Boyd National Park, bound for Katoomba on the traditional K2K route.WARNING: The K2K route is described online as a classic 3 day trip in the southern Blue Mountains. This description was written before the fires and floods since 2019. It cannot be compared with other walks of a similar length such as the Six Foot Track, which is on well-defined management and fire trails. The K2K has always been a challenging walk that pushes the Australian Walking Track Grading System of 1-5, into an unofficial 6. That was then (before the fires) and now, it's an even more punishing route.I've written about this route before with this rescue of Chini who shared her written story.NPWS Alert re: K2K routeHeraclitus was a Greek philosopher, born around 500 BC. Now, I'm wondering if he was a bit of a bushwalker and a lover of nature, cos he's the one who said, “No man ever steps in the same river twice. For it's not the same river and he's not the same man.”He's also the one credited with saying, “the only constant is change.”Nowhere is this more evident than in nature: The turning of the seasons, the changes this brings in life cycles of plants and animals, erosion from weather, climate change, bushfires, my greying hair, you name it.And for those of us who love being out on (or off) the track, it's something that feels more obvious in recent years: there's more people, more bushfires, weather events and landslides are changing the landscapes that we love to spend time in.Learnings from this episode: Having a Personal Locator Beacon (PLB) as a critical part of your packing. The importance of having a 'place to go' within yourself (eg. meditation, mindfulness, faith, etc) when faced with a crisis, helping you remain calm and make wise decisions. The value of having past experience in similar terrain you expect to encounter. Just because a track is marked online or on apps, doesn't meant it exists in the landscape. Other than the official land manager apps (eg: NPWS App) data can be out of date and user reviews or descriptions extremely subjective. Apps like AllTrails, etc should be used in conjunction with other research and navigation skills. This week we will discuss kidney dialysis with Christina & Michael.  Christina and Michael Gilchrist have quickly become known as "BLIND CHICK LIVING" & "THE CARETAKER" but who could forget Christina's service dog MOOSE!    Christina story: early teens kidney disease diagnosed, early 20's pregnant, doctor missed toxemia, seizures which caused retina and optic nerve imploded causing blindness in total kidney failure, oldest brother donated kidney, unfortunately rejected, asked if she could do home dialysis "NO!! YOU ARE BLIND", in center dialysis 5+yrs plus a young single mother, cadaveric kidney donated, start LIVING and having fun that is where Michael slid into Christina's DM's!!, started rejecting again, INSISTED on doing Peritoneal Dialysis and would not take no for an answer, kept LIVING on PD doing dialysis all over from dry camping with solar panels even got married and did PD on honeymoon and YES! on the back of a motorcycle!!, PD stopped working started home hemo with Michaels help, Michael felt he needed support and not being able to find a support group that fit his needs he started his own.  That was the start of LIVING on dialysis!!  We have traveled all over the country advocating for dialysis and sharing how we are LIVING!!

The landscape of options for treating people with kidney failure is shifting.  It used to be that the “only” robust option in the US was dialysis.  You can listen to our prior podcast with Keren Ladin talking about patients who viewed dialysis as their only option, and structural issues that led to this point (including this takedown of for profit dialysis companies by John Oliver).  One of the problems was a lack of an alternative robust option to offer patients.  As one of our guests says, you have to offer them something viable as an alternative to dialysis. Today we interviewed Sam Gelfand, dually trained in nephrology and palliative care, Kate Sciacca, a nurse practitioner (fellowship trained in palliative care), and Josh Lakin, palliative care doc, who together with a social worker and other team members started KidneyPal at DFCI/BWH, a palliative care consult service for people with advanced kidney disease.  As a team, they provide a robust alternative to dialysis for patients with kidney failure: conservative kidney management. And “conservative,” as they note, can mean not only a “conservative approach,” as in non-invasive/less aggressive, but also an effort to “conserve” what kidney function remains. We get right down to the nitty gritty of kidney supportive care techniques they incorporate in clinic, including: Communication about the choice between dialysis and conservative kidney management: what are the tradeoffs?  Function often declines after initiating dialysis, at least among nursing home residents. Dialysis may extend life, but those “additional” days are often spent in the hospital or dialysis, away from home. Symptoms are common in both options, though more anxiety and cramping in dialysis, more pruritus and nausea in conservative kidney management.. Introducing the idea of hospice early, at the time of diagnosis with kidney failure. Listen also to our prior podcast with Melissa Wachterman on hospice and dialysis. Approaches to treating fatigue Approaches to treating pain - the second most common symptom (!) - and the answer isn't tramadol (or tramadon't) - rather think buprenorphine patch or methadone, and how to dose gabapentin and pregabalin. Fatigue is a common and debilitating symptom amongst people with kidney failure but remains under appreciated and understudied. Interventions to address fatigue in people with kidney failure requiring dialysis are lacking, highlighted by the lack of recommendations on managing fatigue in clinical practice guidelines. A Cochrane review was published in 2023 summarizing the benefits and harms of pharmacological and non-pharmacological interventions for fatigue in people on dialysis. On this podcast, the review's first author Dr Patrizia Natale joins us to discuss the study's findings.

One of the biggest challenges is identifying kidney disease in the early and middle stages, says Prabir Roy-Chaudhury. He works to emphasize the importance of simple blood and urine tests for high-risk populations for earlier diagnosis, but also strives for better treatment once kidney failure sets in. He discusses: Why his main interest is dialysis vascular access dysfunction, What are common issues, such as cardiovascular complications in kidney disease patients, How current tests like the GFR and albumin amounts are calculated and why they determine the kidney failure risk equation, and What should patients prioritize in treatment, from Vitamin D importance to new drugs and technologies. Prabir Roy-Chaudhury is a professor of medicine in the Division of Nephrology and Hypertension and co-director of UNC Kidney Center and specializes in uremic vascular biology. He brings listeners online with the basics of kidney disease, who's at risk, and what's being done to make treatment better. We need our kidneys to rid our bodies of toxic byproducts and fluid, he says. If the kidneys can't rid us of our byproducts, these uremic solutes accumulate in the blood and our systems suffer. That's why doctors turn to other ways of filtrating these out of our bodies through dialysis. That's also where Dr. Roy-Chaudhury would like to see improvement. He explains both types of dialysis—hemodialysis and peritoneal dialysis—describing their strengths and weaknesses. He adds that “my dream definitely would be for us to be in a slightly different place” with dialysis. He shares some good news towards that end: that the American Society of Nephrology has partnered with the FDA and Health and Human Services to produce public and private partnerships, such as the Kidney Health Initiative and the Kidney Innovation Accelerator, to improve these treatments.
